FL Podiatrist to Test Walking as a Way to Improve Overall Health

Dr. Gene Pascarella knows all about walking. He's a podiatrist who treats foot problems every day. But now he's initiated a national study to test walking as a way to improve patients' overall health. According to Dr. Pascarella, “Now it's time to get out there and get some exercise, time to lose weight, time to lead a healthier lifestyle.”

Patients will begin the program with a brief check up, blood work, and medical history. Then they will strap on a pedometer from morning to night, to record how many steps he ordinarily takes in a day. After two weeks establishing a baseline, Dr. Pascarella will give them a prescription for walking; how far, how fast, and how often. He'll monitor their progress for 48 weeks, comparing their health markers to patients enrolled in the walking study at 15 sites from Florida to California. That database will give doctors scientifically-based answers to questions about the benefits of good old fashioned walking.

Preventive Care Not Received by Millions of U.S. Residents: Reports

Millions of U.S. residents do not receive necessary preventive care, according to two reports released on Thursday by the Agency for Healthcare Research and Quality, the AP/Arizona Daily Star reports. According to the fourth annual national healthcare quality and disparities reports, although the quality of the healthcare system continues to improve overall, lower-income and minority residents continue to receive a lower quality of care than other residents.

In addition, the reports found that uninsured residents received the lowest quality of care and had the most problems with access. The reports also found that many residents overall -- with the majority low income, minority or uninsured -- do not receive necessary examinations, tests and advice from physicians that can prevent or limit the progression of asthma, cancer, diabetes and obesity.

Query: NPI Number Question

What are the rules regarding the National Provider Identifier (NPI) number? I have a podiatry corporation - one doctor, two office locations. Do I need a NPI # for each office, or just one? The CMS and NPI phone people are giving me different information.

Response: According to NHIC,

- Each doctor needs to have his/her own NPI number.

- Your "group" will need a NPI number for both locations as long as the tax ID number is the same, and the checks go to a single location/main office

- If you want the checks to be separate (such that each office bills and collects monies individually), then each office would need seperate NPI numbers.

For DMERC, you can have the same or a separate NPI number.

How about a little good news? In Springfield, IL, one of our large hospitals has taken the lead in allowing podiatrists to do H&Ps. Luckily, we were in the process of rewriting the entire by-laws for the hospital. We had a meeting with the Surgery Department Head, several internists and orthopedic surgeons. They all voiced backing qualified DPMs to do H&Ps. We will set up H&P privilege criteria, similar to our surgical privilege criteria that we use now, to grant different H&P privilege levels to those that qualify.

One has to wonder what genius at CMS designed the new CMS 08/06 form?
Most window envelopes have the window on the left side. Of course, the geniuses at CMS developed a form which requires the address to be placed only on the right side of the form. Thus a whole new demand for right side window envelopes has now been created. These envelopes seem to be a bit more difficult to come by, and you guessed it, more
expensive than their left-handed counterparts.

I guess the CMS administration has not read the Paperwork Reduction Act nor are they tree-friendly. CMS also figured out a way for us to spend our raise (oh I forgot we didn't get a raise, we just didnt get a reduction).
