PODIATRISTS IN THE NEWS
NY, FL Podiatrists Discuss Bromhidrosis

Going sans socks is a common culprit for foot odor, but stench can also occur with them. That’s because it’s not entirely about what’s on but what’s coming out of your feet (apologies for the visual). “Excessive sweating or hyperhidrosis is the number one cause of smelly feet,” says Sophia Soloman, DPM, a board-certified podiatrist at Manhattan Foot Specialists in New York City. When feet sweat, they essentially create a tropical rainforest inside your shoes (especially between the dark, moist crevices of your toes), or the type of environment odor-causing bacteria and fungus thrive in.

Drs. Sophia Soloman and Suzanne Fuchs

Not plagued with sweaty feet? According to Suzanne C. Fuchs, DPM, a board-certified podiatrist in Palm Beach Gardens, Florida, stinky feet “can be worsened by the type of shoes you wear, poor hygiene, or other foot issues such as fungal infections.” The upside to all of this is there are ways to make your soles smell better.

PODIATRISTS AND FOOTWEAR
Limit Wearing of Holiday High Heels: MI Podiatrist

Winter is here and the desire to look good during holiday events sometimes clashes with the pain of wearing high-heeled shoes. High heeled shoes may make your legs look sexy, but prolonged wear can lead to many disabling deformities, ranging from low back pain to foot pain. Wearing high heels can lead to shortening and contracture of the large Achilles tendon and calf muscles in the back of the leg. Women who habitually wear high heels usually develop the classic aching muscles and Achilles tendonitis, especially when there are shifts to lower heeled shoes.

Dr. Jodie Sengstock

“Many high heeled shoes for women also have pointed toe boxes,” said Dr. Jodie Sengstock, director of professional relations for the Michigan Podiatric Medical Association. “The narrow toe box squeezes the toes and the heel height causes a shift in the body column that must be compensated for by the foot. These issues generally cause aches and soreness of the feet and back, and can often result in sprains or other complications.”

MEDICAL EDUCAYION
Visits to Pre-Health Advising Correlate to Higher Odds of Medical School Admission

Pre-Health Advising, a part of the University’s Career Center, holds weekly advising hours for pre-health students who are in need of advice either on coursework, pre-health application processes, or pre-health professions. With only one year of data, it has been found that the students who met with pre-health advisors had a higher success rate for admission than students who never attended advising hours.

Kim Sauerwein, director of Pre-Health and Law Advising, said that Pre-Health Advising supports students interested in nine different pre-health professions — dentistry, medicine, occupational therapy, optometry, pharmacy, physical therapy, physician assistant, podiatry, and veterinary medicine. According to data provided by Sauerwein, the national acceptance rate to allopathic medical schools — medical schools offering an M.D. — was 42 percent in the 2016-17 cycle while U.Va. applicants had an acceptance rate of 60 percent in the same cycle.

CA Podiatrist Publishes Article on Diabetic Sock and Foot Monitoring System

Siren, the health technology company that developed Neurofabric™, a machine-washable, machine-dryable smart textile with built-in sensors, has announced publication of a foundational paper supporting its approach in the Journal of Medical Internet Research (JMIR), the leading peer-reviewed journal for digital medicine, and health and healthcare in the Internet age.

Dr. Alexander Reyzelman

In the paper, a team of international researchers led by Ran Ma, co-founder and CEO, and Alexander M Reyzelman, DPM, Samuel Merritt University, detail the role of Siren’s Diabetic Sock and Foot Monitoring System in maintaining continuous, wireless skin temperature monitoring for users at home, demonstrating the potential for the reduction of foot ulceration for diabetic patients.

PRACTICE MANAGEMENT TIP OF THE DAY
Tips to Help Physicians Protect Against Embezzlement - Part 1

Embezzlement adds up to $25 billion in annual losses for medical practices across the country every year, and – surprise! – it usually has nothing to do with complicated schemes from outsiders. More often than not, the culprit can be found within the victimized office. Here are some ways you can make sure your practice is secure from embezzlement.

1. Let your employees know you take it seriously - Establishing a zero-tolerance policy for embezzlement and fraud is a great deterrent to anyone who thinks they might be able to catch you “sleeping on the job.” Be transparent with your employees about the measures you’ve taken to prevent embezzlement, as well as the consequences for failure to adhere to any guidelines you’ve set. If you also prepare all financial policies and procedures in writing, everyone will be accountable for understanding the rules.

CODINGLINE CORNER
Query: Special Coding for Patients on Chemo

Is there a special way to code for patients who are undergoing chemotherapy? I assume it affects their immune system. I’m asking for coding information for a patient on chemotherapy for metastatic breast cancer that has spread to her ribs.

PM News Subscriber

Response: For the patient described in the post, the primary diagnosis would be: C50 Malignant neoplasm of breast. This would be extended to 6 digits or characters based upon the specific location of the malignant neoplasm within the breast. The secondary diagnosis would be: C79.51 Secondary malignant neoplasm of bone.

This would be indicative of the metastatic breast cancer that has spread to the ribs. The third diagnosis would be: T88.7XXS Unspecified adverse effect of drug or medicament, sequela.

RESPONSES/COMMENTS (-CLINICAL)
From: Jeffrey Kass, DPM

There have been many responses to date regarding diagnosing second MPJ pain/plantar plate tears. Dr. Kirby gave us excellent literature and cheap diagnostic tools by a simple but astute clinical exam. Dr. Meisler touted the usage of ultrasound. Others like MRI, and some want to inject contrast dye. All are acceptable methods.

From a practice management standpoint, many insurance companies don’t pay for ultrasound in NY, as “this service isn’t payable by this type of provider”. I was wondering if the situation is the same in other states, the legality of that line, and how colleagues handle that response from insurance companies. Do you charge patients? Refer to a radiology center? Stick with a clinical exam? Skip to MRI?

Jeffrey Kass, DPM, Forest Hills, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Connie Lee Bills, DPM, Estelle Albright, DPM

The VAMC buys a lot of shoes from Dr. Comfort to supply veterans in need of shoes. Call them and ask if they could send some complimentary shoe horns. As an alternative, you could contact the O&P office of your VA and they probably have a ton of them available. Dr. Comfort includes a free (long handle) shoe horn with every pair.

Connie Lee Bills, DPM, Mount Pleasant, MI

Dollar Tree stores sell nice very long shoe horns for $1 each. I buy these for my patients and they really appreciate them. You can also buy in bulk on DollarTree.com.

Estelle Albright, DPM, Indianapolis, IN

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Elliot Udell, DPM

On one hand, Dr. Docks is correct. We need to bring in professional epidemiologists to determine whether podiatrists have a greater risk for developing cancer and if the answer is yes, what is the causative factor. But just knowing podiatrists who developed a form of pathology does not in itself mean anything. It could be coincidental or it may not be. In recent years, I have known six people who developed pancreatic cancer. Only one is a podiatrist. Were their tumors caused by some carcinogen? I'd love to find out. Were they related by profession? No.

Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3
From: Allen Jacobs, DPM

This was predictable. I have been watching this evolve and for one am not surprised. Study after study has demonstrated an inverse relationship between access to foot care and amputation rates in the patient with diabetes. The relationship between podiatry and interventional cardiology and vascular surgery has grown exponentially. Wound care centers have populated cities like hamburger franchises. Wound care products are multitudinous. Wound care books, seminars, certifications, journals, are everywhere.

Why then the increased amputation rates reported in...
