MD Podiatrist Comments on Lidocaine Shortage
Lidocaine is one of over 120 drugs that the FDA said is in short supply right now. “I have one vial left in my office,” podiatrist Priya Parthasarathy, DPM said. “A procedure that could simply be done in the office had to be taken to a hospital or surgery center, where IV or general anesthesia has had to be used.”

With patient care at stake, doctors want to see something done. “We need more of these medications being manufactured and our hands are tied. And so just like they were able to manufacture more COVID-19 vaccines, this also takes precedence,” Dr. Parthasarathy said.

Post-Static Dyskinesia a Common Sign of Plantar Fasciitis: IL Podiatrist
Plantar fasciitis can happen to anyone, said Eveline Tan, DPM, a podiatrist at Northwestern Medicine, but it occurs more frequently in people who are on their feet for long periods of time. That pain becomes pronounced after lengthy periods of inactivity. People with the condition feel their feet throb when they step out of bed after a night’s sleep, or when they get out of the car after a long drive, Dr. Tan said.

In more severe cases, when symptoms continue and the foot continues to bear repetitive stress, plantar fasciitis can become chronic, and the band of tissue at the bottom of your foot can degenerate. A doctor may recommend physical therapy, Tan said, or place the patient in a boot or cast. Very rarely, doctors may also prescribe a steroid injection. “You shouldn’t wake up in the morning thinking, ‘My feet are killing me,’” Tan said.

OK Podiatrist to Pay Feds $7 Million in False Claims Act Suit
A podiatrist will pay $7 million to resolve claims he violated the False Claims Act by submitting claims to healthcare programs for medically unnecessary and inappropriate levels of care between 2015 and 2019. The Seriousness of wounds was misrepresented, the complaint said. The federal government filed its complaint against Bryan Blanck on Tuesday, but this is actually “somewhat old news” because the settlement was finalized months ago in connection with an outstanding forfeiture case, said Blanck’s attorney, Elizabeth Stepp of Oberheiden PC.

TX Podiatrist Pays $90,000 to Settle False Billing Allegations
A 39-year-old Cypress man has agreed to pay to resolve allegations he submitted false claims for the placement of electro-acupuncture devices, announced U.S. Attorney Alamdar S. Hamdani. Dr. Amr El-Khashab is a podiatrist who practices in Cypress and whom podiatrist Dr. Judith Rubin previously employed. From March 1, 2018, to Nov. 30, 2019, El-Khashab and Rubin billed Medicare for the surgical implantation of neurostimulator electrodes. These are invasive procedures usually requiring use of an operating room. Medicare pays thousands of dollars per procedure. However, neither Rubin nor El-Khashab performed these surgical procedures. Instead, patients received devices used for electro-acupuncture, which only involves inserting needles into patients’ ears and taping the neurostimulator behind them with an adhesive. Rubin previously entered into an $865,000 settlement to address these allegations.

I would discuss your surgical case experience as a resident with the orthopedic chief. Focus on your confidence that your skills are strong despite the absence from the OR and that you have kept up with current surgical developments. Some hospitals will have a proctoring period where the chief or other designated person can observe some of your cases prior to being granted full privileges. Certainly ask for that.

I have adjusted orthotics for ski boots several times. It’s a much narrower shank and you have to grind the shell quite narrow. I use a functional sports-type orthotic with rearfoot intrinsic post to minimize bulk... 2 mm thickness forefoot extension.

Orthotics are essential! Shell selection is essential. Liners are less so, but our Surefoot custom liners are now so good, they double the life of a boot, and perform better (we use a patented memory foam that grips as tightly as the older foams, but have an elasticity that allows superior biomechanics). They are wired for heat, and pair with your phone. An orthotic that doesn’t fit in the footwear of its intended use will always be problematic, even if it’s otherwise perfectly designed.

Orthotics for pronation in a ski boot would be very effective to improve ski edge control, lessen transverse plane motion at the knees, and reduce foot fatigue. (If the foot is pronated in the ski boot, then one has to use their knees more in order to set the inside edge of the ski when turning). While a “three-quarter length” orthotic from a shoe may work in a ski boot, often the interior of a ski boot heel area fits narrower than a shoe, and so there may be trouble fitting the orthotic to the back of the boot.
I suggest ordering a narrow grind to the orthotics with shallow heel cups and a vinyl cover with Poron or Lunasoft to the toes. Sending the lab a tracing of the ski boot insoles would be helpful— or better still, the insole itself. A rigid cavus foot type with an orthotic in a ski boot can be problematic because of the dorsal midtarsal area pressure from ski boot buckles — and most skiers with this foot type have to find a boot that will accommodate their foot type.
