Podiatry Listed as One of The 9 Highest Paying Jobs of 2014
 
The good news is that these jobs pay a whole lot of money. Unfortunately, you have to spend a lot of money on schooling and specialized education to nab one of those huge paychecks. Careercast is back with its annual list of highest paying jobs. Healthcare occupations rule the day for the most part, taking a whopping seven spots on the list. 
 
Job: Podiatrist - Median salary: $179,923 - Who knew feet were so profitable? Podiatrists – aka foot doctors – are doctors who deal with issues affecting people’s lower extremities. They diagnose and treat people with malformations and other problems involving their feet, ankles, and lower legs. In addition to four years of college, they must also earn a Doctor of Podiatric Medicine degree, complete a 3-year residency requirement, and obtain licensure that varies by state.

The complexities and document requirements of the Medicare diabetic footwear program indeed have run amok, but to the degree depends on whom you speak with. From the perspective of large commercial suppliers, who are used to having to obtain complex and multiple documents from various prescribing entities, this is nothing new. Nor is this type of level of documentation requirements new to the orthotics and prosthetics industry, who often have dire consequences if a claim for a $40K prosthetic is rejected due to a lack of "medical necessity."

Both Drs. Kornfeld and Kaniadakis present valid perspectives regarding the FRAUD that is being committed by many of our colleagues and nurse practitioners. I am glad I never chose to play the "game". If it is a covered service, I bill it, if it is not a covered service, the patient is responsible. Remember, a non-covered service is not subject to limiting charges by Medicare or other insurers. For the past 31 years, as long as I have been practicing, the same issues have presented themselves with no apparent solution. 
 
Those who choose to practice in the murky waters of the CPT code book can continue to do so. In the future we will be subject to...

I don't consider there to be such a thing as "routine foot care" provided by a podiatrist. If a patient has a painful foot from a callus, or painful toe from the pressure of a hypertrophic, fungal, or cryptotic toenail, then medical intervention, assessment, and debridement or trimming are indicated. This is not routine. There may be an underlying capsulitis of the area beneath the callus, and there could be an incipient infection of the toe. Foot care, I believe, to be "routine" is that provided in a beauty shop, i.e., pedicures, etc. 
 
If a patient is compromised in any way, be it diabetes, vascular disease, etc., then, in my opinion, that patient requires additional...

Dr. Kass, I feel your pain and frustration. Where that 2% sequestration deduction goes is anyone's guess. I just looked over my newest mailing from Medicare on the upcoming payment "adjustment" for not meeting the meaningful use/EMR requirements. All said, by 2018, some of us could be hit with a nearly 10% Medicare pay cut for not complying with big brother's rules. I love how they call it a "sequester" or "adjustment". These are penalties and pay cuts, nothing less, and should be termed as such. I believe they should provide incentives or bonuses for those who want to participate in PQRS, EMR, e-prescribing.... not penalize those of us who don't or can't participate. 
 
I don't know about you, but I do not find these penalties very...

I was the lead expert in the Skecher’s Rocker Shoe multi-district Federal lawsuit. I opined that Sketcher’s did a poor job of disclaiming the dangers of their shoes. The case was settled. Rocker shoes, Earth shoes, and minimalistic shoes have potential useful and dangerous applications that have generated opinions from our experts in biomechanics.
 
Fat shoes are no different. There are whole categories such as those obese, those with a fall history, and others who would logically seem poor candidates. There are foot types at risk for injury, overuse, and compensatory sequelae that predictably should be disclaimed. Podiatry, utilizing biomechanics and biomedical engineering, can serve the community once again by alerting those who wish to try “Fat” shoes before the data surfaces.
