VA Podiatrist Discusses Stress Syndrome
According to Katie Evans, DPM, "Plantar fasciitis is by far the most common cause of pain in the heel, but a 'stress syndrome' of the bone does occur. What makes things interesting is how the pain of these two conditions can act very much the same. Our experienced 'detective' work is commonly needed to distinguish between conditions like these. The second area of the foot where a 'stress syndrome' occurs commonly is the central three metatarsal bones. These bones are long and skinny, making them susceptible to stress injuries. Pain and swelling on the top of the foot between the toes and the ankle could be a sign of a stress condition of one of these metatarsals. An otherwise healthy person can be the victim of a metatarsal 'stress syndrome' at the drop of a hat. One moment you’re walking fine and the next your foot is killing you."

"The balls and outside of the feet are two other common areas afflicted by this unkind stress syndrome. There are many reasons, such as genetics, footwear, obesity, overuse, and others that cause these stress syndromes. Once we feel confident a person has one of these conditions, we focus on the footwear, the exercise routine, work conditions and managing the inflammation. Given how much we use our feet, getting a stress syndrome to quiet down isn’t always easy. Besides the obvious goal of relieving the pain, avoiding something worse like a fracture of the bone is mission critical. We know how common it is for people to live with pain for months before seeking medical attention," says Dr. Evans.

Most Medicare Advantage Payers Accused of Fraud Allegations or Settled
To date, nearly every major insurer has been accused of or settled allegations of Medicare Advantage (MA) fraud from the federal government. Payers have been accused of exploiting the program through elaborate coding schemes that make patients appear sicker on medical records than they actually are — thereby leading to higher payments from CMS. Insurers dispute these claims. MA overpayments to payers are estimated to have cost as much as $25 billion in 2020.
Physicians told The Washington Post in June that it is common practice for payers and health systems to "data mine" a patient's medical history if that individual is covered by MA because the program pays a set amount based on patient risk.

Why You Should Consider a Private Equity Offer For Your Practice
Private equity firms typically offer a combination of immediate cash and shares of their existing business to the owner, buying the practice and folding it into a larger healthcare organization. The seller then becomes a shareholder in the combined entity and is in line for a payout upon exiting the business.

Query: Low Reimbursement
I just received reimbursement for a hallux amputation for a patient I managed while they were admitted to the hospital. $26.71 total payment. I have grown accustomed to low reimbursement rates but this seems outrageous to me. How can a surgical procedure for a patient with a bone infection pay less than an order from Panera? Is there any course of action as a practitioner?

Response: The reimbursement you stated is low, but you did not state who the insurance company is? Nor did you stated what the co-pay or co-insurance was? This information helps to determine the overall value of that reimbursement. The amputation code you used is not stated, but for a toe there are two CPT codes:
1) CPT 28820-Amputation, toe; metatarsophalangeal joint
2) CPT 28825-Amputation, toe; interphalangeal joint
Both have a “0” day global period which means any care after the amputation day is an E/M. Have you been billing E/M providing the patient with an evaluation and medical management post amputation? If not, then you need to bill those visits. There was a change January 1, 2021 to make the toe amputation codes ZERO day global. Meaning these two codes were stripped of the E/M component that used to be part of the payment process.

It’s no secret that many doctors have some very strong opinions which criticize the Medicare Advantage Plans. Low pay, providing prior authorization and then denying the claim, illegal data mining and marketing campaigns which promise the sky; these are just only a few of the issues we should be screaming about when dealing with these companies. It’s about time that CMS cracked down on the advertisements and the dollars these companies spend on those commercials. And in 2023, they will need to have CMS vet those commercials prior to allowing them to air.
A great way for CMS to get some dollars back is to charge the Medicare replacement carriers money to review their marketing campaigns at Fair Market Value and tax the revenue they pay to the media (have the insurance carrier pay the tax) for these commercials. And how about some additional transparency with how much they paid...

Yes, I recommend podiatry to patients and others and my thought process is very simple: in the 1980s, the United States Shoe industry was deregulated from government control. From that time forward, the shoes we buy are no longer standardized and buying shoes is pretty much at the risk of the consumer.
Advance forward forty years with an overall increase in sports and activities that put excessive demands on young, developing feet and lower extremities, and those who have completed three-year surgical residencies are about to receive a bonanza in forefoot, rearfoot, and ankle surgeries.
