NY Podiatrists Recommend Moisture-Wicking Socks
 
When it comes to foot care, you want to think about socks almost as much as you think about shoes. "Socks are very important as well," said New York City-based podiatrist Gary Evans, DPM. "Can your foot benefit from a thicker, more cushioned sock? A thinner sock for better shoe fit?" If you're overwhelmed with sock options (there are many), Dr. Evans suggests trying a moisture-wicking sock, which can help prevent blisters, and "socks that are seamless, which means less pressure into and around the toes," he said.

Michelle Castiello, DPM, a podiatrist at Scarsdale Medical Group, an affiliate of White Plains Hospital, said that for walking or exercising, you want to stay away from cotton. "Choose moisture-wicking socks made from fabrics like merino wool, nylon, and polyester," she said via email. "Cotton socks can trap moisture against your skin."

CO Podiatrist is Proponent of Barefoot Running
 
Kristine Hoffman, DPM, a Denver-based sports podiatrist, is one expert banging on the barefoot running drum. “I’m a proponent of runners incorporating barefoot running into their training to help them adopt and maintain a forefoot/midfoot strike gait pattern, which has been shown to lower impact forces and reduce injury risk,” she says. She recommends short durations of barefoot running on surfaces like grass or sand and other barefoot activities, such as barefoot exercises during strength training, as effective for promoting shorter stride length.

Beyond injury prevention, Hoffman believes the positives of barefoot running include “increased strength of the intrinsic foot muscles and improved balance/proprioception from better sensory feedback,” she says. This ideology tracks with that of many podiatrists, but Dr. Hoffman notes that these secondary benefits are more theoretical and aren’t backed by a ton of scientific evidence.

3. Avoid subscription creep.
 
Subscription creep — when the amount you’re paying each month for streaming services and other subscriptions slowly goes up without you noticing — is a common problem. A study by C&R Research found that consumers estimated they were paying a total of $86 per month for all of their subscriptions, less than half their actual $219 monthly bill. The main reason: Automated payments made the charges easy to forget, even for services they no longer used.
 
Tackle your subscriptions by reviewing your credit card and bank statements — ideally for the past year, because some renew annually — and making a list of every recurring charge. Then, check your phone’s app store, Amazon subscriptions and other platforms for any overlooked or forgotten subscriptions you might still be paying for.

Query:  Coding Procedures with Time-Based E&M
 
I normally code an E&M visit with Medical Decision-Making. I have a new job that codes off of time-based visits. There are many denials for the E&M when a procedure is performed. I am trying to appeal these and am stuck when the denial is upheld stating it’s inclusive with the procedure. The documentation clearly states they were seen for unrelated services and based on time before during and after. Time does not include procedure. Why is it still denied and how do I appeal this?

Response from Codinghelpline.com Each E&M has time assigned to it but still requires the provider to document time related to only the non-procedure encounter portion of that visit. For example, when billing time, one must state at least several of those listed below and explain each one that contributed to “Total Time”:
 
i. Preparing to see the patient (e.g., review of tests).
 
ii. Obtaining and/or reviewing separately obtained history.
 
iii. Performing a medically appropriate examination and/or...

Query: Nail Biopsy Results – Candida Parapsilosis
 
We received nail biopsy reports these days that are filled with information and treatment recommendations. From a medical-legal point of view, what should be done in the following scenario. A healthy 35-year-old female's nail PCR report indicates: "isolation of Candida parapsilosis. This should always suggest careful evaluation of the patient, as Candida onychomycosis is frequently associated with diabetes or immunodepression."  
 
This patient is not a diabetic. This patient has no systemic symptoms that would indicate immunodepression. Is there anything else to do to protect oneself?
