"While a very high percentage of plantar fasciitis patients respond to conservative treatments like proper stretching of the foot, massage, shoe inserts, anti-inflammatory medication, and physical therapy, some may require additional intervention, including injections or low-Dye strappings to alleviate the heel pain," says podiatrist Dr. Steven Hawley.

"Endoscopic plantar fasciotomy (EPF) is an outpatient procedure with a history of excellent improvement for patients with chronic plantar fasciitis. Because the incisions are small and less traumatic than traditional open heel surgery, less pain is typically experienced following surgery, complications are decreased, and the patient can even be walking the same day," says Hawley.

PA Podiatrist's Office to Close After Her Untimely Death
Gina Marie Carlo, DPM was a Carbondale native and graduate of Carbondale Area Jr./Sr. High School. After graduating from the Pennsylvania College of Podiatric Medicine at Temple University, she moved her practice back to her hometown of Carbondale. Dr. Carlo was more than just a foot doctor to all of her patients here in town -- she was like family!

On Nov. 16, just nine days after her 56th birthday, Dr. Carlo lost a very long fight with cancer. No matter how bad it got for her in the better part of her 14-year practice at her office, she never let her patients know she was suffering or the battles that she fought each and every day -- something only a doctor of her stature would do.

NY Podiatrist Joins Premier Medical
Premier Medical Group has announced that Dr. Robert Parajon has joined the group. Dr. Parajon is a board certified podiatrist who specializes in all aspects of podiatric care. He attended Dr. William A. Scholl College of Podiatric Medicine in Chicago, IL where he received his doctorate in podiatric medicine.

Dr. Parajon has been practicing in Dutchess County for over 25 years. He is the assistant director of podiatric surgery at Vassar Brothers Medical Center and is involved in many community organizations. Dr. Parajon will be heading Premier's new Podiatry Division.

Oxford Team Releases Major Review on New Medical Microbiological Technologies
A team of four physicians, surgeons, and scientists from the Universities of Arizona and Oxford today published a major technical review detailing groundbreaking technologies that are transforming the field of microbiology for clinicians and scientists, alike. "In basement microbiology labs at hospitals, worldwide, a revolution is going on, but it has yet to bubble up to the surface," noted David G. Armstrong, DPM, MD, PhD, Professor of Surgery and Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA).

Query: VoIP Phone Systems
I need to put a new phone system into my office and have decided to use a VoIP system. I have reviewed systems from Vonage and Jive, and I keep getting offers from Ooma. Is anyone using one of these, and are you happy? Are there hidden costs outside initial equipment setup? Will I need to upgrade my router? Are there any other pitfalls that I should be aware of?

The relationship between podiatric physicians and narcotic-seeking patients is far from being a black and white situation. Not every patient is a faker. There are many patients suffering with chronic painful conditions who will probably be on morphine-based drugs all of their lives. On the other hand, in my state, authorities are not happy with us writing for these drugs. A friend of mine who is a hematologist wrote a script for an opioid for a patient suffering with sickle cell anemia pain. Authorities swooped down on him. He was exonerated, but only after he paid 50K+ in legal fees and a year of aggravation.
In our practice, I do not write for these drugs even if a patient comes in sucking on Fentanyl lollipops justly prescribed by another doctor. I refer these patients to pain management specialists who have carte blanche with prescribing these drugs, and I will restrict my therapies to other aspects of foot and ankle care.

In the very early 1980s, I was berated by one of our colleagues (now deceased) for not taking x-rays of EVERY patient who came through the door because "you're missing out on ten grand a year." As I was a relative newbie, in practice only two years at the time, I asked about the justification for x-raying a patient with a paronychia. His response was "One in ten thousand of those will have osteomyelitis and you don't want to miss that." It's 34 years later and I've never forgotten that interchange.
I too wish to provide my patients the best care that I am capable of and did not mean to sound high-handed or judgmental. I fully accept Dr. Reingold's premise, and did not intend to say for a moment that no podiatrist should engage in dispensing orthotics or appropriate shoes, braces, boots, etc. from their offices. I am certainly not opposed to making a profit in my office, for as Don Barzini said in The Godfather, "we are not communists." My point was, and is, that utilizing a procedure or practice solely for the result of revenue enhancement does not serve us well.

I read the letter regarding improper billing and was wondering if the author was seriously recommending that we pry into other doctor's practices and start reporting our colleagues to licensing boards, etc.? I understand his improper billing methods affected his practice adversely, but did he really not know his billing was unethical or illegal? Most doctors know if they're doing something wrong. They may rationalize their actions as justifiable for many reasons but it's their personal moral road to walk. The last thing we need is for neighborhood practices turning into podiatry versions of the Hatfields and the McCoys.
Third party insurance billing IS a game. So is Russian roulette. Keep playing the game long enough and the results can be dire. Treating patients is supposed to be an art. Getting paid is supposed to be a science. When art and science get mixed up at billing time, it can be problematic. And when a podiatrist is publicly singled out as defrauding Medicaid and Medicare, it does impact all podiatrists. There just has be a way far short of certified letters and reports to licensing boards to make a colleague aware that his or her billing practices are becoming the subject of conversation. Because once you start down that road, the bad-mouthing of colleagues' skills and surgical results will follow, and the lawsuits will come cascading. No one needs that aggravation.
