NY Podiatrist Explains Why Your Feet are Different Sizes
 
The right and left sides of our bodies are often thought of as mirror images of each other. However, the same does not hold true for the feet. So why are our feet often two different sizes? "Nobody's feet are exactly the same," Corrine Renne, DPM, a podiatrist at New York Foot and Ankle, told Live Science. "Our bodies are somewhat symmetrical, but not always symmetrical." A certain amount of asymmetry in the human body is due to normal variation. "Our hands aren't completely symmetrical, nor are our eyes," Dr. Renne said.

There are also other factors that can lead to these size differences. "For instance, women can experience a bit of discrepancy in foot size during or after pregnancy," Dr. Renne said. They can experience ligamentous laxity, a condition in which their joints have a wider range of motion than usual, and their feet may end up differing in size if they naturally favor one side of their bodies over the other. "Sometimes their bodies go back; sometimes they don't," she said.

CA Podiatrist Cautions Against Wearing Sneakers Without Socks
 
Wearing sneakers without socks may feel stylish or comfortable for some, but Ebonie Vincent-Sleet, DPM, a board-certified podiatrist and foot and ankle surgeon at OC Podiatry doesn’t recommend it. "You don’t have that initial barrier of protection [without] a sock,” Dr. Vincent-Sleet said. You could develop athlete’s foot, fungal nails and calluses.

For people who want the no-sock look, she suggests trying footie socks or ankle socks. “There’s so many different sock choices for your footwear out there,” she said. “Pick the one that’s most aesthetically pleasing to you, but definitely wear a sock.”

A PM News Subscriber asked if a podiatrists can "technically and legally treat a mechanical problem of the foot using a special brace on a patient who has an in-toe gait, since the diagnosis is actually a hip rotation problem. There also have been similar questions about whether or not certain tests for systemic conditions that are responsible for foot pathology are within podiatrists' scope of practice. 
 
My opinion about these requests for reader opinions is that any published responses that support podiatric provision of questionable services that are simply conjecture and are not published authorization from reputable sources, including health attorney opinions, should be ignored. We never know when there is going to be a malpractice claim for any reason and this questionable service will be discovered and will open a proverbial can of worms.

Ten or twelve years ago, the television was inundated with commercials for Laser Spine Surgery Institutes or similar organizations. One of the entrepreneurs wanted to establish a program in Belize as a medical tourist destination center. It was short lived. Malpractice claims in the U.S. skyrocketed. The surgery was performed endoscopically and any doctor who wanted to be trained and work for the company could. Patients showed up with $30,000 or so in hand to turn over to the company because no insurance was accepted. Since radiculopathies and painful feet along with foot drop affect patients, we can assume California podiatrists could have been trained to perform the procedure. Unfortunately, non-spinal surgeons were not trained to address the surgical complications resulting in deaths and paralysis. They make intra-operative neurophysiologic monitors to alert board certified neurosurgeons of impending disaster. But a weekend course should obviate the need for that. There is at least a day’s worth of reading concerning the topic on the Internet.
 
Dr. Roth, I would not feel very comfortable performing a stent placement in the Fem-Pop area of the leg because you and your podiatry friends think it’s OK. You cannot possibly believe that you folks define and interpret the law. The opinion of the CPMA attorney makes it legal? It is an opinion. You know that old saying, “opinions are like mouths; everyone has one.” If the California Supreme Court felt stent placement was within the legislated definition of podiatry in California, that may be another thing. Stent placements have been known to go wrong and the vessel ruptured. Are you and your DPM friends prepared to perform a Fem-Pop arterial repair, anastomosis, or insert a new surgical graft? I realize you have not had to address many complications, but there are surgeons who have.

Dr. Roth, once again asks why we as podiatrists cannot be trained to do procedures such as spinal stimulators and stents to treat PVD. 
 
There is no doubt that Dr. Roth and most DPMs are smart enough, with proper training, to do these procedures and do them well. It all boils down to legality. Dr. Roth states that he spoke with legal counsel who said, "If the law says we can do it, why not?" The obvious question is what the law in each locality will and will not allow concerning surgical procedures done way above the ankle which directly affect the treatment of pathology in our anatomic scope of practice. 
 
In essence, our ability to perform these procedures will probably be decided by state legislatures and courts, and we should not expect state medical societies to stand by idly and watch us take these procedures from their members.

RE: NY Medicare Advantage Insurer and CEO Will Pay Up to $100M

To say it's about time is an understatement. In yesterday's Wall Street Journal, an article titled: "UnitedHealth’s Army of Doctors Helped It Collect Billions More from Medicare" is a must read for every provider, patient, and taxpayer. One example cited, was a $16,200 payment vs. a $4,680 payment made by CMS to UHC for a 75-year-old male patient. This was accomplished simply by adding a few ICD-10 codes to the patient's risk assessment. This clearly happened hundreds of thousands of times in unwarranted scenarios. The study also cited how the costs to CMS increased significantly for those patients who went from fee-for-service to UHC Part C plan.
 
The story also recounts the epics of one UHC employed family physician at one of their numerous Optum centers. This provider tells the story that the EHR software would not allow him/her to close out the patient's EMR unless the doctor...
