Most people are lucky to make one lasting contribution to humanity in their lifetime. For Catherine “Kitty” Hand, DPM of Cleona, she has had the good fortune to have been a positive force for the betterment of mankind in her medical practice and via her philanthropy in Lebanon County.

At a recent celebration at Lebanon Country Club to commemorate and celebrate her upcoming 100th birthday on Jan. 17, LebTown asked the soon-to-be centenarian what she believes her biggest legacy to the Lebanon Valley is. “The Palmyra Public Library,” Dr. Hand, who founded the library in the 1950s, said, without missing a beat. “My dream became a reality. Hand, who practiced podiatry for 42 years in Palmyra, not only put her patients on firm footing but also at the library, which grew from a few thousand resources when founded to an asset worth millions of dollars today.

5 Practical Tips to Reduce Denials, Improve Cash Flow
Technology is often the go-to solution for healthcare administrative workflow woes, including billing. Still, it’s just one of several practical steps that can deliver tangible impact, many of which require little to no capital investment. Here are five of the most effective.
2. Use Trained and Credentialed Coders
Slow, inaccurate, or incomplete coding contributes to delayed and denied claims. It’s a wrench in billing workflows that the practice may not even be aware of, particularly when physicians are tasked with self-managing their coding or the practice lumps coding responsibilities in with other adjacent areas, such as registration. Placing coding responsibilities in the hands of non-coders can quietly drain revenue from the practices in several ways:

Denials and Underpayments: Coding errors are responsible for up to 22% of claim denials in private practices, frequently due to mismatched codes, lack of medical necessity, or payer-specific requirements.

Delayed Reimbursements: Incorrect coding leads to delayed payments, which increases days in A/R—contributing to the creeping percentage of aged receivables over 90 days—disrupting cash flow and forcing staff time to be diverted from core responsibilities to re-working claims and managing denials.

Lessons Learned From My First Time in the Operating Room
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
4. Calm Yourself Down With a Deep Breath - Before scrubbing in, take a deep breath to ground yourself. The OR can be chaotic, especially for students, and this practice helps you enter the room with a sense of calm and readiness, reducing the likelihood of mistakes. It has made a huge difference for me! Do the same thing before you walk into a patient's room in clinic.
5. Pay Attention and Anticipate - Always be prepared for the next step, have a lap or the bovie ready, ask for scissors when the resident is suturing. While it's important to anticipate what's needed, avoid overstepping your role. If you are a third or fourth-year student, you won't be the one performing complex skin-to-skin procedures, so stay within your boundaries. Do not take things out of people’s hands.

As a student, I was on a rotation at a hospital and excited to observe one of my first forefoot cases. I asked a supervising nurse which OR the podiatry case was in. She told me, then said something to the effect "you work with feet...oh how disgusting!" On my way to the OR, I passed by another OR and saw several doctors and nurses working between the legs of an extremely obese patient. I then walked into the OR where I was about to observe, I saw a nicely scrubbed, very clean, exposed forefoot. Lesson learned: There are a lot worse things to be working with than feet!

The commentary regarding the preparation of a resident for surgery is important. However, there is the other side of the equation: the role of the attending. Ultimately, the goal is to graduate a resident that will have the confidence to perform surgery independently. We have a responsibility to the resident as they have to the attending. I have devoted my life to the education of students and residents and would like to share some thoughts.
A pre-operative discussion is critical. What factors did the attending consider in making the decision to proceed with surgery? What are the intended goals of the surgery? In what manner is the surgery planned to be performed? What are the intended goals of the resident for the case? Are you prepared to offer constructive...

After reading Dr. Zlotoff's memorable experience, I was reminded of mine that I have not thought about in many years, but still break out in a cold sweat re-living it.
As a first-year residents at the California Podiatry Hospital in San Francisco, we were required to take call and stay overnight at the hospital. Back in the days when dinosaurs roamed the streets, patients having surgery checked into the hospital the night before and we would work them up before their surgery the next day. My surgery schedule had me in the operating room with an attending who was very generous with residents. If the case was B/L, the resident was guaranteed to do a foot. The case I would be scrubbing was a B/L bunionectomy. This was at the beginning of my program and I was very excited to add to my very limited case numbers.
