NY Podiatrist Provides Tips to Prevent Frostbite
To help us take care of our heels and toes this winter, Optum Medical Care podiatrist Raisa Tsvaygenbaum, DPM shares her top tips. With a little effort and pre-planning, your feet will thank you all year long. “Wool can absorb a high amount of moisture, even more than cotton, and stays insulated even when feet are wet,” explains Dr. Tsvaygenbaum. “Toe warmers and leg warmers are also nice options for keeping warm when outside for an extended period of time.”

“If you like to exercise outside, wear lightweight, waterproof running shoes,” she recommends. “Also, make sure to run or walk on groomed pathways. Traction cleats, such as Yaktrax, can help stabilize your feet when walking on snow or ice.” “Ladies, the long boots we all own will not necessarily keep your feet warm,” Tsvaygenbaum advises. If you do plan to wear stylish shoes, try to find a pair that is warm and insulated inside.

MD Podiatrist Pledges $5,000 Matching Fund For Fitness Challenge Supporting Israel
Beth El congregants recently exercised their right to show support for Israel at this critical time by, well, exercising. The Pikesville synagogue’s Israel Affairs Committee raised more than $8,000 for “We Move for Israel,” a virtual physical fitness challenge held for two weeks in December.

For every mile reported by participants, Beth El congregant Jordan R. Stewart, DPM pledged a $1 match, with a goal of a $5,000 donation to the emergency fund. “Moving is good for the body and soul,” said Dr. Stewart, a podiatrist and fitness enthusiast. “I always encourage my patients to exercise and engage in physical fitness. Participation in this event will strengthen our community while supporting Israel.”

UK Podiatrist Spends Christmas Helping Homeless People
A podiatry lecturer from Queen Margaret University in Musselburgh has given up her time over the Christmas period to provide specialist footcare for homeless people in Edinburgh. While the rest of the country has been celebrating the festivities, Evelyn Weir, from Haddington, has been offering a full range of podiatric services as part of the wider Crisis Edinburgh initiative.

Now in its 10th year, Evelyn has been volunteering with the homeless initiative Crisis Edinburgh since it started in the Capital a decade ago. But helping people on the streets is now part of Evelyn’s DNA – she has been providing specialist foot services for homeless people in the Lothians for nearly 30 years and was recently awarded with the Alf Morris Award from the Royal College of Podiatry for her many years of volunteering with the homeless community, and her encouragement of podiatry students to follow in her footsteps.

The escalating mental health crisis in the U.S. has accentuated the importance of early detection. Healthcare organizations are prioritizing and incorporating mental health screenings to flag patients at risk and alert staff. Integrating mental health screening tools that remove the barriers that traditionally prevent accurate completion and pushing the results into electronic health records modernizes this process, ensuring efficient data capture while reducing stress on administrative teams. Research has found that patients are more likely to be candid about mental health issues on private and or remote screening methods – even more so than when they are speaking with their primary care physician.

Was this change made to any other sub-specialties? If you opted to have a status that does not have clinical privileges, is this really an issue for you? I would wager that the legality of this would be based on state statutes. That all said, the hospitals usually can make a requirement for active utilization of the facility in order to maintain credentials, but it generally should not have specialty specifics. Generally, call taking cannot be "forced" but utilization minimums can be required. Also, I would implore you to reach out to your state APMA component.

I find this post extremely upsetting. There is no place for this aggressive negative activity in our profession today. I question both the legality as well as the willingness of the few podiatrists to choose to remain on staff of the facility. I was the first resident at Peninsula Hospital Center in 1970. At the time, there were no surgical privileges for podiatrists and they were part of the adjunct staff. Surgical privileges opened up and soon after, the bylaws were changed, making us officially part of the medical surgical staff.
Similarly at St. John’s Episcopal Hospital in Far Rockaway New York, where I started podiatry, things went from podiatrists not being allowed to be members of the medical board to me being president. These achievements did not come easy. It required us displaying our professionalism and not accepting being treated as secondary citizens. As we are all aware, podiatry is now more than ever not only a crucial medical profession, but an extremely lucrative addition to any hospital. We as a profession absolutely cannot accept situations like this.
I suggest a member of our profession in the area of the hospital request a meeting with the CEO of the hospital and discuss the situation and if possible settle it amicably. If that does not achieve results, the APMA must get involved, and if necessary, pursue legal action against the administration.

Assuming you can do a thigh biopsy in California, is that the best place to do a biopsy for diagnosis of peripheral neuropathy? The answer is no. Next question is when you implant this device, completely out of the scope of your practice, will your malpractice carrier cover you? The answer is no.
Again, I ask when you post such things, why would a patient pay out-of-pocket for this in your office when an actual pain specialist or neurosurgeon will be covered by their insurance? The answer, well you get it by now. If I’m wrong, somebody please elucidate!

I am hopeful that the comment by Dr. Roth, in which he suggests that he would place a spinal cord neuromodulator in a patient with foot or ankle neurological pain, was submitted in humor. If a patient has a drop foot from L4-L5, would he perform a decompression surgery with fusion? What about an anterior cerebral artery aneurysm affecting only the leg and foot, or an aortic aneurysm causing blue toe syndrome or digital infarction? Is Dr. Roth prepared to treat potential complications such as spinal hemorrhage?
By the way, Dr. Roth, I suspect the placement of a spinal neuromodulator may not be comparable with your no insurance cash-only practice you profess to have mastered. You completed a non-ACGME approved podiatry residency, which you believe provided you with adequate training to place anything in a spinal cord? Please tell us you were joking.
