FL Podiatrist Uses His Tri-lingual Skills to Reach Diverse Audience
Dr. Ugonna Anyaugo had a lot of career choices growing up. He could have entered the legal profession, the social service profession, or something industrial. But, he felt the best way for him to serve people would be as a doctor. Dr. Anyaugo, who specializes in podiatry, was educated in podiatric medicine at the Ohio College of Podiatric Medicine.

Although he keeps himself very busy with his practice, Dr. Anyaugo still finds time to write poetry, and his tri-lingual skills (English, Ibo and French) has afforded him the opportunity to speak before a very diverse audience.

PRESENT to Host 2014 and 2015 Desert Foot at Sheraton Phoenix

Query: Hospital Visit Coding
How do we bill for extensive hospital visits, e.g., consults? As podiatrists, can we use CPT 99223 or CPT 99233 for Level 3 visits?

Response: Yes, specialists can bill the hospital encounter E/M codes for Medicare consults (Medicare does not recognize CPT consultation codes). Most other payers continue to allow the standard CPT consult codes.
That said, in order to bill CPT 99223 (initial hospital visit: comprehensive history; a comprehensive examination; and medical decision-making of high complexity) or CPT 99233 (subsequent hospital visit: detailed interval history; a detailed examination; medical decision making of high complexity), you must meet the criteria for using those E/M codes.
In the back of the CPT book, Appendix C, are examples of what some common scenarios would be for these codes. Keep in mind that it is both documentation and medical necessity that determines the code.

RE: Would You Change Occupations if You Could?

On its face, a 55% dissatisfaction rating (plus another 15% 'maybe') sounds pretty horrific. But there is more to analyzing that statistic than is evident. Suppose the same question was asked to lawyers? to family doctors? to dentists? to accountants? to salesmen? To many, the grass is always greener elsewhere. And many people chose careers not having a strong affinity to the one that they chose, but based out of necessity of making some kind of choice. The statistic would only have meaning in context, comparing podiatry dissatisfaction with other occupations' dissatisfaction.
Dr. Reingold said it best. A few years ago, I was walking down the hall on a very busy day at the office. I turned to my assistant and said, "The thought just struck me. Someday I may have to go to work for a living." Love what you do and you will never have to work a day in your life!

It was somewhat disconcerting that such a great percentage of our profession would opt for a change in their occupation as seen in the recent PM News survey. However, we might be surprised as to the statistics of unhappy people in almost any profession, not just in podiatry and medicine.

I was one of the first to actually go through an EMR audit. Much to my surprise, the audit process was fair, but it took over 6 months. Some of the things that they were asking for were burdensome. One main problem we had was with our dashboard changing numbers. Absolutely do a snapshot of your dashboard when you attest to avoid those problems.
With regard to measure 15, we just used a template supplied by our EMR, and that was accepted as adequate.

Right out of residency, I trained in advanced cardiac life support. We started out in the office with all the meds, etc. However, as meds and certifications expired, I began to rethink the risk. You should understand that having those meds exposes you to a higher level of scrutiny and expectations beyond our scope of practice. If you feel comfortable, up-to-date, and capable, I admire you. However, understand that perhaps oxygen and an EpiPen are enough. Call 911.
