NY Podiatrist Discusses Ingrown Toenails
While an ingrown nail typically impacts the big toe, it can occur on any of your toes. According to Ammar Al Rubaiay, DPM, a podiatrist with Northwell Health, it happens when one of the nail edges, either the inside or outside of the nail, digs against the skin on the edge of the nail and causes a small wound. “That small wound usually has difficulty healing because that same sharp nail edge keeps re-opening the wound,” he says.

Dr. Ammar Al Rubaiay |
"If you have any underlying health conditions that affect your immune system or circulation, you might be more likely to get an infection from an ingrown toenail. If that happens, you’ll probably notice pain, swelling, redness, and even some pus draining from the sore spot—an indication it’s time to see a podiatrist. In those instances, treatment might involve antibiotics or even removing part or all of the nail," says Dr. Al Rubaiay.

MEDICARE ADVANTAGE |
Medicare Advantage Programs Receive Boost While Physicians Wait: AMA
“CMS has announced that Medicare Advantage (MA) plans are expected to receive an average payment increase of 4.33% from 2025 to 2026. Meanwhile, physicians treating Medicare patients are facing their fifth consecutive year of payment cuts — this time by 2.8% — despite practice costs rising by 3.5% according to the Medicare Economic Index.
So, while MA plans receive an increase beyond the expected healthcare inflation rate, Congress not only failed to provide a physician payment update but allowed a new round of cuts at the end of the lame duck. It's unbelievable they're giving insurance companies that had record profits an increase while at the same time cutting payment to physician practices that are struggling to survive. This contrast highlights the urgent need for Congress to prioritize linking payment to physician practices to the cost of providing care. Otherwise, with or without MA plans, patient access will suffer if physicians close their practices. A new Congress is meeting — it’s time for a new approach to physician payment reform.”

PRACTICE MANAGEMENT TIP OF THE DAY |
Areas of Financial Waste to Cut at Your Practice
Here are a few ways to reduce financial waste, according to practice management and accounting professionals from across the country:
1. Cut Back on Supplies
"Most medical offices tend to order supplies in huge, unnecessary quantities; anything and everything from printer paper to syringes. It happens because of a lack of communication. The doctor or office manager tells an employee to stock something...[and] tells another person to stock the same things. I can't tell you how many times I've seen double orders. You need a system of checks and balances and have a supplies checklist," says Thomas Moore, CEO of healthcare management firm The Caduceus Group, Atlanta, GA

QUERIES (NON-CLINICAL) |
Query: Authoritative Literature on Barefoot Walking
I am curious as to what the "literature" says about barefoot walking. Most of what I have seen appears to be doctors' "opinions". Please show me the authoritative literature which backs up either positive or negative effects for barefoot walking.
Don R Blum, DPM, JD, Dallas, TX

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 |
RE: Start with Why
From: Mark Hinkes, DPM
I have just finished reading Start with Why by Simon Sinek. He also has a TED lecture. The book has changed my life....and I recommend it to anyone who is looking for their path forward. As I read all of the comments concerning the fate of podiatry as a profession, the thought has come to me... The general public does not know the "Why" of podiatry, and our profession has failed to explain that to the public.
We have been telling people the "How" it could benefit their health and "What" we can do for them but this approach usually fails because we have not aligned people's beliefs to those of our profession. A better approach might be to explain "Why" podiatry is the best profession to provide foot health services. What is our Why? Is it our education? Is it the research we do? Is it the post-graduate training that every student receives? Is it the special tools we use?
What is our "Why"? I challenge those reading this to share what they believe is the "Why" of podiatry.
Mark Hinkes, DPM, Nashville, TN

|
RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Robert Scott Steinberg, DPM
Podiatrists willing to share their offices with colleagues who lost theirs and possibly their homes should contact the California Podiatric Medical Association (CPMA). Podiatrists who lost their offices should call their office phone company to forward the number to their mobile phone. Other associations might want to consider making donations to CPMA so they can, in turn, provide loans to affected practices.
Robert Scott Steinberg, DPM, Schaumburg, IL
|
|
RESPONSES/COMMENTS (NON-CLINICAL) - PART 3 |
From: Vincent Marino, DPM
I may be reading this incorrectly, but it seems to me with the enactment of this new law, the medical community will no longer be able to go after patients for unpaid medical bills since collection companies will no longer be able to ding the patient’s credit report for outstanding bills. Good luck collecting balances. I suggest everyone start collecting the day of service even if you have to refund the patient.
Vincent Marino, DPM, Novato, CA

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) |
From: H. David Gottlieb, DPM
I for one, am always uncomfortable whenever I walk around my house without shoes or sandals on. I have seen so many patients with foreign bodies in their foot over the last 42 years that I am always nervous when walking with bare or socked feet. And I NEVER suggest that my patients do so.
H. David Gottlieb, DPM, Baltimore, MD
