IL Podiatrist is New President of IPMA
 
Loyola University Medical Center podiatrist Francis J. Rottier, DPM is the new president of the Burr Ridge-based Illinois Podiatric Medical Association. As president, one of Rottier’s top goals is to restore full podiatric services to Medicaid patients in Illinois. The state recently limited such services to children and adults with diabetes. Currently excluded are non-diabetic adults who need treatments for such common conditions as plantar fasciitis (heel pain), bunions, circulatory problems, and ingrown toenail infections. “We are fighting for the citizens of Illinois to have full access to foot and ankle care,” Rottier said.

Rottier earned a Doctor of Podiatric Medicine degree from the William M. Scholl College of Podiatric Medicine. He did post-doctoral training at Illinois Masonic Medical Center, Hines VA Medical Center, and Loyola University Medical Center. He is an associate professor 
in the Department of Orthopaedic Surgery and Rehabilitation at Loyola and an attending podiatric surgeon at Hines.

IN Podiatrist Retires After 43 Years
 
Dr. J.K. Winckelbach has announced his retirement. He graduated from the Illinois College of Podiatric Medicine in 1970 and was an active member of the medical staff of St. Francis Hospital and Community Hospital South. He served as chief of the podiatric section of the surgery department at both hospitals.

Winckelbach was also active in the Indiana Podiatric Medicine Association and has served as president. He has been honored repeatedly by his peers with awards including Podiatrist of the Year in 1982 and 2005. His daughter, Wendy S. Winckelbach, DPM, will continue to work at the Southside Foot Clinic along with Michael C. Carroll, DPM.

I am wondering what an at-home hyperbaric chamber is. I have seen some companies promoting a chamber where just the extremity goes in, but I can't see how this would work. As I understand it, simply put, hyperbaric oxygen treatment requires a patient to be placed in a chamber that increases the normal atmospheric pressure of O2 that the patient breathes (the partial pressure of O2). The increase in the partial pressure of oxygen at the alveoli-capillary level will get more oxygen into the blood and therefore to the tissues.  
 
Unless the foot has lungs, how would placing it in a chamber increase oxygen delivery to the area? Please correct me if I am wrong. As for this particular case, perhaps osteomyelitis should be a concern as well as AVN.

RE: Hyperbaric Oxygen for Possible AVN of 1st Met Head?

In reviewing the x-rays, it does not appear to be avascular necrosis. Avascular necrosis appears sclerotic, not osteoporotic as seen on your x-ray. One needs to have blood flow to become osteoporotic. It appears the patient has hyperemic osteoporosis from fracture-healing, with joint narrowing which is common following any osteotomy-type bunionectomy .
 
Mobilize the joint and increase to full weight-bearing ASAP, if the osteotomy has healed. Osteoporosis and joint narrowing may not resolve. Don't treat the x-ray.

First, I am very sorry to see your patient with this complication. I have to disagree with the radiographic assessment of "no positional changes." From the four-week x-ray, the capital fragment has shifted medially and the hallux has deviated laterally. There is loss of correction, and the head has slightly impacted on the shaft. Also, the 1st IM angle has increased, and there may even be some demineralization at the base of the proximal phalanx. Last, there appears to be far greater edema around the 1st MTPJ at 4 weeks than 2 weeks. 
 
Without knowing more about your patient, differentials would include AVN, osteomyelitis/septic joint, Charcot arthropathy, and...

RESPONSES/COMMENTS (RESIDENCY SHORTAGE CRISIS )

It has been sad reading about the ongoing unmatched residents crisis. Out of sight does not mean that solutions have been found – just ask a recent podiatrist grad who does not have a residency! I am wondering if it possible for 10-20 DPMs, as a group, to provide $40,000 annual funding during the next 12 years for a teaching hospital that does not have (or has) a podiatrist resident, to create or add an additional podiatrist resident position? The donations would be tax-deductible and each podiatrist could pledge an annual contribution of $2,000 to $4,000.
 
I know that there are medical-legal obstacles and liability issues to overcome, but a hospital CEO might make it possible for a podiatrist resident position to be created if the funding is there from podiatrists. The podiatrist resident could also do short placements with some podiatrists in private practice. Who knows what goodwill will result from this move? The hospital may continue the funding after the 12 year period has ended.
 
Rather than accusing association(s) or leader(s) of profession myopia or poor understanding of the reality of podiatric economics, a grassroots proposal may help the podiatry graduates who desperately need a PSR sooner than “better luck next year.” This would certainly change the current mantra of “podiatrists eating their young.”

There are many issues facing podiatry, just as there are many issues facing medicine in general. This will be my 15th year in practice. My practice has 4 doctors now, and I do most of the daily practice managing such as payroll, paying all the bills, etc. So, on top of seeing patients 5 days a week, I put a lot of hours in related to my practice. 
 
I typically see patients 8:30 AM to 5 PM Monday through Friday. I do surgeries on Wednesdays, starting usually around 7 AM, and after those are done, I have the rest of that day off. I also stop seeing patients around 2 PM on Fridays so I have time for a long weekend, etc. I never am on call. I rarely get called from surgery patients, thus my...
