CMS Withdraws Skin Substitute LCDs Scheduled for 2026
On December 24, the Centers for Medicare & Medicaid Services (CMS) announced that A/B Medicare Administrative Contractors (MACs) are withdrawing the Local Coverage Determinations (LCDs) for Skin Substitute Grafts/Cellular and Tissue-Based Products for the Treatment of Diabetic Foot Ulcers and Venous Leg Ulcers.
These LCDs were scheduled to take effect on January 1, 2026. With the withdrawal of these LCDs, there are no changes to skin substitute coverage for Part B Medicare beneficiaries in the new year; however, the payment changes finalized in the CY 2026 Medicare Physician Fee Schedule final rule remain in effect. Additionally, three Part B MACs (Novitas, CGS, and First Coast) have active skin substitute policies.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Ways to Show Practice Staff You Appreciate Them This Holiday Season
Here are some holiday-season moves that work for real-world practices — especially when they’re specific, inclusive, and tied to the day-to-day grind.
7) Invest in growth even in small doses
Not everyone wants a “career ladder,” but most people want proof they aren’t stuck. A small investment in development signals that you see staff as long-term teammates.
If you want a healthcare-specific frame, the Agency fo Healthcare Research and Quality (AHRQ) collects resources on clinician and staff burnout and the systems that drive it, including workload and practice design. See AHRQ’s overview on burnout.
Try this: Offer one paid training hour a month per staffer in Q1 (coding refresh, customer service scripting, triage protocols, leadership basics) and let them pick from a short menu.

7 Ways to Save Money on Almost Everything
High prices got you down? These strategies can help you reap deep discounts on everyday spending.
2. Try your luck online. In 2024, the Federal Trade Commission proposed a “click to cancel” rule, aimed at making it just as easy for consumers to cancel online subscriptions and services as it is to sign up for them in the first place. A court blocked the rule’s implementation in July, so companies in most states can still make you jump through hoops when you want to cancel.
The upside is that those hoops might save you money. Providers of phone or internet services, streaming and other digital subscriptions, or other recurring services tend to follow this pattern when you try to cancel: First, they’ll remind you how great their service is, then they’ll offer you a modestly better rate. If you decline, they’ll propose other options, such as reduced service (think streaming with more ads), or suggest suspending service for a few months. If you say no again, they’ll offer an even bigger price cut.

| CODING & BILLING Q&As FROM CODINGHELPLINE.COM | |
Query: Recurrent Nail Mass
I have a patient with a recurring ingrown nail with a suspicious subungual mass. X-rays are negative for an osseous lesion or destruction. I want to remove the nail plate and perform a biopsy of the mass. Can I bill for both the nail avulsion and biopsy of the nail bed and underlying tissues?
Codinghelpline com Subscriber
Response from Codinghelpline.com: CPT 11755 is defined as a biopsy of the nail unit which includes the nail plate, nail bed, matrix and nail folds. This of course assumes these structures were removed and a pathological examination can demonstrate these findings and a histopathological diagnosis.
In this scenario, the removal of the nail plate is not a separate entity unto itself. It is defined with the definition of a code which is frequently otherwise abused. If the patient requires a further definitive surgical procedure, then billing may best wait until the final pathological diagnosis is made.

RE: Modifier -25 Derangement Syndrome
From: Allen M. Jacobs, DPM
Though the courtesy of the ARCHE healthcare amputation prevention program, I had the December 25 OIG office of Audit Services report. I do not profess to be a practice management expert, nor an expert at statistical analysis. With that stated, I believe the paranoia infecting the primary care podiatry healthcare providers is not justified. It is a -25 derangement syndrome.
The "study" examined the ICD-10 and CPT codes for essentially what is callus or nail care by paring, cutting, or debridement. The appropriateness for the utilization of the -25 modifier, i.e.-the concurrent billing of an E/M code for a significant and distinct pathology unrelated to the routine care code was also examined.
To begin with, the sampling frame consisted of 155, 811 claims paid to podiatrists, of which ONLY...
