WesternU Dean Leads TRaIN Conference on Avoiding Amputations
 
A Tuesday Rounds and Intercollegial Networking (TRaIN) Conference on the topic “Prioritizing Risk Factors to Avoid Amputation” was provided January 5, 2016 by Founding Dean, Lawrence B. Harkless. The learning objectives included (1) citing the risk factors for ulcers; (2) Listing the risk spectrum for vascular disease; (3) predicting the likelihood for limb salvage versus amputation based on the vascular risk spectrum.

Suddenly High Arch Could Mean Nerve Damage: MD, IL Podiatrists

You notice a suddenly high arch. It could be nerve damage. "Most high-arched feet are associated with some form of underlying neuromuscular condition," says Gary A. Pichney, DPM, a podiatric surgeon of The Institute for Foot and Ankle Reconstruction at Mercy Medical Center. "If someone has thinning of the arch muscles in the foot, it could be an indication of a neurological condition called Charcot-Marie-Tooth (CMT)."

An inherited disorder that damages the peripheral nerves (those outside the brain and spinal cords), CMT can also cause changes in gait, numbness in the feet, difficulty balancing, loss of muscles in the lower legs, and later on, similar symptoms in the arms and hands. See your doctor if you notice abnormalities. "For anything that’s different or changes when it comes to the foot, see your podiatrist right away," says Marlene Reid, DPM,  a foot specialist in Naperville, IL.

Brunei Podiatrist Discusses Viral Warts
 
Viral warts and ingrown toenails are among the most common conditions treated by the Podiatry Department, Raja Isteri Pengiran Anak Saleha (RIPAS) Hospital. Lim Kai Woon, a podiatrist from the department, told The Brunei Times that they see an average of 30 cases of viral warts and nine cases of ingrown toenails a week.

Lim explained that ingrown toenails refer to deformed toenails that pierce through the flesh and are caused either by genetics or trauma. “(The problem with ingrown toenails) is when it gets infected. This leads to further complications such as swelling, redness, and pain. If they (people with ingrown toenails) remove the nail, there is no guarantee that the problem won’t reoccur,” said Lim. He said that podiatrists can provide the solution to remove and permanently correct the problem by using a caustic chemical called phenol which kills the roots and prevents the nail from growing into the flesh again.

FL Podiatrist was Once a "Pop Princess"
 
Tucked away in a quiet corner of the internet is an ode to the city few Edmontonians have ever heard. It's a curious song; uploaded to SoundCloud under the moniker Edmonton Theme, it has all the trappings of the eighties, with synthesized beats, cheesy crescendos, and a boisterous horn section. The song has been swapped around on social media sites for years, but its origins have been shrouded in mystery, until now. The voice belongs to Edmonton's Annabelle Santos-Dookie, DPM, who now works as a foot doctor in Florida.

She was only 15 years old when she recorded the song back in 1995. At 18, Santos-Dookie moved to the Philippines where she became the acclaimed "pop princess" Maxine. "There was this producer from the Philippines, an older guy, and he said, 'Let's make an album.'" Her songs were played on MTV and she worked as a radio DJ and a game show host. There were cameos on network television, A-list friends, and tabloid articles. Burned out and fed up with her fame, Dookie decided to leave the red carpet behind and pursue a career in medicine. She now runs a successful podiatry practice outside Fort Lauderdale.

QUERIES (MEDICAL-LEGAL)
Query: Non-Compliant Minor Patient
 
If a minor dependent patient is non-compliant with physician instructions regarding aftercare for a surgical procedure performed or a cast application, then what is the physician recourse? Should the patient be discharged from the practice if their non-compliance was so flagrant and against medical advice that it would cause irreparable harm to themselves and possibly cause permanent disability, morbidity or sequela? If the minor dependent patient does not heed the instructions provided by the physician and repeatedly ignores the requests of their parents or guardians to follow instructions provided, then what is the best recourse? Is this grounds for discharge? If these same parents or guardians do not contact the physician regarding their minor dependent's non-compliance, then how should this be handled when the minor patient returns to the office to be seen? What if the parent or guardian of the minor patient is a patient as well, then should they be discharged from the practice?

Editor's comment: PM News does not provide legal advice. Any patient, regardless of age, can be discharged for non-compliance. The key is always to document the non-compliance and leave a paper trail. A letter should be sent to the parent or guardian via certified mail, return receipt requested. If the parent or guardian is also a patient, you have the option to discharge that patient as well if you believe that the minor's non-compliance is attributable to the parent or guardian.

Query: Meaningful Use Measure 10
 
It is my understanding that since the first two sections of Measure 10 are not applicable to podiatry - namely immunizations and syndromic surveillance - we are therefore required to participate with two registries. As far as I know, the only registry now available to podiatry is the US Wound Registry. I have signed up with this registry and have begun uploading data to them already. However, I would like to know if we really do need to participate in two registries, and if so, what other registries are available. 
 
I know that the APMA and other organizations are working at setting these up for us currently, however as far as I know, we need to participate with the registry within the first 60 days of the reporting period...and for 2016 that would be the end of February. Does anybody have any information regarding the registries and the requirement to participate in one or two of them regarding podiatry?

Response: You do not have to participate in two registries if you are using the US Wound Registry. You can both submit PQRS and meet the requirements of the 3rd Measure of Meaningful Use Objective 10 with the US Wound Registry.

Dr. Zale’s query is heartbreaking but not uncommon. If misery loves company, then Dr. Zale, you are among over 256,000 other physicians who will be experiencing the same cuts. It also demonstrates a misunderstanding of the Stimulus program and EMR use. The Meaningful Use Stimulus program was voluntary. You could choose to accept stimulus money to help offset some of your EMR and compliance cost or not accept them. But the biggest misunderstanding is to think that using an EMR in a meaningful way was optional.
 
All covered entities are required to be using an EMR program in a meaningful way. The government was very clear on this. The only option was if you want to take the stimulus money to assist you with this task. So, not “getting involved” with EMR was never an option; the penalties for not using EMR have always been there, it is just that now the penalties begin and some of the penalties will be cumulative and some will be annual until you meet meaningful use.

So are we supporting the certified pedorthists who will take away our orthotics income, but we are opposed to the Scholl orthoses because they claim to be custom, but are probably semi-custom at best that will take away our orthotics income? And why all the fuss over RFC when we're doing three-year residencies to perform rear foot and ankle surgery?

The article referenced concluded that documentation of dermatophyte infection and/or liver disease prior to treatment with oral terbinafine is not cost-effective due to the relatively low incidence of complications with this agent, as well as low cost. However, should the patient have an idiosyncratic reaction to the terbinafine (rare, but possible, and many have occurred) and require a liver transplant, or perhaps even die (very rare, but it happens), try presenting this article in your defense (if litigation ensues), and then ask the authors of the article if they considered this in their "cost-effective" model.
 
As an aside, the article did go on to say that when considering newer topical agents for onychomycosis, that it IS cost-effective to get pre-treatment confirmation that a dermatophyte is causing the infection.
