TX Podiatrist Provides Winter Footcare Tips

Winter is here, which means cold and wet weather. According to podiatrist Dr. Robert Boudreau, "Shoes have to be warm and protective of our feet."

Skin needs to be protected as well. "In the winter months, people's skin tends to become dry. We want to protect it with a good moisturizer such as Vaseline or other foot care products to keep it moist and to prevent it from cracking."

FL Podiatrist Uses SteriShoe to Maintain Fungus-Free Nails

Dr. Eric Weinstein, a podiatrist, recently cleared the fungus nails of patient Isabella Jamis-Giraldo using a laser treatment, but she was worried that the condition would come back. "Once we get rid of the fungus with the laser, then we have to do a little bit of maintenance and that means perhaps a topical on the nails to act as a protective barrier, to not get re-infected, but more importantly, we have to address the shoe which the toes sit in on a day-to-day basis," said Weinstein.

Weinstein suggested Jamis-Giraldo buy a SteriShoe, which emits an ultraviolet light that kills fungus or bacteria that's in the shoes. "It also helps with foot odor because the bacteria in shoes are a contributing factor to foot odor, so the SteriShoe basically sterilizes the inside of your shoe," said Weinstein.

NJ Podiatrist Suspended After Arrest in Alleged Fondling Incident

A Bergen County doctor’s license has been suspended after he was arrested and charged with allegedly fondling a patient's breasts during a routine podiatry exam, authorities said Thursday. Dr. Frederick M. Weintraub, a podiatrist with a practice in Northvale, had his license suspended by the state Board of Medical Examiners in late December. Weintraub also allegedly made inappropriate sexual comments to staff working at assisted-living residences in North Jersey and touched the breasts of a staff member.

Query: PQRS Measures

My doctor wanted to know what measures people are finding that they are able to use for 2013? Have we been in a cave because we aren't finding Measure 126 and 127? We are able to find 163. Is this the only measure that podiatrist are going to be able to use?

Response: Measures 126 and 127 are part of the 2013 PQRS program. From the Medicare measures specification manual for PQRS 2013:

Measure 126: Diabetes Mellitus: Diabetic Foot and Ankle Care, Peripheral Neuropathy - Neurological Evaluation (C, R p. 274)

Measure 127: Diabetes Mellitus: Diabetic Foot and Ankle Care, Ulcer Prevention – Evaluation of Footwear (C, R p. 277)

If you are referring to the quality measures in meaningful use, they are not part of that program.

RE: Confusion Over New AFO Height Requirements (Paul Kesselman, DPM)

I read with interest Dr. Kesselman's post regarding the new height requirements for AFOs. We are in complete agreement with Dr. Kesselman's statement regarding the ultimate responsibility belonging to the party who bills Medicare. We also believe that the new policy article makes little clinical sense and should be reconsidered in light of a voluminous amount of scientific literature supporting the efficacy of the braces commonly used in podiatry, and orthotics and prosthetics today. We are being asked to operate in a bizarre environment where two different Medicare contractors are stating completely opposite positions about how to apply the new policies. If that weren’t confusing enough, there are two different policy articles which are currently valid which contain contradictory information.

Dr. Kesselman’s post might have given PM News readers the sense that...

I don't have a defibrillator in my office, but I have performed CPR successfully twice. Both patients had cardiac arrests. In both incidents, I had my office manager call 911 while I was pumping away. Both patients survived, but had to be completely stabilized in the cardiac intensive care unit of our local hospital.

I have been reflecting on the notion that you could load your 1st ray cases with cheilectomies (or any single procedure) and think that you have enough diversity to sit for the exam. I just want to share a bit of clarification.

I presented my cases two years ago, and cheilectomies were not included as part of acceptable 1st ray cases, then. So, with regard to your gripe of suddenly having the rules changed, your complaint is really without merit. Also, ABPS is looking for...

Hopefully, Dr. Sasiene realizes that not all podiatrists accept insurance. I have practiced for 23 years in a significantly podiatrically-challenged area and have a mixed practice with about 50% coming from surgeries. My attitude and practice have not changed. It is very simple. As podiatric physicians, the agreement we make to care for patients' feet is between us and them. The responsibility to pay me in that relationship does not reside with an insurance company. It is the individual podiatrist's choice to accept insurance as an intermediary in that relationship.

Will you see more patients by accepting insurance? —of course! Will you make more money? Years ago, I would say "of course", but...

I have dealt with the residency shortage issue firsthand. A couple of years ago, a recent graduate contacted me stating she had matched for a residency with the Army, however because she had skin cancer (which was successfully removed 4 years prior), she did not qualify for the program since the Army required her to be cancer-free for 5 years. In her graduating year, 20 students did not get a residency, and the following year was even worse, making it nearly impossible for her to match. It would be like trying to get drafted in the NFL after being out of college for a year. When she contacted me, she was working for a grain company for $12 per hour with her student loans bearing down on her like a freight train.

Since our group has 4 doctors and 4 clinics, I felt we would be able to generate enough cases to qualify for...
