NPDB Personal Accounts Are Now Available for Healthcare Professionals

On May 8, 2026, the National Practitioner Data
Bank (NPDB) launched personal accounts for health
care professionals. Personal accounts provide a
centralized, secure way to access NPDB Self-Query
and Report services from a single sign-in page.

What Is an NPDB Personal Account?
A free personal account allows health care
professionals to:
• Search the NPDB for reports that match the
information they provide and view the results
• Review and respond to NPDB reports, if
applicable
• Manage their personal and contact information
• Get notified by email and text message if any
future reports are submitted to the NPDB
Health care professionals can create a personal
account and complete their NPDB profile to get
started. Once the profile is completed, the NPDB
will search for matching reports and display the
results at no cost.

Subscription for Certified Self-Query Responses
and Report Disclosure History
For a $3 subscription health care professionals
can:
• Get unlimited access to a current certified
Self-Query response PDF
• Access, download, and save Self-Query responses
at any time during the subscription period
• View a report disclosure history that shows
which entities have received their report(s), if
any

Review your past orders
• Healthcare professionals can also:
• Review previous Self-Query orders
• Track order status
• Access historical Self-Query responses
Prior Self-Query responses reflect the information
available at the time the search was completed.

Please Update Your NPDB Self-Query Instructions
Review and update any Self-Query instructions to
direct applicants to the NPDB website to establish
a personal account and generate a Self-Query PDF
response. Replace your current instructions with
the following:
• Visit the National Practitioner Data Bank (NPDB)
to create a personal account then complete a Self-
Query.

The text "complete a Self-Query" is linked to the
NPDB's Self-Query Basics page. This is a permanent
page address. It is regularly refreshed with the
most recent Self-Query instructions and fees.
Organizations are encouraged to accept the
digitally certified Self-Query PDF file in lieu of
a paper copy.
