PODIATRISTS IN THE COMMUNITY |
MI Podiatrists Team up With Senior Center for Clothing Drive
NorthPointe Foot & Ankle of Berkley joined the Berkley Senior Center in collecting new gloves, hats, scarves, and socks for local residents in need. The drive was held Nov. 17 – Dec. 12. “We want to thank all of our patients and community members that contributed to the success of this year’s donation drive,” said Dr. Lee Hoffman, podiatrist at NorthPointe Foot & Ankle. “We also want to acknowledge and thank the Berkley Senior Center team for their support of the drive and their effort in getting these donations to those that really need them.”

This was the fifth consecutive year that the two organizations have partnered to host the drive. Monetary donations were used to purchase needed items, and hundreds of donated pieces have been collected to distribute to the community.

APMA Advocacy Victory with TRICARE
APMA is celebrating a regulatory advocacy victory this holiday season. On December 31, the Department of Defense (DoD) released a final rule that ended a long-term disparity faced by podiatrists, preventing DPMs from prescribing and dispensing durable medical equipment (DME) to TRICARE patients. APMA worked tirelessly for more than five years to correct this disparity.
TRICARE Management Activity (TMA, TRICARE’s administrating body) has readily admitted it was not its intent to exclude podiatrists from prescribing and dispensing DME, but the movement toward amending the existing regulations has been slow. In addition to addressing the issue directly with the DoD and TMA, APMA has lobbied Congress, including members of the Senate Armed Services Committee, to put pressure on DoD and TMA to correct the disparity. Despite the long process, APMA is pleased with an end result that recognizes podiatrists as eligible and necessary providers of DME under TRICARE.

FOOT-RELATED RESEARCH |
In Overweight Children, Exercise May Strain Feet: Study
In overweight children, exercise can put pressure on developing feet, Australian researchers have said. In a recently published paper, they pointed out that overweight children are at risk of foot pain and discomfort because they tend to have fatter and flatter feet, which can lead to increased pressure on the arched portion of the sole. That, in turn, could stop them from participating in physical activity, they warned. “Physical activity is certainly crucial to health and well-being, and halting weight gain is important,” said Dr. Diane Riddiford-Harland of the University of Wollongong, who led a study of the effect of exercise on the feet of overweight children.

Dr. Alan MacGill |
Podiatrist Dr. Alan MacGill, a fellow of the American College of Foot and Ankle Surgeons who was not involved in the study, pointed out that supportive shoes and orthotics would help children with flat feet. “The devices can help control excessive motion in the foot,” he said. “The shoe should be the correct size for the child and not too flimsy from toe to heel.”

RESPONSES/COMMENTS (DME) |
RE: Therapeutic Diabetic Footwear Collapse
From: Bryan C. Markinson, DPM
The complexities and document requirements of the Medicare diabetic footwear program have run amok to the point that many long-term, non-podiatrist shoe providers are getting out of the custom-molded business and just doing off-the-shelf footwear. I do not as a podiatrist dispense footwear. A few years ago I tried, but after the first few patients started complaining about "too heavy" or "too ugly," etc., I decided to just write the prescriptions and have shoe providers take the headache. Now, with ever increasing frequency, I am getting requests for notes from the shoe providers as if patients who have had an amputation years ago can grow back the part that was amputated.
I predict the total collapse of the program due to over-regulation. In New York City, now it is very difficult to get custom-molded footwear. The problem is starting to jam up my phone lines, with angry patients and shoe providers who know that if they stop participating with Medicare, they can work a third as much and make the same money.

RESPONSES/COMMENTS (NON-CLINICAL) |
From: Steven J. Kaniadakis, DPM
Dr. Kornfeld, who is really our competition? It is not your fellow podiatrists who you have to be so critical about. It is the rise of other professions, many who are allied professionals, or even worse, career institute graduates. Other physicians and surgeons are hiring them to perform services and treatment-related procedures which are the typical podiatrist's bread and butter.
So, are you really thinking that other people do not bill and code properly? Even if it might well be an unintentional act, many other treating providers are billing and coding for typical podiatric-type office evaluations and procedures. One small example is a physician assistant billing for debridement (skin, nails, ingrown nail procedures, verrucae, biopsies, primary repair) and an office visit together. What is worse is that they are getting paid! Even independent of the physician or surgeon, they are using their own provider numbers! So, I ask you. Who is really your competition, doctor?

RESPONSES/COMMENTS (OBITUARIES) |
From: Alec Hochstein, DPM
I can only say how truly shocked and sad I was to hear of the passing of Dr. Williams. Claude was perhaps one of the sweetest people I ever met. He was a dedicated podiatrist to his patients and a respected professional among his colleagues of all specialties. I was fortunate enough to work side by side with him for many years as a friend and colleague, having consulted with him on many occasions. We often covered for each other during holidays, and enjoyed many conversations on various topics of which he was very knowledgeable.
Claude always had a smile on his face and was truly happy to go to work every day. I cannot remember a time that he would return from a clinic break without a cup of coffee in hand for me or the nurses. The world has lost one of the good ones, and I have lost a friend, someone I will not forget. May he rest in peace.
Alec Hochstein, DPM, Great Neck, NY
