Brief Shoe Wearing Not a Danger to Babies, But Not Needed: Podiatrist
Jarrod Smith, DPM, a podiatrist and professor of podiatric medicine at Des Moines University, said putting shoes on babies is not inherently dangerous as long as it's not a "tight, structured shoe (worn) for an extended period of time." He referenced the now-banned Chinese practice of foot-binding as an example of the harm that could be caused by doing so. But he said that practice is not akin to putting well-fitting shoes on babies.

As for the complications some Facebook posts suggest shoe-wearing can cause, Dr. Smith said he has "never seen it manifested in real life in treating my own patients." He added that the claim can't be definitively answered through science because research ethics would not permit such a study to happen.

FL Podiatrist Pleads Guilty to $356,000 PPP Loan Application Fraud
Alexander Blaise, DPM of Plantation and his brother, Dumarais Blaise of Georgia, pleaded guilty to wire fraud conspiracy charges in Fort Lauderdale federal court stemming from a bogus $356,000 PPP loan application for a South Florida business, Acute Care Coordinating Systems PA. Alexander Blaise admitted that he was assisted by his brother, Dumarais Blaise, a tax preparer, in obtaining the pandemic loan and that the proceeds were deposited into another business, Blaise Podiatry, court records show. The brothers then split the illicit funds.

4 Ways to Improve the Patient Experience for Older Patients
Improving the patient experience is not only for enhancing retention and care but also for bolstering population health and reducing the cost of medical care. Here are four steps you can take at your practice to make that change:
1. Reduce wait times to see a specialist or doctor. When patients have to wait for hours to see specialists, they may go elsewhere in search of better service or treatment options. This could cause the loss of valuable patients who would have stayed if they had received better service sooner. Long wait times are a common patient complaint, ranking among the top five in a study conducted by the Office of the Ombudsman.

Query: Challenges with Submitting Claims
Are any other practice having issues with United Health Care/Oxford and The Empire Plan when billing for E/M codes? Our office now has to print the medical claim, attach medical notes, and send the old fashioned way as opposed to sending claims electronically. If the documentation and paperwork does not meet the criteria, they are denying the claim. We certainly can submit electronically. But if the carrier is just going to ask for medical notes from the beginning, then why wait for them to ask? The process is taking much longer for reimbursement. Even when I download the documentation, the turnaround time is longer by the time they receive it, view it, consider it, etc. Has anyone experienced this frustration with these carriers? Any input with this issue would be greatly appreciated.

Response: Sadly, I think this has become the norm as opposed to the unusual. Carriers are trying to bundle all that they can and that really hits home on the E/M usage and modifiers. It is in my opinion, a tremendous waste of time and resources versus having clear cut policies which are transparent and available to all providers.
The APMA lawsuits of years ago focused on changing codes, transparency of policies, and clear directions from the payers. That has apparently gone by the wayside. It has been seen and proven that if the requirements become so onerous for providers to do, they simply stop doing them.
Classic example is the declining use of the proven benefit of diabetic shoes. The paperwork and “hoops to jump through” have become so egregious, many doctors have simply stopped prescribing/dispensing them. What a travesty to the patients, the doctors providing them, and the system. Turnaround slowdowns let them keep funds longer but does neither our practices or the patients we serve any justice. Stay the course.

One of the responsibilities of hospital credentialing committees is to ensure "current competence". They look at either what the new applicant did during their residency if they are coming straight from training or the prior two to three years if they have been out of their residency for more than (usually) two years. When a physician re-credentials, then the committee looks at their activity for the time period since their last credentialing, usually two to three years.
This applies regardless of degree, DPM, MD, DO, DDS, etc. What credential committees that I serve on have done to give a physician the ability to obtain credentialing in situations such as yours, is to have them do a "mini-residency", where they do a set number and types of procedures under the tutelage of a residency program. I know that we have had one DPM and several MD obstetricians use this to gain current competence that our hospital requires. I don't know what residency program was used for the DPM.
Another thing to be aware of is a majority of hospitals are now requiring Board eligibility or certification to even qualify to apply for surgical privileges.

I have made ski boot-specific orthotics and have been doing custom boot fitting for over 30 years. I am a certified member of the Professional Ski Instructors of America, Rocky Mountain division. I worked with the Vail Ski School to refine the technique of using ski boot orthotics not only for support but to improve knee position and tracking. Making a proper orthotic for a ski boot differs from one for gait. When I put my running orthotics into my ski boot, my center of knee mass is misaligned!

I’ve had a great deal of experience both as an avid skier and podiatrist. My recommendation is to consider semi-weight-bearing impressions for custom ski orthotics. The foot is very compressed within the ski boot and full control can be quite painful. Lowering the MLA via the impression technique is very helpful. And since the medial column of the foot controls the medial edge of the ski, 1st ray cutouts permit improved 1st metatarsal plantarflexion, thereby improving edge control without the need for excessive posting or arch support.

I skied about 60 days a year for over 20 years. I improved my technique and comfort by working on my old rohador orthotics using layers of duct tape on the medial edge to determine the proper pronatory control. After finding the sweet spot to gain symmetry and edge control, I took them back to the office and added Korex posts to match. They are met head length and I inserted a flat insole to fill out the shell. They eliminated all foot pain and allowed me to ski on any trail I wanted.
The foot should not move in a boot. That causes foot fatigue and poor technique. You should be skiing with your knees and hips with additional balance from your upper body and poles. I found it challenging to design ski orthotics for patients. Without an expert boot fitter, it is difficult. As a result, I developed a close relationship with the fitter and most patients were satisfied.
