RE: Pruritis in Well-Controlled Diabetic (John Spina, DPM)
From: Multiple Respondents It doesn't matter how well-controlled her diabetes is now, but what has been her control in the past ? Years ago there was a saying in the ADA, "what is happening to your body today depends on how you took care of it 10 years ago. .How long has she had diabetes? Type 1 or Type 2? Hgb A1C? I've personally experienced a similar condition, and have come to the conclusion that it is a type of neuropathy. Consider a peripheral neuropathy, such as DM or spinal, tarsal tunnel. Maybe consider EMG/NCV, Lyrica, Capsaicin. Not all that itches is necessarily due to a dermatitis.

There is a good chance her pain can be originating from her lower back. A lumbar radiculopathy from a herniated disc can cause similar symptoms. See if she has a history of lower back pain or Sciatica.

From: Multiple Respondents I worked for Charlie Turchin, DPM, in 1970, the year he was President of the APMA. He was the podiatrist for many well-known political figures in the nation's capital, including President Lyndon B. Johnson and his family. I had met Charlie in 1963. I visited him many times during the years leading up to my employment as his associate. When President Johnson needed care, he would send a car for Charlie. The story goes that one day, President Johnson summoned Charlie to drop everything and come to the White House. He sent a secret service detail to pick Charlie up.
Charlie was concerned when he walked into the Oval Office because Johnson had his three chief physicians waiting there. The president was very free with his language in berating his band of physicians. You see, a new drug called Griseofulvin has just come on the market. President Johnson had a fungal nail challenge and Charlie gave him a prescription for it. Unbeknownst to Charlie at that time, the president had suffered (for years) with jock itch that his physicians were unable to clear up. Charlie cleared it up as a side-effect of the nail treatment. "I have the top doctors in the world taking care of me, but my podiatrist cleared up my worst problem", said the president. I wasn't aware of what Jack Valenti did for us in those days and perhaps it was as noted, but I do know that podiatry got into Medicare because a case of resistant "jock itch" was cleared up by one of our best and brightest, Charles R. As such, he often made house calls to the White House during the Johnson administration. Charlie was a remarkable fellow; Intelligent, gregarious, charming, and charismatic. Charlie treated not
only LBJ, but Lady Bird as well. I had the pleasure of meeting Charlie in 1979 (he passed away over 25 years ago). I still treat his brother. Charlie once told me the story of the inclusion of our profession in the Medicare program. Charlie was working on Johnson's feet while Johnson was discussing the Medicare bill with his aides. Johnson looked at Charlie, thought for a moment and then turned to his aides and asked if podiatry was included in the program. The aides responded that they did not know. Johnson then turned toward his feet and asked Charlie what he thought. Charlie told Johnson that he felt podiatry should definitely be included in the program. Johnson made it so. As a profession, we all owe an enormous debt to the great Dr. Charles Turchin. RE: Severe Pain After Sclerosing Alcohol Injection (Robert Lagman, DPM)
From: Steven H. Goldstein, DPM, Simon Young, DPM This can be a fairly common reaction after alcohol injection as I have personally experienced this situation. For some patients the alcohol can become quite irritating. I was unable to bear any weight on the ball of the foot for the first 24 hours and resorted to percocet for the pain. I can tell you that what has worked for me was to re-inject the area with some marcaine and fast acting steroid such as dexamethasone phosphate 4mg. 1/2ml. plus place the patient on a 4mg. Medrol dose pack in combination with the injection. Steven H. Ideally, with sclerosing you want to be as close to the nerve as possible.

I agree with what others have said about the abuse and overuse of these skin graft substitutes. The vast majority of wound patients do not need these to heal. I was approached by a rep from a very large and publicly-traded medical product company to start using these grafts on my patients. He told me a tale of their top performer, a DPM in New York who did a lot of nursing home and wound care work, who had used about $20 million of their product in the last year alone. When I related that seemed fraudulent to me, the rep suggested that an offshore account could be set up to hide the money so it would be untouchable in the event that any of the treatment was determined fraudulent. I advised him that I was not interested in such schemes.
A few months passed and a patient came to my office with non-healing diabetic foot ulcers. Another provider in town had applied a series of 4-5 grafts on weekly intervals, costing over $100,000! The wounds did not heal. I am amazed that CMS has not completely shut down all use of these grafts with this type of abuse occurring.

Insurances process claims based on what taxonomy code has been assigned to the provider during the credentialing process. If they are credentialed as a specialist with a particular insurance, then a specialist copay will apply to claims. If they have been mistakenly credentialed as a PCP/IM doc (it happens more than you would think), then a lesser co-pay (sometimes no co-pay) will apply.
I urge every DPM to make sure that they have been credentialed as a DPM, and not as a PCP/IM with all their insurances. If there is a particular insurance that is mistakenly assigning the wrong co-pays, contact them directly to correct this.

PODIATRISTS AND SPORTS MEDICINE
Podiatrists, Residents, and Students Volunteer at the Boston Marathon
The 129th running of the Boston Marathon took place yesterday under gorgeous weather conditions. This year’s marathon consisted of over 32,000 runners with thousands of medical volunteers. A group of 45 podiatrists participated at the finish line area treating the runners. The team was led by Jonathan Kaplan, DPM who has been volunteering at this event for over 35 years.

Scott Aronson, Tara Blitz, Carl Conui, Samuel Gorelik, Paul Heffernan, Robert Kelerman, Brandon Kelerman, Lori Lundberg, Paula Marella, Kirk Neustrom, John Pizzuto, Joel Segalman, David Sipala, Sandra Weakland and Walter Wolf. Students from both NYCPM, Temple University Podiatry, and residents from Cambridge Health Alliance, Mt Auburn Hospital and Yale New Haven Hospital also participated in treating the runners’ injuries.

RE: The Passing of Edward J. Fiffick, DPM
Edward J. “Bud” Fiffick, 91, of Osprey, Florida, formerly of Youngstown, Ohio, passed away peacefully on April 15, 2025, following a brief battle with congestive heart failure. He earned his degree from the Ohio College of Podiatric Medicine in 1959.

Bud practiced podiatric medicine in Boardman, Ohio, for 40 years and served as Chief of Podiatric Medicine and Surgery at both Western Reserve Care System and Tod Babies and Children’s Hospital for 16 years. Bud's commitment to education and the advancement of his field was recognized in 2007 with the honorary degree of Doctor of Humane Letters from Kent State University. He also established the Edward J. Fiffick, DPM & Family Anatomy Lab at Kent State University.

pathways, particularly in cases of tendinopathy or
enthesopathy.
5. Joint Stabilization and Function Restoration
As weakened connective tissues regain structural
integrity, joint mechanics improve, leading to:
o Decreased aberrant movement or microinstability
o Reduction in nociceptive input
o Functional improvement and decreased reliance on
analgesics or anti-inflammatory medications
