Louis L. Levine, President and CEO of NYCPM, Dies at 95
 
Louis L. Levine, 95, President and CEO of the New York College of Podiatric Medicine (NYCPM), died on December 23rd. Levine has been President and Chief Executive Officer of NYCPM since 1991. Prior to his selection by NYCPM’s Board of Trustees, he was Corporate Vice-President for Governmental and Public Affairs at Empire Blue Cross-Blue Shield, from 1980 to 1991. In March 1976, he became Chairman of the Executive Committee and Deputy Chairman of the Board of Directors of Group Health, Inc., serving until 1979.

From 1966 to 1976, Mr. Levine served as Commissioner of Labor Affairs and as Industrial Commissioner of New York State under Governors Nelson Rockefeller, Malcolm Wilson, and Hugh Carey. Mr. Levine served in the U.S. armed forces for more than four decades, first in the Army Air Corps during World War II, and then in the U.S. Army Reserve, from which he retired in 1987 as a Lt. Colonel. In 2022, President Levine was inducted into the Podiatry Management Hall of Fame and received its Lifetime Achievement Award.

With every step, your feet take a pounding equivalent to up to 2.5 times your body weight. After decades of wear and tear, it’s no wonder foot woes start to crop up in your 50s. “Age plus weight plus inappropriate footwear plus genetics equals foot trouble,” says Eric Stamps, DPM, Dean of the California School of Podiatric Medicine at Samuel Merritt University.

For most foot ailments, starting with an over-the-counter fix is usually okay, Dr. Stamps says. See a foot doctor if pain is severe or things don’t get better in a week, or if the ailment interferes with your daily activities.

The American Diabetes Association (ADA) recently hosted a worldwide webinar entitled “Dialogue with ADA: Addressing Opportunities & Challenges of Preventing Diabetes-Related Amputations”. This virtual event featured leading experts in the field, including Professors Rodica Pop Busui of University of Michigan, Richard Neville, Chief of Surgery at Innova Healthcare, and Professor David G. Armstrong, Director of the Southwestern Academic Limb Salvage Alliance at Keck School of Medicine of University of Southern California. The program was moderated by Lisa Murdock, Chief Advocacy Officer of the ADA.

RELEVANT RESEARCH
Study of 1,583 Scarf Osteotomies Uncovers Outcome Stats
 
A team from the Gunderson Health System in La Crosse, Wisconsin have completed a systematic review of 1,583 scarf osteotomies and teased out the average rates of recurrence, troughing, avascular necrosis, nonunion, and more outcome statistics. The study, “The Incidence of Complications Following Scarf Osteotomy for the Treatment of Hallux Valgus: A Systematic Review with Meta-Analysis,” was published online on November 26, 2022, in The Journal of Foot & Ankle Surgery.
 
They reported that their meta-analysis was able to collect data from 1,583 scarf osteotomies. The weighted mean follow-up was 26.4 months. The researchers calculated a 5.1% rate of recurrence, 3.5% rate of troughing, 1.0% rate of avascular necrosis, 1.8% rate of nonunion, 2.7% rate of malunion, 2.4% rate of infection, 5.3% rate of complex regional pain syndrome, and 3.4% rate of hallux varus. The researchers also observed an average decrease in intermetatarsal angle of 6.3°. Study authors include Matthew R. Sieloff, DPM, Alexander R. Tokarski, DPM, Andrew D. Elliott, DPM, P. Michael Jacobs, DPM, and Andrew J. Borgert, PhD.

Adaptability and flexibility underscore nearly every major trend facing the healthcare industry in 2023.
 
5) Focus on the next generation - Practicing medicine is an incredibly rewarding and gratifying profession. To develop the next generation of the highest quality physicians, nurses, technicians, and other medical staff, the profession needs to put its best foot forward to recruit the most brilliant minds in 2023 and beyond, knowing the competition from other sectors will only make it more challenging.
 
This task is easier said than done. With the current generation, emphasizing workplace flexibility will be key. Medical students, wondering whether working 12-hour days are right for them, want to be assured that they will not be sentenced to a lifetime of grueling shifts. While the shifts might be longer and more demanding for younger workers as they develop experience, practices can offer better work-life balance as the staff advances in their careers. The ideal path might provide flexibility such that early-career physicians perform surgery and other procedures that require being on-call nights and weekends – then, as they advance in career, transition to hospital shifts or clinic work, where the urgent on-call requirements are less common.

I enjoyed reading Dr. Felley’s article about residency programs. I had been a resident at a high-powered program that required 12-14 hours of daily responsibilities from 6 am pre-op rounds, operative assignments, post-operative rounds, and educational lectures, etc. My real life was suspended for years while I participated in my program. My marriage was tested and my health was stressed at times.
 
Then I went into private practice which was surgically oriented, and I quickly realized what a gift I was given. Private practitioners handed me the blade to cut on their patients, under their supervision of course, knowing I had no idea what I was doing. That takes unique character and strength once I became the surgeon handing over the blade.  
 
Residents are vital to our medical system and their experiences in the operating room more than compensate them for the hours of work, inconveniences, and frustrations a program places on them. So, attending surgeons, please take a few moments to thank your staff and residents. And likewise, residents, take a moment to communicate your appreciation to be given this unique opportunity in your medical training!

We are also getting denied on those codes with basically every non-Medicare plan. Appeals are a waste of time. On top of it, they pay the lesser paying code instead of the higher paying code. Insurance companies are basically a legal means of extortion. It would be nice to see them punished financially for their purposeful denial of legitimate claims to bolster their profits. A class action suit would be the best way to go if this was possible.

I was one of the few to have this issue first. I made APMA aware of it, as the negotiation has stalled to a crawl between Aetna and APMA. I did suggest APMA get to threatening the health plan with exposure and recommending legal action, class actions, etc. The appeal letter is not the answer. Yes, I win every time, but I have to appeal over and over again. I suggest the following: APMA members should submit their good notes with the EOMBs to APMA. Flood them with this and demand they use your dues money to lawyer up, pay billing experts, and fight them.
 
Furthermore, we should all write letters to the U.S. Department of Labor asking them to take sanctions and fines for such abuse! Maybe we should start telling our patients to switch plans to avoid us from charging them what the health plan rejects. APMA and AMA should demand a cost of living increase of 5 percent from this health plan and others. We are being taken advantage of.
