MI Podiatrist Discusses Frostbite Prevention
The coldest temperatures we've seen so far this season are making their way across the Upper Peninsula this week. "Last year, in particular, I saw more cases of frostbite than I have ever seen," said Dr. Daniel Reminga, podiatrist. Dr. Reminga at Northern Footcare Center in Houghton saw a record number of cases of frostbite. "Frostbite is the freezing of human tissue due to exposed areas of the cold that includes the nose, the ears, even the eyeballs, but in particular at risk are the fingers and toes," said Dr. Reminga.

With the bitter cold temperatures dropping into the 30 to 40 below zero range, frostbite can occur within just 15 minutes. "I've seen one or two cases already," reported Reminga. "I'm assuming there will be more this year based on the temps that we're seeing already." With temperatures struggling to reach double digits through the rest of the week, Dr. Reminga says it's extremely important to dress warmly in layers and, if possible, limit your time outside.

Dr. Dalerie Wilkerson, a podiatrist at Parkland Hospital says we don't stop to think about the long hours we spend on our feet during our normal daily routine - not to mention all those activities over the holidays like shopping, parties, and extra cooking. Skin problems from winter weather don't help either.

"Stretch your feet (pull your toes toward your nose) before getting out of bed, because there's some muscle at the bottom of your foot called the plantar fascia. And it kind of goes to sleep when you go to sleep, so you want to stretch it out before you step out of bed every morning. When you step out of bed, don’t walk barefoot," says Wilkerson.

Metatarsalgia May Mimic Morton's Neuroma in Runners: IL Podiatrist
According to podiatrist George Tsatsos, DPM, "While pain between the third and fourth toes may sound very much like a Morton's neuroma, it may in fact be metatarsalgia, which mimics it. This condition is caused by a metatarsal head which is a little lower than the other four heads and receives more 'banging' when running."

"A true Morton's neuroma is painful with tight shoes and relief is obtained almost immediately by removing the shoe and rubbing the affected area. If your shoes are not tight, think metatarsalgia. If the forefoot is tight, get a wider pair of shoes. A good pair of orthotics may help. A cortisone injection may help. A precise diagnosis will help the most," says Dr. Tsatsos.

Would someone inform me more clearly about the change in Tricare and DME. We have never had a problem dispensing DME such as boots or braces.

Query: New *X* Modifier Use
With the farewell to modifier "-59", would someone clarify the usage of modifier "XS" and "XU" with reference to the following scenario? If CPT 11721 is performed with a CPT 20550 (and J1100-A5), would the correct submission be CPT 11721-XS, CPT 20550, J1100-A5?

Response: Modifier "-59" is not gone. If you are compelled to use these new modifiers, then select whichever of these best fits your service. Here are the CMS definitions for the two modifiers expected to be used by foot and ankle specialists:
XS - Separate structure, a service that is distinct because it was performed on a separate organ/structure
XU - Unusual non-overlapping service, the use of a service that is distinct because it does not overlap usual components of the main service
In the example you gave, prior to January 1, 2015, the coding for Medicare would be: CPT 11721, CPT 20550, J1100. There is no CCI edit bundling of CPT 20550 and CPT 11721. Medicare, however, has said that if or when there is an edit between two codes, it will still recognize the "-59" modifier. I feel that modifier "-59" should be able to be used in 2015. We are all looking for more information and direction from CMS on their modifier creations.

McKesson Medisoft Clinical v.11 has a software module in their EHR which allows the practitioner to enter a diagnosis by name or ICD-9 code to get an appropriate ICD-10 code. The physician can choose laterality and diagnosis specifics to get the appropriate code.

ICD-10 America is a tool that allows you to use text such as "hammertoe", "bunion", etc. to find ICD-10 codes; or you can type in an ICD-9 code and get the potential ICD-10 codes. It also allows you to easily insert the proper text associated with the ICD-10 code into your note by simply pasting it into your EHR (Control V). It is pre-configured with all common podiatric codes.

There is indeed a program called SmartSheet10. It is INCLUDED in the TRAKnet 3.0 EHR program. SmartSheet10 is one of the most intuitive programs I have worked with in regard to conversions between ICD-9 and 10. I was amazed at how easy it is and failsafe as well. You may want to speak with the TRAKnet folks and they can give you information as they are now the proprietors of the SmartSheet10 program.

Dr. Davis - I agree with you 100 percent! These "penalties" are nothing more than government "shakedowns", and while you ask where is the podiatric "superstar" to help us, I ask where is the APMA? Sequestration, PQRS, EMR....these penalties are nonsense! One of the reasons I belong to the APMA is for an organization to offer some sort of protection to our profession. I agree with you, Dr. Davis, that doctors should get bonus pay if they want to do this extra work that is now being asked. They should not be hit with penalties. The medical profession should be yelling at the top of their lungs at the lunacy of these shakedowns. Soon, we will have no money left to pay our APMA dues, as we will have it taken away by sequestration and various penalties.
I think we can all agree that this is NOT the profession we chose when we signed up years ago. The landscape is certainly changing and not for the better. Professional societies are needed to help practices adapt to the new terrain. Ours needs to be more vocal and proactive. We don't need a podiatric superstar; we need a public outcry.

Dr. Kass, first let me say that as a practicing podiatrist and a private citizen, I empathize with your frustration with our government. But I must vehemently disagree with the insinuation that APMA has failed to act on behalf of the profession to avoid cuts to physician payment and to assist members in navigating incentive programs.
As I stated in my letter to Dr. Kass on January 2, APMA has long advocated for a sustainable and meaningful replacement to Medicare’s flawed...

RE: Therapeutic Diabetic Footwear Collapse (Bryan C. Markinson, DPM)

Given all the recent comments in PM News regarding the Medicare Diabetic Shoe program, I am suggesting a new subject for a poll. What percentage of DPMs went to podiatry school and three-year surgical residencies in order to sell shoes? The primary reason I read from our colleagues endorsing participation concerns a "revenue stream" that we shouldn't miss. Really? Is that how we are making our livings now? And is revenue enhancement the best reason to join up?
Please do not misinterpret my lack of participation with any DME program as suggesting that such shoes, orthotics, etc. are unneeded. On the contrary, they are...
