Winter Foot and Ankle Mishaps Require Prompt Attention: CT Podiatrist
 
With winter upon us, serious injuries from ice-related falls can occur. When an individual falls on icy surfaces, there are major causes of foot and ankle sprains and fractures, and it's critical to seek prompt treatment to prevent further damage that can prolong healing. "Foot and ankle joints are vulnerable to serious injury, particularly with any hard fall on the ice. The mechanism of the fall on ice accentuates the fall and often causes more severe trauma because the foot can go in any direction after slipping," says podiatrist Dr. Robert F. Weiss.

Runners should use critical thinking before running on icy or snow-covered roads. In cases of less severe fractures and sprains, it is possible to walk and mistakenly believe the injury doesn't require medical treatment. "Never assume the ability to walk means your ankle isn't broken or badly sprained," says Weiss.

Drugs Companies "Routinely Withhold Results of Medical Trials" - UK Report 
 
Drug companies have “routinely and legally” withheld the results of medical trials from doctors, researchers, and patients for decades, members of Parliament (MPs) have said. In a damning report, the Public Accounts Committee said it was of “extreme concern” that about half of all trial results for medicines available on the global market were not subject to public and independent scrutiny. Warning that doctors and patients are being “undermined” in their ability to make informed decisions on treatment, the committee called on the Government to act to ensure that the results of all clinical trials of every medicine currently being prescribed are made available. 
 
The report will significantly increase pressure on the world’s pharmaceutical giants to commit to full transparency over clinical trials. Critics have accused “Big Pharma” of systematically putting profit before patients, withholding data that might undermine confidence in their blockbuster drugs. MPs said that “trials which gave a favorable verdict” were “about twice as likely to be published as trials giving unfavorable results.

HIPAA UPDATE
Derm Practice That Lost Patient Data Will Pay Feds $150K
 
In September 2011, someone broke into the locked car of an employee of Adult & Pediatric Dermatology in Concord, Massachusetts, and stole a computer bag. Inside the computer bag was a thumb drive containing information on roughly 2,200 patients who had undergone Mohs surgery. The thumb drive was unencrypted. It was as if 2,200 paper charts had fallen off a truck and fluttered down the highway.
 
Last week, the US Department of Health and Human Services (HHS) announced that the practice had agreed to pay the government $150,000 as part of a settlement of potential violations of the Health Insurance Portability and Accountability Act (HIPAA), passed by Congress to keep patient data out of the wrong hands. The settlement does not amount to admission of liability by the dermatology practice nor a concession by HHS that the law had not been broken. The 12-physician practice also agreed to analyze the security risks of its computer systems and electronic media and then develop a plan to improve security.

Query: Flatfoot Surgery

 

I have always stayed away from transfer of the FDL into the navicular because I theoretically believed that digital deformities were bound to happen down the road that would require further surgery (extensor overpowering). So far, I have been able to either advance the PT tendon (split or graft) or use the AT tendon to support any instability following osseous procedures without using FDL. I have never done a soft tissue flatfoot repair without some sort of osseous stabilization (not including PTTD in older adults).

 

A local orthopedic foot surgeon fairly routinely does an FDL transfer to augment flatfoot repairs in adults and adolescents. I am having a difficult time finding any definitive clinical literature on the long-term effects of doing an FDL transfer or any suggestion of what to do, if anything, about an unopposed EDL when the FDL is used for transfer in flatfoot repair. Who else is using this in combination with flatfoot repairs or in the course of PTTD repair? What is your experience regarding the long-term effect at the MTPJ and PIPJ clinically?

While there may be several surgical as well as conservative solutions for this unfortunate occurrence, what I can say without hesitation is stay away from writing an Rx for a hyperbaric chamber. This is a very dangerous and slippery slope you may be headed down. In NY, Medicare, as well as most other insurers, are on a 100% focused review regarding HBO2 treatments. Now, if this patient is willing to purchase her own, more power to her. However, more importantly, who is going to run this chamber and monitor this patient while undergoing her treatment? 
 
If you are the prescribing doctor for this chamber, I would be completely certain you are not responsible if and when she has an unpleasant result, which can range from simple conditions such as anxiety and run the full gamut, including respiratory disasters, to blowing herself and her entire family up. Prescriber beware.

There has been some disinformation about Measure 15. This rule will be a meaningful use killer for most small practices. You can get the info direct from CMS. It should be noted that a "checklist", as previously suggested, will not be accepted. You cannot get security assessment from your EMR vendor. It says that small practices are not exempt, and if you do it yourself, it will most likely not be approved. It has to be done in the year you attest, so if you haven't done it yet, you can't attest until next year.

I see Dr. Bondar’s concerns as a gift - a gift of time to acculturate practice. The golden goose in private practice, to me, is the fact that insurance coverage for foot orthotics and other markets is either non-covered or paid so poorly that in-plan DPMs must make concessions on quality and care. This means once educated, foot sufferers will shop for quality care out-of-plan.
 
Offering wellness, prevention, performance enhancement, quality of life upgrading, and balance and fall prevention in a friendly, warm, personal environment that mega practices and hospital-based DPMs cannot offer is the gift we offer to the foot-suffering public. In addition, having more time allows you to do more than routine care on the patients you still see in-plan.
 
My advice is to reduce investing so much in ICD-10, meaningful use, government and insurance-driven sludge, and the propaganda being dispensed by those profiting on your back administratively and politically. Invest your new found time, energy, and money in modern biomechanics, closed-chain dermatology, in-office dispensing, heel pain, ingrown toenails, and routine foot care.
