NY Podiatrist Named Businessman of the Year

If you're a fan of the CDM Chamber of Commerce's annual blues festival at Cedar Beach, you're a fan of Dr. Mark Gresser too. The Mount Sinai podiatrist and CDM first vice president has been the driving force behind the event since its inception in 1991, booking the acts and expanding the show to meet increasing demand over the years. The first performance consisted of "two bands on a hay cart," Gresser recalled, while last year's featured 35 groups on two stages.

When it comes to music, the Cedar Beach show is only one aspect of Gresser's dedication. He writes music reviews, is involved with the Riverhead Blues Festival and serves as both vice president of the Long Island Blues Society and vice chairman of the recently created Long Island Music Hall of Fame. Music and his medical practice may be Gresser's main focuses, aside from his wife and children, but he also finds time to teach at Briarcliffe College, volunteer with the Boy Scouts and keep up with civic affairs.

Retired CA Podiatrist Charged With Molestation of 7 Year Old

A retired Marin County podiatrist whose drug arrest made headlines a decade ago when he argued that the marijuana on his property was actually medicine was arraigned Friday on charges of sexually molesting a 7-year-old boy.

Dr. Alan Ager

Alan Lawrence Ager, 59, was charged in Marin Superior Court with one count of sexual abuse and two counts of lewd and lascivious behavior with a child under age 14, all felonies. He was arrested at his Nicasio home Thursday morning after the alleged abuse was reported to the Sheriff's Department patrol division, prompting an investigation. In 1999, Ager was sentenced to a year in jail and his son Daniel was sentenced to six months for cultivating and selling marijuana.

Bioterrorism/All-hazards Preparedness Available Free

Podiatric physicians are encouraged to take the first of a series of courses in bioterrorism/all-hazards preparedness included in a grant project funded to Nova Southeastern University College of Osteopathic Medicine by the U.S. Public Health Service, Health Resources and Services Administration as physicians. The first course is available without cost and is completely on-line. Registration is done on-line and a certificate of completion is granted upon completion. QUERIES

Query: Pediatric Hallux Valgus Bunion Splint

I have a family as patients where all of the very young children have a hallux valgus deformity. The children range in age from 3 to 8. Surgery is not an option with this family, nor do I feel it is appropriate at this age. Does anyone know of a small pediatric bunion splint which could be used until the children are older. Orthotic control is being used with several of the older children with minimal benefit.

RE: STJ Arthroeresis vs Evans (Tip Sullivan DPM)

To decide which procedure to perform on a pediatric patient with a painful flatfoot, try holding the patient's rearfoot in STJ neutral with one hand cupped around the calcaneus. Then, with the other hand load the forefoot under the 4th+5th met heads. If there is a locking sensation, then the midfoot is locked, and thus an arthroeresis procedure would be the procedure of choice. If and when you load the forefoot, and there is no 'locking sensation', and the midfoot is unstable while the rearfoot is in neutral, a calcaneal osteotomy (Evans) would be appropriate.

Also, be sure to carefully look at the weight-bearing x-rays. If there is significant metatarsus adductus, an Evans procedure is not warranted, as this would exacerbate the pre-existing deformity.

I do not feel it is appropriate to substitute one of these procedures for the other. Subtalor arthroresis is used to control frontal plane motion. The Evans calcaneal osteotomy corrects in the transverse plane. In children, I will typically perform both procedures if I can identify the deformity on more than one plane (valgus heel weight-bearing and obliquity of the calcaneocuboid joint). Another procedure which I perform concomitantly, is an endoscopic gastroc recession. I find by eliminating the equinus first, I get a better representation of what the rearfoot is doing in relationship to the forefoot. I believe Dr. Barrett wrote a paper on this in late 2004 or early 2005.

RE: Lost Soft Tissue Mass

I had a patient with a similar lesion, strictly by appearance. It turned out to be benign, a cavernous hemangioma - not very common. She has healed without incident. I would watch it closely just in case there is a malignancy with recurrence.

Dr. Glass's lost specimen looks like a giant cell tumor of the tendon sheath I took out 10+ years ago. According to Cole and DeLauro's book it is a variant of a pigmented villonodular synovitis.

The mass described in the photo looks vascular on clinical appearance. If that is the case it looks like a hemangioma.

RE: Gold Standard for Osteomyelitis (James Breedlove, DPM)

Its true, by operating on the patient and removing osseous tissue and sending it to pathology you can have the most definitive diagnosis of osteomyelitis. It will be more accurate than MRI or any of the scans but you may in the process compromise your patient’s health causing a bad condition to get worse. If a person has poor circulation and that is why he or she developed and infected ulcer and OM in the first place, doing a deep surgical excision of bone might be enough to permanently compromise healing. If the statistics are such as Dr Katz indicates that if MRI is positive, it is 99% sure that the patient has Osteomyelitis, I'd rather err on the side of conservative treatment, and rely on the MRI rather than on risking doing possible harm to the patient by doing a bone biopsy of every case of a deep ulcer I see.
