MI Podiatrist Discusses Psoriatic Arthritis
According to Conway McLean, DPM, "Early on, psoriatic arthritis (PsA) was considered to be a less aggressive condition as compared to rheumatoid arthritis (RA). But this opinion has changed; psoriatic arthritis can cause as much if not more disease and disability. The pattern of joint involvement will vary from person to person. Some may have just a few joints affected, while others have many. Generally, the joint changes develop on only one side of the body, unlike RA where both limbs are. Foot involvement is a significant concern in PsA as it can lead to pain and disability. Ultimately, this means reduced mobility and quality of life."

"One diagnostic feature is the development of 'sausage toes,' referring to the swollen, painful toes associated with psoriatic arthritis. A single digit can be affected or many. Other anatomical structures that may be affected include the Achilles tendon, the plantar fascia (the structure usually responsible for heel pain), as well as the joints in the ball of the foot. A joint attack can be aggravated by dietary factors. Saturated fats, sugar, alcohol, processed meats, and carbs are notorious candidates since, along with adding pounds, they increase levels of inflammation in the bloodstream. These are known to be triggers for a psoriatic arthritis flare," says Dr. McLean.

4 Ways to Improve the Patient Experience for Older Patients
Improving the patient experience is not only for enhancing retention and care but also for bolstering population health and reducing the cost of medical care. Here are four steps you can take at your practice to make that change:
3. Show concern over their symptoms. Another way to improve your patients’ experience is to show empathy toward their concerns. This can be done by showing concern over their symptoms and asking questions about their feelings (e.g., “How is your pain level right now?”). It can also be done by listening when they speak instead of interrupting or talking over them. Doing these things will show your patient how much you care about them and what’s happening in their lives outside of medical appointments, making them feel more comfortable with you.
Teach your staff team how to listen carefully to patients and their families. Instruct them to write questions for the patient to ensure all issues are addressed. Showing concern could also provide timely patient feedback.

I was very saddened to hear of the passing of Lou Levine. When I was president of NYSPMA in 2003, Governor Pataki omitted the funding for podiatry in the Medicaid program in his budget. This would have been devastating for our residency programs, NYCPM, and our profession. Lou and I, as well our school and our association, worked together to avert this.
Lou cancelled classes and had the students join hundreds of our members to go to Albany. We were able to convince the governor to reinstate the funding in the Medicaid program. Without Lou’s leadership and the relationship between the college and NYSPMA, I do not know if this would have happened.
May his memory be a blessing.

Query: Resection of Fibula
A resection of the distal fibula is scheduled. This will not be related to infection, but rather due to an avulsion fracture. How would I code this? I was looking at CPT 27641, but the description says that is for osteomyelitis.

Response: CPT 27641-Partial excision (craterization, saucerization, or diaphysectomy) bone (e.g., osteomyelitis); fibula.
The example given is for osteomyelitis, but it is not saying it is the only example; it is just one example. The definition shows it is a partial excision of bone of the fibula. So, the avulsion fracture excision from the distal fibula would be properly billed as CPT 27641.

I am licensed in Florida and New Jersey and this license includes operations on the ankle. I used to be licensed in PA which license includes ankle surgery. Being licensed does not mean one is independently competent to operate on the ankle. Yet, this is the first step needed to book ankle cases to gain the experience and training to do so.
There are podiatrists out there in favor of closing this learning path regarding ankle surgery and limit it only to those who are board certified in reconstructive rearfoot/ankle surgery. If MDs have a license to treat the ankle without being board certified, is the DPM degree that inferior so that board certification is needed for a basic license to enter a path to upgrade one’s ability to perform ankle surgery?
