"Practicing good foot care is something you should start from a young age. Once you first know you have diabetes, you need to gather your healthcare team and plan for the future. It's important to keep an eye out for any foot abnormalities," says Dr. John Murphy, Ellicott City, MD podiatrist.

If you check your feet regularly, you may catch diabetic foot problems early on, and they will be easier to treat. To combat further complications, you may need to alter your lifestyle to accommodate your diabetes. Dr. Murphy recommends that diabetics control their diabetes and develop better habits.

IFAF Holds Board of Directors Meeting

The first meeting of the International Foot & Ankle Foundation's Board of Directors for 2014 was held on January 2. The board reviewed the past year's events and projects and then previewed all of the upcoming scientific meetings and venues for 2014 and 2015. "It looks like it is going to be an exciting line-up of wonderful locations, top speakers, up-to-date topics and research projects," says Dr. Dock Dockery, Chairman of the Board and Director of Scientific Affairs.

David Yeager: One of the things I hate is that many students do not commit to knowledge items that I feel are important. Going to Google and getting me an answer a few minutes later is not going to make an outstanding surgeon. You can't search for information while wearing gloves in the OR; thus, I encourage all residency applicants to really learn all the information and not rely on Google.

This would be a very rapid AVN onset, if indeed this is the case. I am not certain that this is not more a case of bone remodeling with two nearby metallic screws in place and nothing more. No diagnostic tests would help, considering the recent surgery.
 
Why would AVN present without pain/swelling, and fragmentation of the bone with loosening of the hardware? Does this patient have polyneuropathy or other underlying conditions? If they are now NWB, would this not just be osteopenia from lack of weight forces?
 
I would wait this one out and allow the bone remodeling to continue. Purchasing HBO chambers would be a last option with little clinical strength behind it; and if that sort of income is available, why not consider placental graft injections with PRP to the joint in place of an HBO chamber? At least with some orthobiologics, you can increase bone depostion; thus strengthening the osteotomy, with slightly more data behind it.

I have not heard of superficial oxygen therapy as a therapy. My recommendation is a series of posterior tibial nerve blocks every other day for 8-10 days. The vasodilation does wonders in RSD. It certainly would not hurt.

Please note that any "HBOT chamber" that the patient can buy herself at home, won't provide therapeutic compression above 2.0 ATA (Atmospheric pressure absolute). Those chambers with zippers or the boxes that you put your extremities in won't do it. If you want genuine HBO therapy, I suggest contacting your local wound care centers. You can learn more about HBOT from the Undersea and Hyperbaric Medical Society webpage (uhms.org). Unfortunately, AVN is not one of the recognized HBOT indications.

After not matching on March 20th, 2013, along with more than 100 other qualified residency applicants, I truly believed that the APMA and CPME, along with the whole of the podiatry community would come together and deal with this devastating situation. Other than publish a couple of letters asking existing programs to increase their programs by one or two spots, absolutely nothing has been done. 
 
I have spoken to several very frustrated people who have shown interest in creating new programs, but who claim that the CPME is exceedingly difficult to deal with and have shown no...

RE: Have you recently been dropped from an insurance network?

According to last week's PM News Quick Poll, 23% of us were dropped by an insurance network. This should be the number one issue for the APMA. This means that these doctors will not get paid from some of the largest insurers, and those patients affected will have to change doctors. The entire concept of an insurance panel is insane. How could we have let insurance companies decide which doctors a patient can use. They have no right to interfere with that selection.
 
We should fight it - and get the MD groups as well as patient groups to press for an "any willing provider" law. About 1/2 of the states have it - but NY does not. It should be enacted in all states.
