INTERNATIONAL PODIATRISTS IN THE NEWS
Onus is on Diabetic Patients to Seek Foot Treatment: UK Podiatrist

The team staff at Brocklehurst House says that while many improvements have been made in diabetics getting foot checks while in the hospital, the onus is on patients checking their own feet at home and recognizing signs early so amputations can be avoided. "We rely on people phoning us when there is an emergency foot problem," says Donna Welch, advanced podiatrist for diabetes.

"Unfortunately, we have had occasions when people have delayed getting specialist care and they haven't been aware of the fact they can come to Brocklehurst House straight away, rather than coming through their GP or district nurse," says Welch.

QUERIES (NON-CLINICAL)
Query: Storz Shock Wave Machines

I am considering buying a new shock wave device by Storz. There are three models, MP 50, MP 100 and MP 200. The only real difference between MP 50 and MP 100 is that MP 100 can go to 5 bars of pressure vs. 4 bars for the MP 50. Is there any reason to spend an extra $5,000 for the MP 200?

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Single-Payer System
From: Jim Fisher, DPM

ICD-10, decreases in our remuneration, increases in Medicare scrutiny, and a whole lot more anxiety seem to portend where we are going. There is no quick fix on the horizon. But let us just for a moment (re)consider getting the insurance companies out of medicine, i.e., a single-payer system.

The following is an excerpt from the New York Times, "The Single-Payer Alternative" by Nancy Folbre, November 25, 2013: "My fellow Economix blogger Uwe E Reinhardt, expanding on this theme, notes that the Institute of Medicine of the National Academy of Sciences recently estimated excess administrative costs [insurance companies over Medicare and Medicaid] of $191 billion, again more than enough to attain truly universal healthcare coverage."

If 70% of podiatrists (and I am guessing as many allopaths, osteopaths, dentists) are as unhappy with their profession, a single payor system does promise one thing - simplicity. If you agree, consider joining Physicians for a National Health Program (pnhp.org). It is not expensive to join and it can be the future.

RESPONSES/COMMENTS (PM NEWS QUICK POLL)
RE: Would You Change Occupations if You Could?
From: Alan Sherman, DPM

So what are we to make of the PM News poll results of 925 of our colleagues indicating 55% would change professions if they could, and another 15% might? Are these doctors that unhappy with their profession? Are they really that dissatisfied? I must admit, I was shocked to see these poll results unfolding and have shared quite a few thoughts on what they might mean with my colleagues. I wanted to see a few responses from readers before chiming in with my thought…

The United States was the first country to state that every man has a right to the pursuit of happiness, and promising it to its citizens. Note that the Declaration of Independence didn't purport to guarantee happiness...only...

Editor's note: Dr. Sherman's extended-length letter can be read here.

RESPONSES/COMMENTS (MEDICAL/LEGAL)
From: Steven J. Kaniadakis, DPM

My post related to the Banyan® crash cart. We kept it under our surgical suite back table. We also had Armstrong's Medintech® flooring. This is a type of non-conductive flooring. Electrical-type currents  from an AED or electro-cautery require grounding (plates, floors, etc). This is especially true when oxygen is flowing.
