MI Podiatrist Discusses Chronic Posterior Tibial Tendonitis
According to Conway McLean, DPM, "Treatments for chronic posterior tibial tendonitis vary. The most effective approach will address the biomechanical problem leading to recurrent excessive stress to the tendon. Excessively used or aged footgear can be a factor and is a common occurrence. Strengthening exercises for the muscles can be performed, whether at home or by a physical therapist. But make no mistake, soft tissue enhancements can’t overcome skeletal alignment pathologies in the long run. Many structures must be evaluated to determine what is leading to increased PT tendon stress. For example, a tight Achilles tendon can pull up too strongly on the heel, forcing the arch down more. Predictably, this will exert more force on the posterior tibial tendon, which is why Achilles stretches can be helpful for PT tendonitis."

These non-pharmacologic therapies have the goal of creating a stronger and healthier tendon, and therefore less prone to becoming inflamed. Scottish Podiatrists Trial New Behavioral Foot Ulcer Treatment
Podiatry researchers have been awarded almost £300,000 to trial a novel way of preventing diabetes-related foot ulcers. The team, from Glasgow Caledonian University, hope treatment based on motivational interviewing will empower patients to make lifestyle changes in order to reduce the likelihood of developing foot ulcers, which can lead to fatal complications in extreme cases.

People with diabetes are well informed about their condition but this does not necessarily lead to behavior change. “We have developed a new treatment based on motivational interviewing – a way of having a conversation with a person that helps them understand their reasons and barriers to positive lifestyle change, and supports adoption of new behaviors, or stopping unhelpful behaviors, to prevent foot ulcers."

Kids Footwear Startup is Brainchild of MD Podiatrist
Baltimore-based kids’ footwear startup Jbrds is taking a “blue ocean” approach to expansion, according to co-founder and CEO Mike Gugat. Since launching in July 2022, Jbrds has been selling direct-to-consumer via its e-commerce site and building a following among parents through performance marketing programs. After securing $500,000 in private seed funding this year, Gugat said the children’s brand is ready to take the next leap into wholesale starting in 2024. And it’s tapping into the expertise of its founders to carve out a white space.

Query: Coding For Injection
We had a little discussion in a meeting about the coding of an injection of the plantar fascia. The question revolved on whether or not to use CPT 20550 (that actually says “plantar fascia”) or CPT 20551 which says injection of a ligament or tendon origin or insertion. When we administer a plantar fascia injection, we are injecting the origin of an aponeurosis (a form of ligament-loosely speaking). Anyone want to weigh in?

Response: When it comes to coding and selecting the most appropriate CPT code to bill for a procedure or service that was provided to a patient, specificity is the key. When it comes to administering a cortisone injection into the plantar fascia at its insertion into the calcaneus, CPT 20551 might appear to be an option. The problem is that when you look at the definition of CPT 20551, Injection(s); single tendon origin/insertion, clearly it is specific for the injection of a tendon and the plantar fascia is not a tendon.
You can describe the plantar fascia as an aponeurosis or as a ligament, but the bottom line is that CPT 20550 is specific for an injection of the plantar fascia. It is defined as the following: Injection(s); single tendon sheath, or ligament, aponeurosis (eg, plantar “fascia”). End of story.

RE: Payers are Hiring Physicians with Histories of Malpractice

Some payers are hiring physicians with questionable backgrounds that go beyond malpractice history such as having state licensing board problems. While I have no problem with payers hiring physicians with a few liability claims, one should question why carriers hire physicians who have had multiple separate complaints logged by their state licensing boards. We are not talking about sham peer reviews, but actual legitimate complaints of past inappropriate behavior. For more information please click here.

To piggyback on Dr. Pressman's note, we recently published a case report utilizing a modified Allen test. The patient presented with increasing pain, swelling, and numbness in her bilateral hallux toes for 2 weeks duration. We employed a modified lower-extremity Allen test when we heard an irregularity in vascular perfusion in the foot after a difference in pitch between the dorsalis pedis artery and the posterior tibial artery was heard using a hand-held Doppler.
Non-invasive vascular studies were unremarkable and did not suggest peripheral artery disease. Further angiographic testing revealed occlusions at multiple levels of the lower extremity. The simplified lower-extremity Allen test allowed for efficient bedside assessment of the relative vascular contribution to the foot and can be an important tool in the clinical diagnosis of peripheral artery disease.
