Heroic 10 Year Old Daughter Saves CA Podiatrist's Life
 
Young Sammye Collis was wise and cool-headed beyond her years when confronted with the sight of her father, Dr. Sheldon Collis, collapsed on the floor of their home. Doctors and nurses at Eisenhower Medical Center in Rancho Mirage say the 10-year-old girl’s fast action in alerting her mother to call 911 and then working with paramedics to detail her father’s symptoms from the massive stroke he’d suffered were nothing short of heroic.

“You saved his life; you really did,” Sue Effinger, a registered nurse and director of the hospital’s stroke unit, told Sammye as she sat with her dad this week at the hospital. Collis a podiatrist, was rushed to the hospital where he was given a drug called tissue plasminogen activator — commonly known as TPA — which can reverse the effects of a stroke.

Diabetics have more problems than non-diabetics says Dr. Joseph Borreggine. "Diabetics suffer from circulatory and neurological problems, such as losing sensation in their feet. They also tend to heal wounds more slowly. When blood sugar increases, so do infections," says Borreggine.

"Diabetics typically present to podiatry offices for what we call 'at-risk' foot care. You might think that cutting calluses and trimming nails are not important things, but to the diabetic these are very important things. We are able to provide palliative care, and we are able to assess any major problems that are occuring with that patient over time. We are able to stave off any complications that might occur, and to identify any structural deformities which may lead to complications down the road," says Borreggine.

Half of Docs May Not Attest to Stage 2 MU in 2015
 
Fifty-five percent of physicians do not plan to attest to Stage 2 Meaningful Use in 2015, according to a survey of almost 2,000 members of SERMO, a social network for physicians. Doctors cited “patient engagement, lack of workflow usability, and excessive time consumption” as reasons for not attesting to Stage 2 MU this year. The survey results reflect the struggles that physicians have been experiencing in meeting Stage 2 requirements.
 
According to the Centers for Medicare and Medicaid Services, only 4 percent (16,455) of eligible professionals have attested to Stage 2 MU as of Dec. 1. Though EPs have until the end of February to attest for calendar year 2014, attestation numbers have been anemic, and little progress is expected by the end of next month. Adding to the bad news for physicians, CMS announced last month that more than 50 percent of EPs—more than 257,000—will face financial penalties under the MU program in 2015.

I have used Dragon for 12 years, most recently Dragon Medical with Practice Fusion for the past few years. Dictating with Dragon directly into the Practice Fusion field works REASONABLY well, but not optimal in terms of commands and formatting. Also, the accuracy is somewhat affected, as is separating sentences, etc. It is much smoother to use the Dragon dictation box, then quickly transfer into the EMR field.  
 
Medical Dragon is highly accurate, fairly quickly trainable, BUT you should use a very accurate microphone - handheld or headset type, retrain your mistakes as phrases, and use at least 8-12 G Ram and Intel 5 or higher processor. The time saver in that you have a great complete unique note if you use templates with bookmarks WITHIN Dragon, then transfer rather than using the cumbersome - my opinion - templating option within the latest Practice Fusion.

The last Dragon Naturally Speaking that could be used with any Practice Fusion is Version 11.5. The newer ones have been constructed to not work with EMRs so that Dragon can sell the medical version for over $1,000 as compared to $99 for the non-medical version. I have used the non-medical version for years with great results.

I use Dragon with PF. I use the Medical Professional edition. You will want a powerful/fast computer, preferably an I7 intel chip, fourth generation processor, with 16 gig of memory in a 64 bit system of Windows. Adding devices from the website knowbrainer.com is not mandatory, but adds to the system's functionality. It is absolutely mandatory you have a very fast Internet connection - fiber optic is best. You will lose some functionality of Dragon, but in my opinion, that is more than made up for by the ease and speed of using the two together. All-in-all, it is a good combination.

RE: Eliminate Medicare Deductibles

As we start a new year, so starts the headaches of Medicare deductibles. Any provider who deals with Medicare knows all of the scenarios. I must be missing the boat because I don't understand why our government can't deduct the Medicare deductible amount from the beneficiaries social security check; even deducting three equal amounts from the first three checks. Some might argue that it's unfair because some beneficiaries do not use up their deductible in the year. Oh well, that is the price one pays for the privilege of having Medicare. This concept would save millions if not billions for Medicare, along with saving both time and money for providers and beneficiaries. What am I missing?

I found Dr Reese' s posting interesting in the respect that he said that Medicare was the worst paying insurance in the early days. On the contrary, from one who was there at the beginning of Medicare in 1965. Medicare paid whatever we charged in the early days. Medical insurance was just becoming in vogue and the great majority of patients paid the day of service. Then insurance started to pay very well, probably more than we were worth. 
 
Now, the reversal will be true, and patients will have to pay their share upfront in the form of deductibles, and we will get our fees paid up-front. There are few cases that go above $6,000, so I think we will survive.

Although it saddens me to say it, I agree that many of our colleagues are filing for RFC. I have also had patients tell me that their RFC is covered by certain DPMs. My response to this is to give them a written copy of Medicare's guidelines for RFC coverage and tell them that their situation may have changed since they last saw a podiatrist, but at this time, I do not find that they meet that criteria. I have worked with patients long enough to realize that the story they tell me may not be accurate (although it is always the same four DPMs who come up). Unfortunately, I have a personal experience that seems to validate the comments.
 
Our state has an emergency coverage plan where many of us provide pro bono coverage for colleagues who must be out of their office for extended times. A number of years ago, I was working in such a practice. I found patients whose chart notes indicated that they had no pulses, but I could easily palpate pulses. In other patients, I attempted to obtain some kind of history justifying coverage and was told by the patient that "I just come here to have my nails cut and Medicare always takes care of it." The frightening thing to me is that this doctor clearly must believe the practices are acceptable or would not have been comfortable with others covering. Later, I attempted to discuss the issue with the DPM, but was not effective.
