NY Podiatrist Offers Sneaker Buying Advice

Are your sneakers in bad shape? Some help is on the way. Dr. Stacey Sarmiento is a podiatrist. She said shopping for sneakers does not need to be overwhelming. Sarmiento recommends a good running sneaker for most workouts. The price is right ranging from $40 to $70.

"What I always tell patients, make sure you can't squeeze this back heel coming together. Make sure it's nice and stiff and you can't fold the shoe in half. The more stiff the sneaker is, the more the sneaker takes the shock of the activity and not transferring it into the foot," said Sarmiento. The best time to try on sneakers or any shoe is later in the day. Also, make sure you wear your workout socks.

Two NY Podiatrists Sentenced for $10 Million Medicare Fraud

Two podiatrists who ran several New York clinics that primarily served indigent and elderly patients were each sentenced yesterday to more than five years in prison - by a judge who noted that neither showed remorse for their multimillion-dollar scam. U.S. District Judge Kimba Wood imposed terms of five years and three months and ordered Michael Brumer of Long Island and Lawrence Klein of Brooklyn to pay a total of $29,000 in fines. Brumer and Klein, both 61, pleaded guilty in 2003 to fraud that the government said cost victims more than $10 million.

They devised a system to defraud Medicare and private insurers while overseeing local clinics from 1986 to 2000 - charging for surgeries and other expensive procedures after performing only routine foot care. Assistant U.S. Attorney Marc Weinstein said their misdeeds also resulted in the conviction of 15 people on related felony charges.

House Passes Drug Negotiations Bill

The House voted 255-170 to require HHS to negotiate with drugmakers over prices for prescription drugs covered by Medicare Part D. The bill, sponsored primarily by Rep. John Dingell (D-MI), also requires HHS to report to Congress by June 1 and every six months after on the results of negotiations. A similar bill is pending in the Senate, although it requires negotiations only when there is no competition for a drug, the drug was created with substantial taxpayer support or a Part D plan requests help.

The Senate is expected to take longer to consider the matter than the House. President Bush has said he will veto legislation that requires negotiations. Under the law that created the Part D benefit, the federal government is prohibited from directly negotiating with drug makers. Instead, private insurers that offer the benefit or pharmacy benefits managers negotiate for price discounts. –

Query: H&P Precedents

I have just been denied the privilege at my hospital to perform H&P's. The explanation that I was given was that they were going to wait for a precedent to be set by other hospitals. Obviously, if all hospitals did this, nothing would ever get done. If there is anyone currently doing H&P's at there hospital, could they please e-mail me.

The more examples of other hospitals supporting podiatrists doing H&P's, the easier it will be to convince my hospital that they should allow it.

I have seen and treated two patients in the last six months that have presented with calcaneal fractures resulting from ESWT. Unfortunately, the treating podiatrist never mentioned that this is a possible complication nor did he follow up on the patient's persistent complaint of pain.
