The America College of Foot and Ankle Surgery (ACFAS) has announced its newly elected board members from online balloting that ended on December 28: George T. Liu, DPM (Incumbent) and Andrew J. Meyr, DPM. Drs. Liu and Meyr will serve three-year terms (2021–2024).

IA Podiatrist Appointed Chief Medical Officer of WMC and Mayo Clinic Health System
Winneshiek Medical Center (WMC) and Mayo Clinic Health System leaders have announced that Thomas Marquardt, DPM, has been appointed to serve as the next Chief Medical Officer of Winneshiek Medical Center. Dr. Marquardt received his podiatric medical degree from the Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science in Chicago.

Marquardt was in private practice for nine years in the Milwaukee area, working closely with and training residents at St. Joseph's Regional Medical Center, and he joined Mayo Clinic Health System in 2006. In addition to his medical and surgical practice, he has served Decorah Clinic Physicians as Vice President for Practice and Professional Development for approximately 10 years and as Specialty Medical Director since 2015.

NY Podiatrist Named a “King of New York”
On December 15, 2020 Laurence D. Landau, DPM was named a “King of New York” by the Queens Courier. This awards show was held virtually this year due to COVID-19. This award is given to outstanding businessmen in leadership roles. Among the honorees were heads of universities, CEOs, COOs and business owners of large companies who have been able to run good businesses despite the COVID-19 pandemic.

Dr. Landau is currently the co-owner of New York Foot and Ankle, a large podiatry group with several offices in NYC and Long Island. Dr. Landau along with wife and co-owner Corrine E. Renne Landau, DPM were able to keep all of their employees and doctors employed through this pandemic. In attendance that night was his daughter. Samantha Landau, DPM.

Here are some ideas to get you started on your new year resolutions, whether you’re a practicing physician or a practice manager.
2. Get All My Notes Done in a Timely Fashion
I'm pretty good at finishing my charts, but every now and then I get a bit lazy and it gets away from me. This creates problems for my billers. It also creates a big problem for me. If I have had a really busy day, I sometimes have a hard time remembering exactly who a specific patient was and what was wrong with them. By getting my notes promptly, I will ensure that the patient chart is accurate and that the bill can be submitted quickly. This also applies to on-call notes and patient callbacks. I plan to improve on all of these.

The post goes on to state "these skin conditions could occur at any time during and after infection and may contribute to the feeling of burning skin". The 98 symptoms were reported by what are now called "Covid Long Haulers", who may have had a severe or mild initial case of the contagion, but gets relapses of these symptoms for some time. About 10% of all clinical cases (not positive blood tests that have been conflated as cases) may be in that category. Can a dermatology maven out there translate these two descriptions into usable clinical images?

RE: The Passing of Alan J. Kleiman, DPM
Alan J. Kleiman of Baltimore, MD, a 1978 graduate of the Pennsylvania College of Podiatric Medicine passed away last month after battling numerous health issues for the past several years. He was affectionately known to his PCPM classmates as “OX”. Alan was always upbeat and never complained about his ailments.

Despite being in and out of hospitals and rehab centers, he continued to see patients, primarily performing home care even though he had great difficulty walking to and from his car or even carrying his medical bag. He is survived by 3 daughters, 2 sons in-law, and four grandchildren. Alan was pre-deceased by his loving wife of more than 40 years Debra.

It is with both shock and sadness to hear about the passing of Howard Sokoloff. He was a classmate. He followed his love of leadership positions in our profession. That was his destiny that started with being class president in school and going on to bigger and better things. I still remember Rick Jay, Howard, and their wives adopting me as their fifth wheel on social outings as students. My deepest condolences to his family. Way too young.

Common sense (admittedly not high on the scale of levels of evidence) causes me to take issue with the notion that a posterior tibial block affects the solubility of Na urate crystals in joint fluid during an acute gouty attack. The studies that were cited support that a posterior tibial block increases blood flow by vasodilation. I can't argue with that. But those studies were done on normal intact feet. The reason a patient's foot is warm, red, and swollen in a gouty attack is that it's inflamed...vasodilated! Do you truly believe that a foot which is already swollen, warm, and red can be additionally dilated just because you give it a nerve block?
Posterior tibial block for pain relief...why not? But I think we're kidding ourselves if the intent is to provide any therapeutic benefit. Again, my conclusions are based upon my own logic and I'm willing to be proven wrong by a study looking at the effect of a posterior tibial block on an already vasodilated foot.

Almost by definition, temperature in the human foot is normally less than core body temperature. I think we can all agree that there is no need for double-blinded studies to determine this to be a fact. During a hyperuricemic episode, it is believed that urate crystals coming out of solution from a “saturated” plasma concentration, precipitate in these naturally cooler, distal articular regions which initiate the gouty attack. In conjunction with other therapeutic measures, many podiatrists utilize posterior tibial blocks to induce a temporary, localized sympathectomy for increased perfusion as well as the analgesic effect. Yet, the Arthritis Foundation, most all rheumatologists, as well as most of my colleagues would argue that ice (rather than mildly increasing the temperature of the site) is appropriate therapy for the ACUTE phase of the attack.
Dr. Levy appears to suggest (as do I) that consideration towards increasing perfusion (and perhaps warming of the affected area of the gouty attack) rather than ice may be more appropriate. Yes, ice is an analgesic in this situation, yet its use seems counterintuitive since it decreases temperature, perfusion, and would tend to prolong and enhance localized urate crystal formation. I am aware that once a gouty attack has begun, uric acid concentration in the plasma often has already decreased. Why would ice be the recommended therapy simply for analgesic purposes, when it may cause further, localized urate crystal formation?
