Intractable Plantar Keratosis

Query: Intractable Plantar Keratosis

What would be the appropriate diagnosis code for
an "intractable plantar keratosis" - ICD-9
757.39 (other specified anomalies of the skin;
keratoma [congenital]) or ICD-9 701.1
(keratoderma [acquired])?

Codingline Response: When podiatrists use the
term "Intractable Plantar Keratoma" (IPK), what
is usually referred to is a discrete
hyperkeratosis under a pressure point rather
than a congenital condition. Thus, the proper
diagnosis code would be ICD-9 701.1
(keratoderma, acquired) such as acquired:
ichthyosis, keratoderma palmaris et plantaris;
elastosis perforans serpiginosa; hyperkeratosis:
NOS, follicularis in cutem penetrans,
palmoplantaris climacterica; keratoderma:
climactericum, tylodes, progressive; keratosis
(blennorrhagica)".
