PODIATRISTS IN THE NEWS
MI Podiatrist Discusses Risks Associated with Abnormal Foot Types

According to Conway T. McLean, DPM, "There are two basic categories of pathologic foot type: supinated or pronated. Apparently, only about 20 percent of the population has a normal foot type, one that is well-aligned and functions properly. You can determine your foot type by looking at your foot imprint after a shower. If there is minimal indentation at the arch, it indicates a pronated or lower arched foot. Conversely, if there is a large indentation, you have a more supinated or higher arched foot. With a supinated foot type, you tend to put extra pressure on the outer part of your feet while walking or running. Because of their bony architecture, this type is prone to ankle instability and sprains."

Dr. Conway McLean

"The motion of pronation, visualized as a lowering of the arch, makes one more prone to hammertoes and bunions, knee degeneration, and chronic low back pain. And those are only the headliners, some of the main acts in the litany of conditions resulting from these abnormalities in stance and gait. Pinched nerves in the feet (neuromas), shin splints, corns, chronic rearfoot joint instability, all are significantly more common in the pronated foot type," says Dr. McLean.

INTERNATIONAL PODIATRISTS IN THE NEWS
Aussie Podiatrist Raises Awareness of Early Treatment of Onychomycosis

Podiatrists are encouraging people to be conscious of the risk of picking up toenail fungal infections. Some podiatrists say they have seen an increase in toenail fungal infections linked to pedicures at nail salons. The Australian Podiatry Association says there are steps people can take to avoid fungal infection, including never sharing nail clippers or footwear with others.

Dr. Tracey Meyer

Dr. Tracey Meyer is a podiatrist with Irymple Foot Clinic near Mildura, who has more than 30 years' experience. She said the condition, known as onychomycosis or tinea unguium, was extremely common. "The sooner people get it looked at by a podiatrist, the quicker we can get it under control. Infections can be particularly difficult to get rid of, especially when they progress and get into the root of the nail," Dr. Meyer said.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Tips for Preparing for New Opportunities

We’ve seen a lot of positive movement to address some key issues leading to healthcare attrition and burnout. For example, there is stronger encouragement for work-life balance, more staff, mental health resources, and overall better support systems for physicians. However, there is still work to do and some considerations at the onset of a career move. I encourage physicians to look at these five areas before they consider a new role:

5. Create a sense of community

Douglas Farrago notes that studies show that physician retention is improved when doctors feel that they are part of a community, which has become increasingly difficult as doctors are siloed and isolated from each other in daily practice. Practices can create a sense of community by creating physician-only spaces, like the traditional doctors’ lounge, which Farrago believes should be exclusively for physicians. “Non-physicians can have their own space, but doctors need a place where they can talk to each other.” In addition, practices can help create community by providing meeting opportunities offsite and by encouraging physician membership in local county medical societies, including funding membership dues.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Los Angeles Fire Devastation
From: Richard A. Simmons, DPM

We have all watched the devastation of many communities of Los Angeles. Unfortunately, this means the total loss for many of our colleagues. This is one of those unrecoverable losses for almost a generation of a medical practice. I hope that there is some kind of GoFundMe or other plan set up for our podiatric brothers and sisters.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Federal Watchdog Agency Bans Medical Debt from Credit Reports
From: Keith Gurnick, DPM

Rohit Chopra, President Biden's Consumer Financial Protection Bureau director, has  rolled out another controversial new regulation. This week, the watchdog agency finalized rules that bar unpaid medical bills from being included in consumer credit reports, thus removing another existing tool (or threat) that doctor's offices and collection agencies have used for years to get paid from patients who owe money after their claims have been processed by their insurance carriers.

"People who get sick shouldn’t have their financial future upended,” Chopra said in a statement. “The CFPB’s final rule will close a special carve-out that has allowed debt collectors to abuse the credit reporting system to coerce people into paying medical bills they may not even owe.”

Republicans have hinted they may try to undo some of Chopra’s moves using the Congressional Review Act, which allows lawmakers and the president to spike recently completed rules. What’s more, the law bans agencies from enacting new regulations that are “substantially the same” as one Congress has reversed — meaning they could, in theory, be permanently repealed.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3
From: Allen M. Jacobs, DPM

The reasons for a declining applicant pool to the colleges of podiatric medicine are multifactorial. There is need for some reality discussion. Years ago, there were somewhat limited alternative options for those who desired to enter healthcare as a provider, but were, for whatever reason, not accepted into medical school. There were few island medical schools, few colleges of osteopathic medicine. There were no PAs or NPs. There was always dentistry, physical therapy, optometry, chiropractic medicine. Competition to medical school is intense, and there are many excellent individuals who fail to succeed in admission to medical college in the United States.

It is not a matter of failure, it's a matter of numbers. Those who argue that diminishing reimbursement for services is the reason for declining applicants should ask why this factor has not reduced applications to...

RESPONSES/COMMENTS (PODIATRISTS IN THE NEWS)
From: Bruce I Kaczander, DPM

When I was in practice, I would, daily, advise my elderly patients that walking barefoot at home, increases their fall risk tenfold. That has been proven multiple times in a variety of studies, too numerous to mention here. Keeping a shoe on at home is critical, reminding them that  a slipper is not a shoe!

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Lawrence Rubin, DPM

Having been a podiatrist and loving it for 66 years, it saddens me to read in PM News that only 42.86% of today's podiatrists are very satisfied with their choice of podiatry as a profession. I believe it would be enlightening to have comments from those who are disappointed in aspects of the profession to express their reasons, and what, if any, recommendations they have for positive change.

I truly believe that podiatry can solve all the negativity of its present problems if we all unite and work together to overcome this present high state of dissatisfaction.
