Query: Place of Service Determinations
Who determines the place of service code for
various facilities? Does the physician/staff
make that determination based on the place of
service descriptions, or is it determined by the
facility in question? Are there particular
guidelines to assist in making the
determination?
In the case of an assisted living facility (POS
13), which CPT codes apply, those for a
domiciliary, rest home, custodial care, or home?

Codingline Response; The domiciliary, rest home
and custodial care facilities are reported as
place of service (POS) 13, and are assisted
living facilities. Assisted living facilities
may also be called adult living facilities, and
are characterized by provision of room, board,
and other personal services.
A group home, however, is designated as POS 14.
Both POS 13 and 14 utilize E/M codes, CPT 99321 -
CPT 99333.
Home services are defined as a private home, an
apartment town home, or other non-
congregate/shared facility living arrangement,
and its E/M service codes are CPT 99341 - CPT
99350.
