Playing Through Ankle Sprains Can Cause Further Damage: OR Podiatrist
Starting a new fitness routine as part of your New Year’s resolutions? Protecting your feet is critical. “No pain, no gain may be a motto for most workouts, but too much, too soon can lead to foot and ankle injuries, which can sabotage fitness goals,” says Jeffrey Loveland, DPM, a foot and ankle surgeon and Fellow of the American College of Foot and Ankle Surgeons.
|
|
Dr. Jeffrey Loveland |
Playing through an ankle sprain can cause further damage to tendons, which may take much longer to heal or possibly require surgery. What’s more, you may be overlooking a more serious injury. “Seek treatment for potentially-sprained ankles right away,” says Dr. Loveland. “Untreated or repeated ankle sprains may lead to chronic ankle instability, a condition that causes persistent pain and a ‘giving way’ of the ankle.”

NY Podiatrist Says Good Riddance to Outgoing NYC Mayor
Outside Gracie Mansion Friday, Mayor Bill de Blasio’s soon-to-be-former neighbors on Manhattan’s Upper East Side were overjoyed today to see him finally packing up and preparing to leave. Neighborhood podiatrist Sam Carr, DPM, used his Friday lunch break to stand outside Gracie Mansion with a sign that bid de Blasio “Good Riddance!!” and warned: “Don’t let the door hit your ‘toochas’ on the way out!”

Dr. Sam Carr (Photo: James Messerschmidt) |
“I really care about New York and it’s just so sad to see the destruction over the past eight years,” Dr. Carr said. “We’re like the Wild Wild West now thanks to him. So, I said: What the heck? I’m right around the corner from Gracie Mansion. It’s his last day. Let me give him a proper send-off.”

3 Digital Payment Trends that Physicians Need to Know
Following are three digital payment trends that have already begun and will continue to accelerate in the coming years.
1. The continued rise of contactless payments: In recent years, most merchants have migrated towards the acceptance of chip-enabled (also known as “EMV-enabled”) cards and away from the traditional swipe-enabled option. What many consumers don’t realize is that there is no mandate or requirement around use of chip cards; rather the change occurred as a means of shifting potential liability from merchants to banks.
If a criminal skims credit card information from a merchant’s EMV terminal and then uses that information for a purchase with another merchant that does not have EMV capabilities, the second merchant is liable for losses. However, if the second merchant does have EMV capabilities, liability is with the card issuer.

| RESPONSES/COMMENTS (NON-CLINICAL) | |
From: Paul Kesselman, DPM
There are three issues with long-term care insurance.1) The state’s insurance commissioners give the carriers a free reign to increase their rates almost without restrictions. A doubling of rates in just a few years is usury. Perhaps the only choice you have to maintain your current premium is to reduce the benefit period or monthly allowance. That certainly is not what you signed up for.
2) Old policies sold back in the ‘70s or ‘80s allowed for lifetime benefits and once you started collecting benefits because of a covered illness or disability, you no longer had to pay for premiums. Unfortunately, that may no longer be the case. New policies have a limited benefit period (e.g. 36 months) and often a couple purchasing them has a shared benefit period. Thus, to purchase a new policy from a new carrier is likely not sage advice.
3) The last and equally damaging issue is that those who often can afford to pay the egregious premiums can afford them and may actually have sufficient funds so as not to require LTC coverage. Those who can't afford premiums ultimately end up on State Medicaid and perhaps can have some of the care augmented by pooled trusts and other venues by which families can protect senior's savings.
Paul Kesselman, DPM, Oceanside, NY

| RESPONSES/COMMENTS (SPECIAL COMMUNICATION) | |
From: Richard Bloch
Dr. Levy, I congratulate you on your career and efforts on behalf of podiatry. I am writing this letter as an individual, not on behalf of the Maryland Podiatric Medical Association in response to your article, “RE: Preparing Podiatric Medicine for its Future Role in Healthcare: A Half Century is Much Too Long.”
I have had the privilege of working as General Counsel to the Maryland Podiatric Medical Association since 1979 and also as Executive Director since 1991. I am proud to be associated with podiatry and have worked diligently to advance the profession, especially to expand its scope of practice legislatively, as well as recognition as the only non-MD/DO profession that is licensed to perform surgery. With the advent of the 4-4-3 model that is equal to the MD/DOs, podiatrists achieved “equality” in...

Kudos to Dr. Ivar Roth for his innovated treatment for both humans and dogs. What a positive marketing opportunity for our profession to educate the population of what a podiatrist aspires to.

| RESPONSES/COMMENTS (NEWS STORIES) - PART 2 | |
From: Jay Kerner, DPM
Suggesting an anti-inflammatory diet to patients with gout, inflammatory arthritis, diabetes, etc. is good medicine. Suggesting cherry juice, turmeric, bananas, watercress, and low fat milk products as gout preventatives is, however, in the realm of alternative medicine. Though there have been individual studies suggesting the effectiveness of cherry juice as a gout preventive, there are recent studies that show it has no effect. There are papers now questioning the role of purines in initiating gouty attacks.
There are no studies for these foods for any pathology with the strength of the vitamin D3 or anti-inflammatory diet studies. Advising patients to take them based on ‘recent studies’ is more a belief or exaggeration than evidence-based medicine...regardless of ‘epigenetic factors’.
Jay Kerner, DPM, Rockville Centre, NY
