NY Conference Offers Free Student Programming and Accommodations

The 2014 New York Podiatric Clinical Conference, “Mapping the Future of Podiatry,” will take place at the Marriott Marquis in the heart of Manhattan January 24-26. Sponsored by the Foundation for Podiatric Medicine, the conference is the largest of its kind in the country. Included for the first time is a FREE two-day Student Program designed for 2nd and 3rd year students, addressing the national shortage of podiatric residency programs. The program includes complimentary shared hotel accommodations for those participating in the complete program.

Dr. William Spielfogel

The Student Program will be held Saturday and Sunday to accommodate school schedules and travel time. “As post-graduate residency programs have become increasingly competitive, our curriculum has been developed to provide students with realistic strategies and solutions that will assist them in advancing their chosen profession,” said Dr. William D. Spielfogel, Vice President of the Foundation for Podiatric Medicine and Scientific Chairman of the conference.

PODIATRISTS IN THE COMMUNITY
NC Podiatrist Makes a "Big" Difference in Little Brother's Life and His Own

This month, the nation celebrates National Mentoring Month. Twenty years ago, podiatrist Dr. Daniel Waldman volunteered to be a Big Brothers Big Sisters mentor. Years later, he realizes that decision changed his life for the better, forever. "I’d like to think it did the same for my Little Brother. My Little Brother Jon was eight years old at the time. He was having a challenging time adjusting to the recent death of his father. My wife and I did not yet have children, and I was just starting my medical practice in Asheville, but made it a point to spend time with my Little Brother several times a month," says Waldman.

Dr. Daniel Waldman

"I often wondered if I made a difference in his life. A couple of years ago, I received a well-written letter from an attorney named Jonathan Spoon. Yes, my Little Brother was now a 28 year-old man specializing in art law. That letter expressed gratitude and affirmed that I did make a major difference in Jon’s life," says Waldman.

HemiCAP Toe Implant Does Not Require Cement: TX Podiatrist

Recently, Arthrosurface received FDA Clearance for the HemiCAP® Toe implant as a device which can be used without bone cement. This FDA Clearance was supported by multiple published studies that demonstrated the stability of the tapered post fixation, highlighting the unique advantages of the patented HemiCAP® design.

Dr. Brian Carpenter

This has been a great addition for all foot and ankle surgeons in providing up-to-date medical care to their patients. Fixation remains the main shortfall of all other toe implant devices on the market which is why the HemiCAP's® threaded cementless taper post is so crucial to our positive outcomes. Without cement, there is less bone loss, and it's very easy to convert to a fusion if that is ever needed. That being said, I have never had one loosen when used without cement," said  Brian Carpenter, DPM, UNT Bone and Joint Institute in Fort Worth, Texas.

RESPONSES/COMMENTS (CLINICAL) - PART 1
From: Terrance J. Mueller, DPM

I have used the FDL muscle/tendon unit since 1980, not just for grafting/replacement of the degenerated TP tendon, but also because the TP muscle is dysfunctional. Loss consequences of the transferred FDL are minimized by the actions of the quadratus plantae, flexor digitorum brevis, and the lumbricales. Plus, the distal FDL slips are still active via Henry's knot (anastomosis of the common FDL tendon to the FHL tendon prior to digital insertion).

Flexor overactivity as a consequence of attempted substitution for the dysfunctional TP muscle *and* tendon unit is markedly reduced as a result of the FDL transfer, in my experience. Additionally, the FDL acts in phase with the TP and is easily transferred with minimal tissue disruption, making it a superior choice to a TA transfer. Also, no iatrogenic biomechanical factors are introduced.

As it happens, I'm currently completing a summary of my 33-year experience with tibialis posterior dsfunction (TPD) for Podiatry Management. References for my papers, lectures, videos, & TPD AFO are available, but perhaps the most relevant one to Dr. Sullivan's query would be: "Acquired Flatfoot Secondary to Tibialis Posterior Dysfunction: Biomechanical Aspects" JACFAS, Vol. 30:1, pp 2-11.

RESPONSES/COMMENTS (OBAMACARE)
From: Ted Cohen, DPM

As expected, on the first full day back to work after the implementation of the Affordable Care Act, there was mass confusion for the four patients with brand-new coverage who called my office. And of course, because none had their insurance cards, it was almost impossible to get through to insurers to verify that these patients had coverage.

But when we did get through, lo and behold, our four patients had absolutely not a clue that their deductibles ranged from $2,000 to $5,000. When my office manager explained to each patient that they would have to pay for the visit, you know how many chose to do so? Zero. They all left in a huff.

This was Day One, and already the ACA enrollees aren’t sure what they signed up for in healthcare benefits. I can only shake my head as to what next week will bring.

RESPONSES/COMMENTS (PM NEWS QUICK POLL)- PART 1A
From: Matthew Garoufalis, DPM

APMA recognizes Dr. Bondar’s frustration at being terminated from the Multiplan network; however, APMA takes umbrage with Dr. Bondar’s comments that the association has not been educating its members. In fact, APMA has been diligent in sharing information with members about provider terminations, narrowing of provider networks, and managed care contracting through its various communication vehicles, including APMA News, APMA.org, APMA Weekly Focus, and APMA News Brief

APMA contacted Multiplan in support of Dr. Bondar when she first reported the issue in November and requested a meeting with the company’s medical director to discuss...

RESPONSES/COMMENTS (PM NEWS QUICK POLL - PART !B)
From: John V. Guiliana, DPM, MS

Being able to predict today’s healthcare climate is a challenge. For those of  you in practice long enough, you can likely recall the days when the business side of  medicine was a simple transaction, devoid of convoluted managed care contracts, government regulation, billing oversight, government-hired audit contractors, and state board investigations. I’m certain that we will see this trend continue and even worsen throughout this next decade.  This is exactly why PICA, with its eyes and ears on our profession, has deepened their focus on their Administrative Defense Coverage (ADC).

I have the privilege of sitting on PICA’s Risk Management and Underwriting Committees with many colleagues. That privilege comes with a deep understanding of what many understandably underestimate. Only a malpractice carrier run by a podiatrist and supported by a staff of experienced podiatric claims specialists would have had the foresight to increase the standard ADC coverage for our profession, as long as all policy conditions are in force, to $100,000 in response to these challenges that we all face. That coverage indeed includes improper removal from a private insurance panel. So anyone unfortunate enough to receive one of those letters challenging your integrity or your livelihood, please remember that PICA is far more than just our profession's malpractice insurance company!

RESPONSES/COMMENTS (NEWS STORIES)
From: Elliot Udell, DPM

In 2008, the New England Journal of Medicine published an article titled "Selective Publication of Antidepressant Trials and its Influence on Apparent Efficacy." The article showed back then that journals may be biased toward drug companies by publishing research papers that show that certain drugs are effective, and not publishing others. In the past few months, the New York Times did its own expose on this very problem.

The current award winning movie titled "The Dallas Buyers Club," which is based on a true story, deals with the issue of how drug companies often mercilessly promote dangerous products and dangerous protocols.
