Supportive Slipper Can Be Beneficial During Pregnancy: NY Podiatrist
Perhaps the most surprising difference between pregnancy and the non-pregnant life is feet changes. "During pregnancy, it is not uncommon to notice flattening of the foot arches, swelling, an increase in show size, and even ingrown toenails," says Mohammed Rimawi, DPM, a podiatrist based in New York. "Slippers can certainly be of help as they work to mitigate some of the changes associated with pregnancy," Dr. Rimawi says. Before you go strolling, here are a few things he wants you to consider. Since the arches collapse, it "can lead to imbalances and alter the distribution of pressure along the foot, which can lead to pain and injury," he says. To counter these effects, look for slippers with arch support.

Feet also tend to balloon during pregnancy, so you want to keep your eye peeled for adjustable or wide-width shoes for any swelling. "Slippers can certainly be of help as they work to mitigate some of the changes associated with pregnancy," Dr. Rimawi says.

PODIATRIC PODCASTS |
VA Podiatrist Discusses Regenerative Energy Therapies
Ben Pearl, DPM was recently interviewed by Donald Pelto, DPM regarding regenerative energy technologies and electromagnetic therapy. Dr. Pearl was asked about the Voodoo stigma associated with some forms of magnetic therapy. Pearl said part of this is about the use of very low magnetic power wearables like insoles or mats that might have 1/2 gauss and are sold on the Internet. The more powerful clinical units have 2,000 - 5,000 gauss, similar to the energy level of an MRI chamber.

When asked about combing technologies such as radial or focal shockwave with electromagnetic therapy, Pearl agreed that this is beneficial. He prefers to do the combination of technologies on different patient visits to have a better idea of the immediate benefits of the treatment. Pearl believes that electromagnetic therapy will continue to grow over the next decade as doctors realize the benefits for bone, tendon, and nerve healing, as well as pain relief.

PRACTICE MANAGEMENT TIP OF THE DAY |
4 Ways to Improve the Patient Experience for Older Patients
Improving the patient experience is not only for enhancing retention and care but also for bolstering population health and reducing the cost of medical care. Here are four steps you can take at your practice to make that change:
2. The waiting area should be comfortable. Making the waiting area comfortable demonstrates that a hospital cares about the comfort and well-being of its patients. If there’s one thing we know about older patients, they’re more likely to have mobility issues than younger ones. That means they’ll find it difficult to sit for long, especially if they’ve been waiting for hours already.
Providing comfortable seating, magazines, and books, as well as having a television or radio playing in the background, can give older patients a sense of calm and serenity to replace some of the anxiety that comes with appointment anticipation.

RESPONSES/COMMENTS (NON-CLINICAL) |
RE: NY Podiatrist was Executive Producer For the Cowsills
From: Greg Caringi, DPM
I was reading a music article about the band, The Cowsills, and I ran across information unknown-to-me. "On that tour a few years back, they found the inspiration to write new material and then decided to put the songs to tape. That led them to a disastrous relationship with the now-defunct crowd funding entity Pledge Music, where all of the money they had raised to record a new record was lost in the company’s bankruptcy.
Fortunately, Ahmet Ertegun protégé Rock Positano, DPM arrived soon after to save the day. Dr. Positano was visiting Newport and looking upon its waterfront, decided that he had to work with the Cowsills. He reached out at just the right time and he and the band migrated to Louisiana where the three original members (siblings Bob, Susan, and Paul), along with their nephews and touring band, huddled and made what would become only the third studio album they’d release in over 40 years."
I have known podiatrists who were amateur musicians, but never an executive producer!
Greg Caringi, DPM, Lansdale, PA

RE: Medicare Advantage Plans
From: Glenn McClendon, DPM
This is my biggest headache right now. With chronic denials of multiple charges, payment of only lowest value charge or failure to pay the EM code and procedure code on first and only visit, I'm powerless to combat these powerful insurance companies. This is where APMA should be directing some of their attention. With collapsing revenues from Advantage plans and potential legislative reductions upcoming, dues and ancillary expenses will be scrutinized heavily for effectiveness. Declining revenue always leads people/doctors to look for alternatives.
Please address this issue. I've considered telling these patients I'm limited to only an E/M visit on first presentation. Many PCPs already limit their E/M visits due to this factor. Will patients be mad? You bet. But performing the work and knowing you won't get paid makes me mad and catches up one day, and then you throw in the towel. So why not let the patient be unhappy and tell them to contact their penny pinching insurance company. When do we get some guidance about how to deal with this? If not APMA, then who?
Glenn McClendon, DPM, Conway, AR
