MO Podiatrist Discusses Neuropathy in the Feet
“Neuropathy refers to a process causing inflammation of the nerves, leading to symptoms such as numbness, tingling, and sometimes burning sensations,” explains Timothy Oldani, DPM, a podiatrist at Missouri Foot and Ankle. “When it involves the feet, it is more specifically described as peripheral neuropathy. The most common causes of peripheral neuropathy are diabetes, vitamin deficiencies such as B12, metabolic disorders such as thyroid disease, and alcoholism.

Neuropathy can cause hypersensitivity—like experiencing pain even with the lightest touch from your socks and shoes, says Dr. Oldani. “On the other end of the spectrum, some people with peripheral neuropathy will not be able to appreciate light touch, pain, or temperature, putting them at risk for infections and wounds,” he adds. Sometimes, this diminished sensation can also lead to balance issues, making it difficult to walk.

Query: Medicare Refusal to Pay for TMA
I performed 2 debridements (5 days apart) using codes 28122, 28003, 28110, 28820, 20240 of a foot just under 2 months prior to billing a TMA 28805. The patient is diabetic. Medicare is refusing to pay for the TMA, saying it falls within the global of my debridements and partial amputation of the foot. We have appealed and they still say no. What to do now? Is there a recourse to get paid?

RE: The Passing of William Allen Webber, DPM
Dr. William A. Webber passed away on January 5, 2024 after a series of long illnesses. Bill was a 1966 graduate of the Illinois College of Podiatry. He was a member of the faculty for almost two decades, spending most of his time in the Clinics working to hone the clinical skills of the students (including me). He was a quiet and unassuming doctor, never becoming a household name in our field, but his contributions were many.
Bill and his wife Barbara became close personal friends, and we enjoyed many good times together. As I look back to those days, I remember others who contributed selflessly to student education whose names have been lost by time and poor memory, but whose efforts should never be forgotten.

It is with great sadness that the Ontario Podiatric Medical Association (OPMA) announces the passing of long-time podiatrist and former pharmacist Paul O’Connell.
Paul had many health issues, and his imposing figure will be missed here in Ontario Canada. Paul’s personality and great sense of humor and contributions will be missed by our profession.

As a student at OCPM, I first became aware of his contributions by Dr. Dalton McGlamry in the mid-70s. All three of these doctors have been giants in our field and contributed to the success of countless podiatrists.

I am writing this after reading Dr. Allen Jacobs’ thoughts on Dr. James Ganley three times. That brought back memories from 1977-1981 during my time at PCPM; during that time, Dr. Ganley lectured mostly on pediatrics to our class. Dr. Jacobs’ comments are “spot on” as to the influence Dr. Ganley had on his students! Personally during my 23 years in the USAF, we had Dr. Ganley speak to our armed forces DPMs several times. We had one particular seminar where Drs. Ganley and E. Dalton McGlamry lectured a full day, each back to back.
I still remember the "pearls of knowledge" from both of these giants of our profession. Dr. Ganley would sit in a chair on stage, reflecting back on his few slides - but was talking to us rather than lecturing about various topics. He always mentioned the medicine/surgery education and training he received as a corpsman in the U.S. Navy, and like all of our armed forces DPMs, his experiences in the military mirrored ours as far as our training with and working with orthopedic...

Concerning Dr. Manolian's comment about the best place to do a nerve biopsy... I agree that the three location biopsy we all do in the lower leg to me is where this should be done. But if the manufacturer requires a thigh location to be in compliance, probably this is where you would have to do this biopsy. I have checked with legal counsel here in California with the APMA and their response is this is legal here.
As far as why would anyone pay for this procedure vs getting it covered by a pain doc, the answer is simple. The surgical fee is the only fee the patient would be responsible for as their insurance covers the surgical center fee and thus the equipment and the anesthesia charges. As a concierge podiatrist, patients who you have a relationship with you want YOU to do the surgery and are willing to pay for it. If the patient is okay with you doing the surgery and you are upfront with the fees and they approve, what would be wrong with that?
Having said the above, almost all podiatrists accept insurance and the fee to do this would be generous I am sure. So it is a great area for advancement of our profession. Why not do these surgeries?
