PODIATRISTS IN THE NEWS
Choose Comfort Over High Fashion: NY Podiatrist

Joanie Galestro loves wearing high heels so much, she sports a tattoo of one on her ankle and has a collection of some 300 shoe figurines in her Great Kills home. She particularly favors stilettos with heels rising up to four inches high. The result? She’s seeing New Dorp podiatrist Dr. Cindy Resnick for treatment of blisters that formed on the sides of her big toes from the shoes rubbing up against them.

Crowding of toes inside a tight toebox often aggravates bunions and hammertoes, and can cause blisters and ingrown toenails from shoe irritation, explained Dr. Resnick. She advises women to stick with the "most comfortable shoes they have" in the aftermath of the holidays, suggesting a good running shoe that’s roomy and well-padded. And if possible, wear trouser socks, which offer extra cushioning.

AT THE COLLEGES
NY Podiatrist Elected to NYCPM Board of Trustees

Kathleen Reilly Fallon, DPM, a Board Certified Wound Care Specialist and Foot & Ankle Specialist, has been elected by unanimous vote to the Board of Trustees of the New York College of Podiatric Medicine at the most recent meeting of the Board. Dr. Reilly obtained her DPM degree in 1994 from the New York College of Podiatric Medicine, and went on for further medical studies at New York Medical College.

OUTSIDE INTERESTS
CA Podiatrist Receives Grammy Nomination for Best Children's Album

This could be a banner year for San Jose resident David Sharpe, whose third album, "Are We There Yet?" has been nominated for Best Children's Album at the 54th annual Grammy Awards next month. "This is like a dream come true for me," said Sharpe, perhaps better known musically as PapaHugs.

Dr. David Sharpe

All three albums -- including 2006's "All the Things You Dream," which was inspired by Sharpe's grandchildren, and 2008's "A Great Day for You and Me" were recorded at Open Path Music Studio in Willow Glen. A Grammy nomination could make a guy think about quitting his day job, but a lot of Bay Area feet hope that doesn't happen: Sharpe has had a podiatry practice for more than 30 years and was a founding member of the division of podiatric surgery at Good Samaritan Hospital in San Jose.

MEDICARE NEWS
CMS: EHR Incentive Payments Total $2.5 Billion

Total federal incentive payments for the use of electronic health-record systems have soared to just over $2.5 billion, according to the CMS. Robert Tagalicod, director of the Office of eHealth Standards and Services at the CMS, joined with staffers Robert Anthony and Jessica Kahn in delivering the news to the ONC's Health IT Policy Committee this morning.

Tagalicod said the preliminary December numbers don't tell the full story for the first year of the dual CMS programs under Medicare and Medicaid, because so-called eligible professionals still have until February to attest that they have met meaningful-use criteria during calendar year 2011 as required under the Medicare portion of the program.

QUERIES (EMR)
Query: Attestation Problem

I am in the process of attesting for the CMS EHR incentive program. I have successfully completed 15 core and 5 menu criteria. I have come to a roadblock to completion trying to satisfy the CQM requirement for 9 criteria. The only one appropriate for podiatrists that I could see is the diabetic foot exam. Any advice from those who have completed the attest process and received their incentive check would be appreciated.

Query: Alternate Source for the Cryoprobe Filters

Does anyone know the specifications of the Cryoprobe filters or an alternate source for the Cryoprobe filters other than the Cryoprobe company?

Perfect indication! Cimetidine dose is 25 mg/kg/day in divided doses. Elixir available as 300 mg/tsp. Therapy duration varies between 3-12 weeks and is often dependent upon pre-therapy condition duration.

RESPONSES/COMMENTS (NON-CLINICAL)
From: G. Stephen Gill, DPM, MBA

A hospital can bill ala carte for services. ASCs get a flat fee inclusive, except for the anesthesia charges. Example: an inter-operative C-arm x-ray read and noted by the surgeon (DPM/MD) and recorded in the record is inclusive to the flat ASC fee. Taken in the PO area by a tech, the film must be read by a hospital radiologist and the cost deducted from the ASC flat reimbursment for the procedure.

In our ASC, we must use Synthes implants, etc., or other preferences must meet the Synthes price per unit. Expect flack if a surgeon demands otherwise (it reduces ASC profit margin, sometimes substantially).  Waste not, want not: think, plan, anticipate, and don't make exceptions!

Regarding the quote from Dr. Otiko about the 3% tetracycline topical called Diabecline, please correct me if I am wrong, but topical antibiotics don't heal diabetic ulcers. MRSA is sensitive to tetracycline, but there are 3 others with better coverage.

RESPONSES/COMMENTS (NEWS STORIES ) - PART 2
From: Leonard Kuzmicki, DPM

Couldn't it be presumed that if a patient decides to write online about an experience at a physician's office, they are effectively foregoing their right to privacy? As long as the patient feels free enough to disclose that they received care from a particular provider, why could it not be discussed as long as the reason for their visit isn't disclosed?  It seems that this would be similar to a patient having another family member or friend present during a visit - we presume that we may fully discuss the patient's case with this third person present.
