Unsterilized Salon Tools Can Lead to Infections: OH Podiatrist
A woman in Tampa, Florida whose leg was amputated after a pedicure recently received a $1.75 million settlement for the incident, local news station WFLA reported. Podiatrist Joy Rowland, DPM told the Cleveland Clinic that people can receive skin and nail infections if tools aren't cleaned and sterilized properly. These infections include bacterial, fungal, and viral infections.

To have the safest experience possible, Dr. Rowland recommended that people "pre-screen" their chosen salon. "Visit the place you want to go. As you're in the waiting area, watch and see what they do. Make sure they bring out new instruments for each client and clean and disinfect their bowls after each use," she urged.

Pain Injections for Humans Also Seem to Work on Pets: CA Podiatrist
After treating more than 100 people in what appears to be a breakthrough clinical trial for management of consistent, unbearable foot and leg pain, Newport Beach podiatrist Ivar Roth, DPM, MPH’s unique treatment, called Paincur, has literally gone to the dogs. And that’s a good thing, said Newport-Mesa Veterinarian Tom Irwin, who happens to be a Roth patient being treated for incessant degenerative/arthritic knee pain. During one session, “Dr. Roth asked me if I would be open to new ideas for pet health and pain,” Irwin shared. In the last couple of months, “I have conducted clinical trials on 15 geriatric dogs,” all of which have been suffering severe arthritis and spinal pain issues leading to varying degrees of immobility.

“A couple of the animals responded (to the injections) within 20 to 30 minutes; the majority within 24 hours,” Irwin said. Most of the canine “patients” have been Labs, but a German Shepherd, Pit Bull, and Terrier have joined the mix. Although it’s too soon to tell long-term results, “three patients have had remarkable success, four have had good response, three have had fair response, two have had minimal success (but those never completed the recommended series of five Paincur4pets injections),” Irwin reported.

MD Podiatrist RecallsTrauma from Assault by Patient
Tammer Elmarsafi, DPM, director of podiatric surgery at Vascular Surgery Associates in Maryland, was a chief resident leading rounds with two junior residents when he was physically assaulted by a patient. When the patient became agitated in the exam room, Elmarsafi asked the junior resident to step out of the room. When the patient showed additional warning signs, he tried to de-escalate the situation. When that didn't work, he left the room.

"The patient had assaulted two nurses on two separate occasions that day, but there was nothing to indicate this in our system," Elmarsafi said. "While I was trying to gather information, he came out of the room and sucker punched me in the face. It was the first time I'd ever experienced anything like this and it was traumatic, both emotionally and physically."

2022 Medical Practice Asset Protection
Asset protection for doctors goes far beyond protecting personal assets from malpractice claims and must include both passive risk management and active defensive measures. Here’s a short list that includes both traditional business and medical practice risks and some of the newer, growing risks specific to current events like social and economic pressures, covid, and the politicization of healthcare.
5. Defend your medical business against digital threats. Medical practices of all sizes are prime targets for phishing attacks, ransomware, and other active criminal schemes. Add in HIPAA liability and an increasingly remote work force and the scope of the risk starts to emerge.

Have a strictly enforced IT security protocols in place that includes training, regular back-ups, updates and professional support.

Be insured for seven figures against various forms of data breaches with a seven figure, stand-alone data breach/cyber liability policy.

I was deeply saddened to learn of Marty’s recent passing. I had the great pleasure of meeting Marty and Larry when I first joined The Academy of Ambulatory Foot & Ankle Surgery more than 40 years ago. Their skill and knowledge of minimally invasive foot surgery was only exceeded by their humor and warmth. As I rose up through the ranks of the Academy board on my way to becoming president, I had the opportunity to work with them extensively. They were always willing to offer their assistance. Nothing was ever too difficult and they were never too busy to help.
Most people don’t know this, but when Larry became president of The Academy before Marty, some people asked Marty if he was disappointed that he hadn’t been elected president first as he was the older brother. Marty had a look of disbelief on his face and said, “Are you kidding? That’s my brother!”
A number of years ago, I unfortunately became estranged from my one and only brother. Knowing that I was going through a difficult personal time, Larry and Marty came over to me, gave me a big hug and told me not to worry and that they were now my brothers. From that day on, there wasn’t a single time that we were together, spoke on the phone, or communicated in writing that we didn’t remind each other of our love and brotherhood. This has meant the world to me. Those who, like me, grew to know, love, and respect Marty know that this is truly the end of an era. My deepest condolences, prayers, and blessings to Marty’s wonderful family. May he rest in peace. He will certainly never be forgotten.

I have been using the NaOH technique for 40 years of practice. I have seen every possible reaction, but I don't think I every suspected a true allergy. NaOH is an inorganic compound. We use a very small amount, and we immediately neutralize it with dilute acetic acid. In addition to the occasional post-op infection, I have seen reactions to the Amerigel we use as part of the post-op recovery that look like this. Seems like something other than the NaOH would be the likely allergen.
I guess you should also consider the possibility of a vascular compromise. When I see an odd reaction, I back-off everything but ask the patients to soak in warm Domeboro and apply nothing more than a dry gauze dressing. And... they all eventually heal. An older podiatrist once said to me - the tincture of time heals all.

Leonard’s comments bring back a lot of history and factual information. I was part of that movement at the California College of Podiatric Medicine. I had just finished my two year residency at Highland Hospital in Oakland California under Henri DuVries and Pacific Coast Hospital in San Francisco, Leonard created the first two-year program in podiatric surgery leading to a Master of Science Degree in Surgery. I was Assistant Chairman of the Department of Podiatric Surgery and soon to be Chairman of that Department and Chairman of Graduate education including our surgical residency program.
John P. Hubbard, MD was President of the National Board of Medical Examiners and he wrote a very dismissive letter over 50 years ago and again we just saw a letter from the USMLE in...

Kudos to Dr. Mitchell for discussing foods that not only can cause gout attacks (which we all know), but foods that can ward it off. The healing effects of food (and likewise the pathology inducing effects of poor diet) are not applied enough clinically in our profession. Epigenetic assessment is critical in so many conditions and simply treating symptoms alone does not improve the overall health of our patients.
