Increase in Metatarsal Fractures Doomed Miminalist Shoes: CA Podiatrist
At the height of the fad from 2012-2013, podiatrist Dr. Kevin Kirby saw an uptick in injuries like metatarsal stress fractures—people were breaking the foot bones closest to their toes. He says this made sense, given that barefoot runners tend to run toe-heel, absorbing ground force impacts with the midfoot rather than the heel. Minimalist shoes have no additional heels to absorb the forces generated when runners’ feet impact the ground, unlike more traditional sneakers.

These days, more of Kirby’s patients are running with thicker-soled shoes than ever before. Every third runner he sees wears Hokas, a sneaker brand with the thickest underfoot cushions in the market today.

PA Podiatrist Gets Probation for Fraud and Opioid Sales
A Hermitage doctor was sentenced Friday in federal court to probation for possession of an opioid pain killer with intent to distribute and for healthcare fraud. Dr. Anthony Rossi, 66, received three years of probation, was ordered to pay restitution of $15,695, fined $6,000, and must serve 150 hours of community service. Law enforcement said Rossi was distributing Tramadol for cash from Feb. 24 to Nov. 19, 2015. He also submitted claims to Medicaid for services, including examination and injections, for services that were never provided from March 2013 to April 2015.

With employment litigation prevalent, most employers have come to expect that a former or current employee will, at some point, sue or file a complaint. Medical practices have to be more proactive to protect themselves. The following steps may not only help prevent the problem, but better position a medical practice to defend a case brought by an employee, should one arise.
7. Address Equal Pay Concerns. With the passage of the Lilly Ledbetter Act in 2009, which helps extend the applicable status of limitations for a potential Equal Pay Act violation, and with the increased focus on equal pay statutes across the country, medical offices are advised to evaluate pay practices to ensure that compensation is not in any way motivated by an employee's gender.

Query: Bunionectomy with Implant Coding
Now that CPT has revised the bunionectomy codes for 2017, deleting CPT 28293 (correction, hallux valgus [bunion], with or without sesamoidectomy; resection of joint with implant), what is the new code for insertion of a total 1st metatarsal-phalangeal joint implant?

Response: If the 1st metatarsal-phalangeal joint implant is inserted as part of a hallux rigidus repair, the appropriate code for 2017 is CPT 28291 (hallux rigidus correction with cheilectomy, debridement and capsular release of the first metatarsophalangeal joint; with implant).

The standard soccer cleats commonly available for youth soccer shoes are no problem for children playing soccer on outdoor grassy surfaces. I coached youth soccer for a number of years, had both my sons grow up on traveling soccer teams, and now have grandchildren playing year-round soccer. Standard cleat pattern soccer shoes seem only to be a problem for youth soccer when the fields are very dry, hard, and lacking adequate grass coverage. If the youth is playing a lot of soccer on these fields (thankfully not very common now in Northern California), then “turf cleats”, with soles consisting of many smaller, shorter cleats designed for playing on artificial grass surfaces, may be appropriate.
I have yet to see any injury caused by soccer cleats in the hundreds of youth soccer games I have attended over the past three decades. However, I have seen children trying to play soccer on a wet soccer field in old soccer cleats where the cleats have become overly worn, where children slip and take hard falls, or suffer groin strains, since their soccer cleats were not “grabbing “properly on the wet, grassy field. My advice if you are still not convinced? Go to your local soccer specialty store, introduce yourself as a local podiatrist, and ask the store owner to give you a short lesson on all the different soccer shoes and cleat patterns available. It will be a very worthwhile education for those podiatrists who are less familiar with the great sport of soccer.

I'm not an attorney, but I can relate a problem faced by some of the residents coming out of a surgical program and joining an orthopedic group. The group agrees to pay the podiatrist some salary (say $150,000 per year). The podiatrist agrees to put some amount of collectable revenues through the group practice (say $30,000 per month).
For the first 6-8 months, the group refers to the podiatrist and he/she is able to meet the monthly quota. Then, the referrals slow or stop and the podiatrist struggles and then fails to make his/her monthly quota. The annual salary may continue, but the unmet monthly quota is subtracted (monthly) from the account. If the podiatrist leaves the group practice voluntarily, he/she is still obligated for $30,000.00 per month for the duration of the employment contract.
Consider having the healthcare attorney you find specifically evaluate the contract for this scenario. Good luck! The coming years may be the best time in a decade to consider staying in or starting a private practice.
