The American Academy of Podiatric Sports Medicine (AAPSM) has announced that Joseph Agostinelli, DPM, a sports medicine podiatrist in Niceville, FL was elected President. Dr. Agostinelli has over 23 years of active duty in the U.S. Air Force and 13 years in private practice. He is board certified in the ABFAS/ABPM and is a Fellow/Emeritus in the ACFAS/AAPSM. He was inducted into the World Softball Association Hall of Fame in 2015.

PODIATRISTS AND THE CORONAVIRUS PANDEMIC
NY Podiatrist and His FL Podiatrist Brother Receive Vaccine
 
Ronald Iannacone, DPM a podiatrist, was one of the first staff members of the Queens Boulevard Extended Care Facility to be inoculated Wednesday. He said the vaccine was a major relief after treating patients at the nursing home and his private clinic in Brooklyn throughout the pandemic. By coincidence, his brother, Robert Iannacone, DPM, a podiatrist in Florida, was also receiving the vaccine Wednesday morning.

Iannocone said he looks forward to eventually being able to spend more time with his 93-year-old mother on Long Island. “I see her, but she sits there and I sit 10 feet away from her,” he said. “I’m just trying to do everything humanly possible to keep everybody safe.” Iannacone and the other staff and residents will return to the nursing home cafeteria for their second dose of the vaccine on Jan. 20.

New Year Resolutions for Private Practice - Part 1
 
Here are some ideas to get you started on your new year resolutions, whether you’re a practicing physician or a practice manager.
 
1. Arrive to the Office on Time- NO MATTER WHAT
 
I recently wrote a blog about tardiness, concerning both patients and staff. I have been guilty of not arriving exactly on time, often getting in 10 minutes to 15 minutes late (justifying it that an 8 a.m. patient won’t be ready for me until 5 minutes or 10 minutes, if they arrived on time). This is selfish on my part and I aim to correct this. Yes, my time as a physician is valuable, but so is the time taken by my patients’ parents to bring their children to see me. Also, it is not fair to the staff to demand they show up on time but set a different standard for me.

RE: The Passing of Howard M. Sokoloff, DPM
 
It’s with a heavy heart and great sadness to report that my brother Howard Sokoloff passed away yesterday morning peacefully and with his family by his side. He was a good friend to many, a great podiatric surgeon, teacher, mentor, and past president of the California Podiatric Medical Association and past president of the American College of Foot and Ankle Surgeons.

Howard was an avid golfer and long-time supporter of the Kaplan-Clarke-Foster Tournament. Howard’s legacy to the podiatric medical profession is legend and he will be missed. He is survived by his wife Sheri, his children Jordan, Jared, Marla, and his grandchildren, brother Tilden, and sister-in-law Francine Sokoloff. Rest In Peace “Hesh”; you are loved.

PM News is sad to report the passing of Joseph Sciandra, DPM of Cheektowaga, NY. Dr. Sciandra was a 1980 graduate of the New York College of Podiatric Medicine.

I have a Bloomex BX200SC low frequency muscle stimulator which needs repairs. The manufacturer no longer repairs or sells these units (only accessories). Can anyone recommend a service center where this can be repaired?

Dr. Kass brings up a significant point. The requirement for biomechanical case experience has suffered from an inexorable decline over the past several incarnations of CPME Document 320, despite the increased unanimity in residency training and duration. The current proposal of yet a further decrease, to 50 cases over a three-year curriculum, is a continuance of this destructive trend. This decrease is not, nor has it ever been, what the ABPM had proposed during the 320 re-write committee activity. Nevertheless, and despite the stated intent that the 320 re-write process be collaborative among the CPME and the JCRSB recognized certifying boards, the process has been otherwise. The ABPM has offered practical alternatives to assist residency programs in focusing residents on “pathology specific” biomechanical evaluations of commonly encountered pathology (without compromising the overall assessment), and this too was summarily ignored at the committee level.
 
The biomechanical case requirement is essential for the comprehensive understanding of foot and ankle management which is, in fact, the specialty of podiatry. It has never been the domain of the “non-surgical” aspect of foot and ankle management alone. Its increased need in residency training should be appreciated by all those involved in residency training and should remain fully integrated within the curriculum.

I am not sure of Dr. Budny's point. Is he rationalizing his decision? Here is what I do know. "The general recommendation is to get the vaccine, even if you were previously infected," said Dr. David Thomas, a professor of medicine and director of the infectious diseases division at the Johns Hopkins University School of Medicine. "There are some nuanced questions that we don't have the answer to yet, but from what we know now, it's the right call to get the vaccine." 
 
Everyone be as safe as you can. You owe it to your family, your patients, and yourself.

After a successful posterior tibial block, simply by touching the plantar aspect of the foot, the patient feels anesthesia and also, a significant rise in temperature occurs. This is prolonged by including 1:1000 epinephrine in the anesthetic agent. Statistically significant increases in cutaneous temperature after nerve blocks compared to the same skin area before the procedure have been reported in the literature (Anesth Analg. 2009 Mar;108(3).
 
An article by Lima A, et al. in Critical Care (2009; 13 (Suppl 1): p 237) also indicates that after successful regional anesthetic blocks, local vasodilatation and increased blood flow occur as a result of blockade of sympathetic nerve fibers. Of course, the posterior tibial nerve block for acute gout is not the primary treatment of the condition. It markedly reduces pain but also begins the therapeutic process of bringing more fluid to the area as a result of vasodilation. This is followed by any of the oral medications that are used during the attack as well as after the attack resolves. In addition, injection of the affected joint with a corticosteroid may also be used.
