Apple Cider Vinegar Antibacterial and Antifungal: NY Podiatrist
 
Bye-bye, stinky boots. “Apple cider vinegar (ACV) is antibacterial, and can balance the skin’s pH levels by killing the bacteria that causes foot odor,” says Jacqueline Sutera, DPM. Foot odor is caused when the moisture (sweat) on your feet interacts with bacteria, and since bacteria thrive in warm, moist environments, the inside of your socks can be a petri dish of sorts. By using ACV to clear away some of that bacteria, you can subtly lessen foot odor.

You may have heard of using ACV to get rid of dandruff on your scalp (which is caused by an overgrowth of yeast and fungus), and the same principle can apply to using it on your feet. According to Dr. Sutera, it has anti-fungal properties that can help to treat skin and nail infections.

3. Don't Let the turkeys Get Me Down
 
Sometimes, when I look at my schedule, I notice appointments for difficult patients or difficult parents. These are the ones that I feel somewhat helpless in terms of adequately serving and everything I say is a disappointment to them. Interestingly, these challenging families are often "frequent fliers" that prefer to see only me. I need to remind myself that if they keep coming back to my practice and specifically to see me, I must be helping them in some way. I need to keep a positive attitude, especially with these patients and parents. They are the ones that need me the most.

Query: Coding Chemical Destruction of Nucleated Calluses
 
We often encounter nucleated calluses/intractable plantar keratoses which we debride and apply silver nitrate or cantarone in an attempt to burn the cores. Will I be able to use the CPT code 17110 since I have technically done a chemical destruction of a benign lesion?

Response: You are correct. The issue here is “destruction” and benign lesions.” Is an IPK/nucleated callous a benign lesion? It is a lesion (very vague term) and it is not malignant so I guess it is a benign lesion…or is it? I would opine that it is not.
 
Secondly, are you really destroying anything? Yes, you removed some tissue by some method but as long as the patient is walking, more than likely the callus will recur. Destruction would imply that the lesion is gone permanently.
 
I agree with Dr. Warshaw’s previous post on this. Otherwise, you are trying to fit a square peg in a round hole. Utilization goes up and then policies are formulated to address the improper coding of this scenario. These calluses and/or “benign lesions” are billed as a routine foot care treatment when appropriate, possibly using the allowance for pain with ambulation using CPT 1105X…otherwise this is a CASH service.

RE: The Illusionary CPT 99072 Code

With the advent, and effects of COVID-19, there was an attempt by AMA through the CPT to add code 99072 for the extra cost of providing PPE to healthcare providers. A reasonable amount of about $6 was proposed. This was to help cover the extra screening, sanitizing products, and other factors which increased costs of normal activities at a time when practices were forced to increase costs, and/or to cut back on hours and staffing. This seemed reasonable as a small effort to try to reduce the extra burden during this unusual time.
 
You would think this effort would be accepted by the insurers (especially government programs) as a small price to pay for the increasing cost to physicians providing ongoing healthcare in trying times. Instead, we now know the response (through intermediaries such as Noridian) is to "bundle" those costs into the already decreasing amount they are reimbursing. In addition, if you used the CPT 99072 code in your billing, it resulted in a claims rejection or freeze with no explanation. I know we go into healthcare to assist people with problems, but when you are treated like indentured servants, the phrase "the beatings will continue until morale improves" comes to mind.

He was a very insightful man with many great ideas. He surely made his mark for us all in podiatric medicine and foot & ankle surgery. RIP Howard.
