PODIATRISTS IN THE NEWS |
DC Podiatrist Discusses Cold Feet
Cold feet are common during the winter when the rest of your body tends to be chilly. Usually, you and your feet will warm up once you’re inside a heated space, or slip on your wooly socks. Still, Saylee Tulpule, DPM, a Washington, D.C.-based podiatrist and a spokesperson for the American Podiatric Medical Association, says, “When cold feet continue to progress — they’re cold even if you’re at home, or the feet are cold and causing pain — that’s a warning sign that must get treated by a physician.”
|
Dr. Saylee Tulpule |
Poor circulation — caused by a heart problem, smoking, high blood pressure, Reynaud’s disease, or diabetes — is the most common cause of chronic cold feet, Tulpule says. Blocked or narrowed blood vessels makes it harder for blood to flow freely. It takes longer for blood to reach your feet, which makes your toes cold. “Poor circulation isn’t going to get better without some sort of intervention,” Tulpule says. “It could be because of a poor diet or a smoking history, you could have major blockages that you’re just not aware of.”

FL Podiatrist Joins Modern Foot & Ankle
For the past twenty years, Orlandoans in need of foot and ankle reconstruction, heel pain treatment, or help with a badly stubbed toe have heard the same refrain: Go see Todd Talbert, DPM. Now, the venerable doctor has joined Modern Foot & Ankle and is excited to serve new, growing communities in the area.

Dr. Todd Talbert |
Board-certified podiatric surgeon Dr. Talbert came aboard the Central Florida healthcare group earlier this month. He practices primarily in the Celebration, FL clinic while offering evening availability at the group’s brand-new location in Kissimmee, FL.

IN THE COURTS |
MS Hospital to Face Black Podiatrist’s Bias Claims at Trial
A jury must decide whether a Mississippi hospital paid a Black doctor less than a White doctor because of his race, or due to differences in their productivity. Preston Boles sufficiently discredited Greenwood Leflore Hospital’s argument that his rate for work exceeding his contract minimum was lower than his White colleague’s because the White doctor was more productive, the U.S. District Court for the Northern District of Mississippi ruled Tuesday.
Boles worked as a podiatrist at Greenwood and received a base salary of $215,000 in 2019, while fellow podiatrist Joseph Assini received $250,000. Dr. Assini received $52 per RVU worked for every year he was employed at the hospital, while Dr. Boles received between $44 and $45 per RVU from 2013-19, the lawsuit alleges.

QUERIES (NON-CLINICAL) |
Query: Tip Jar or Charity Jar in Waiting Room
How do others feel about a "tip jar" left in the waiting room or a charity jar helping poor people pay for foot care? All responses appreciated.

RESPONSES/COMMENTS (RELEVANT RESEARCH) - PART 1A |
From: Elliot Udell, DPM
There is a question that all readers of the abstract will invariably ask. How does the complication rate of the Scarf procedure compare to complication rates of other first ray osteotomy procedures? Has this group done met analyses of other bunion procedures? Does the Scarf procedure have more or less complications as compared to other bunion procedures that utilize an osteotomy?
Another problem which relates to the Scarf procedure and similar types of osteotomies is whether the skill of a particular surgeon plays a role in complication rates. The study of 1,583 cases was not able to determine if the complications they listed applied to all surgeons or to only a handful.
Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS (RELEVANT RESEARCH) |
From: Douglas Richie, DPM
Thank you, PM News for posting the findings of a recent study published in the Journal of Foot and Ankle Surgery which showed a 5.1% recurrence rate after 1,583 Scarf osteotomies were performed for correction of hallux valgus deformity. These findings reinforce other quality studies which also show low recurrence rates after standard osteotomy procedures used for hallux valgus surgery. A systematic review of 229 published studies found a 4.9% recurrence rate after standard hallux valgus surgery. (Barg, et al. J Bone Joint Surg. 2018:100(18), 1563-1573.)
This begs the question: Why is one particular medical device company advertising to the general public via the Internet and warning that standard bunionectomy surgery has a 70% recurrence rate?
Douglas Richie, DPM, Long Beach, CA

RESPONSES/COMMENTS (RESIDENCY PERSPECTIVES) |
From: Daniel Chaskin, DPM
Getting enough sleep is important to keep one’s immune system healthy, especially while working in a pandemic. My guess is that most podiatric residency directors care about the health of their podiatric residents. They likely understand this and voluntarily suggest their residents get enough sleep for both their own health as well as the health of the patients being treated. More rest and sleep are needed to better avoid mistakes that affect patient care.
Daniel Chaskin, DPM, Flushing, NY
