According to most insurance companies, ESWT is "experimental", which is their way of not having to cover a service. Having done hundreds of like treatments, both high and low voltage, I agree with your assessment. The other difficulty we have is that the unit has to be used in a hospital setting and brought in from another city. Because of these factors, I have been performing a procedure using Tenex ultrasonic probes. I have been doing this for chronic plantar fasciitis and Achilles tendinitis for the past 6 months and have been very pleased with the results, especially on the Achilles. In fact, some of the results have been amazing. I have no long-term patients yet, but hopefully the improvement seen in these patients will be permanent.
Brian Kiel, DPM, Memphis, TN
I request that Dr. Steinberg and Dr. Rubin publish their results of their 90% success rate.

Social Media in Medicine: Pearls, Pitfalls, and Finding Your Voice
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
1. Why It Matters Whether we like it or not, social media has become part of how patients, colleagues, and even future trainees understand our profession. Most of my personal social media is me promoting podiatry as a profession and trying to show parts of my life as a resident. It allows us to: Connect with patients and communities we might never meet otherwise. Collaborate across institutions, specialties, and even countries. Share knowledge in real time. Build a network of support during some of the hardest years of training. For many of us, it also becomes a space to tell our story—something medicine doesn’t always give us time to do in day-to-day life.

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION)- PART 1B
From: Sev Hrywnak, DPM, MD
College seniors increasingly rely on online resources to compare healthcare paths, focusing on cost, time-to-licensure, and scope of practice. When evaluating whether a profession provides a full license after education, the four common tracks are physician assistant (PA), nurse practitioner (NP), medical doctor (MD) and doctor of osteopathy (DO). NPs and PAs practice autonomously in many states and under supervision in other states. Cost considerations are critical. MD/DO programs typically require the longest training (pre-med, four years of medical school, residency), with high tuition, but potential lifetime earnings can be substantial. PA and NP programs are shorter and less expensive upfront, often resulting in lower debt and earlier entry into.. Editor's note: Dr. Hrywnak's extended-length letter appears here.

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION)- PART 1A
From: Rod Tomczak, DPM, MD, EdD
There is a third alternative. At Ohio State, we offer an independence study program for the first two years. Students are given assigned readings and at the end of that assignment, which is usually one of the subjects that is being taught in the classroom, students come in and take an exam on the computer. They do have to show up for anatomy and any other clinical lab required class. They can complete the first two years at whatever speed they need. They can take step one of the national boards when they feel ready.
It might be possible to get a student to graduate with an independent study in basic science and then clinical work in three years or less. No one says we have to start residency programs July 1; students can be placed in residency programs outside of the match. We can call it early match just like MDs do for urology.

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1C
From: Donald R Blum, DPM, JD
When I applied to podiatry college in 1973, the California College offered a 3- year program, 36 months; rather than a 4-year (4 nine-month school years).

RE: A 3-Year DPM Degree (Allen M. Jacobs, DPM)
PA and NP programs are shorter and less expensive upfront, often resulting in lower debt and earlier entry into the workforce but have the plenary license.
Podiatric education comes close but not close enough to wave the parity flag. A cited example would be a podiatric physician treating a diabetic sub- second metatarsal ulceration. The treatment stops at the debridement and wound care, yet the PA or NP can do the same but also adjust the patient's diabetic medication and blood pressure medication, all with less education then a podiatric physician has.
If one was to address the "enrollment crisis" for podiatry colleges, then establishing a curriculum with appropriate clinical rotations compatible to MD/DO/PA/NP programs would make the most sense.
Online comparisons should prioritize: curriculum length, tuition, average indebtedness, state licensure rules, scope of practice, and average starting salaries. For truly full licensure, MD/DO training remains the most straightforward path, but the combined online information landscape helps students weigh debt, time, and long-term career flexibility.
The current college generation is seeking the shortest path for a healthcare career with the lowest accumulated debt and a paycheck at the end of the work week.

PRACTICE MANAGEMENT TIP OF THE DAY
10 Ways to Modernize Patient Financial Engagement
A panel of revenue cycle leaders at the 2026 MGMA Summit Digital Conference laid out the playbook for digital patient billing — and the order of operations practice administrators should follow.
9. Capture a card on file at every time of service
Matt Chadwell, DPM, revenue cycle product manager at MEDITECH made the case that the time-of-service interaction is one of the highest-yield collection moments in the workflow, and most practices give it away. The wrong question at the front desk, he said, is "Would you like to pay your co-pay today, or should I bill you?" The right question is closer to: "I see we have a Visa ending in 1234 on file. Should I charge that for today's $40 co-pay?"
Beyond co-pays, a credit or debit card on file with explicit patient authorization can be used to bill the patient’s responsibility after claim adjudication, subject to the practice's stated terms. The administrative effect is significant: it converts the chase that follows a paper statement into a routine post-adjudication charge, and it removes one of the largest sources of accounts-receivable aging in most practices.

PODIATRIC PRODUCTS IN THE NEWS
Treace Medical Completes First Surgery with New HyperPlate XM Implant
Treace Medical Concepts, Inc. has announced the completion of the first surgical case using its HyperPlate XM Dynamic Compression Locking Implant, a new addition to the company's foot and ankle product portfolio. The implant combines FastPitch locking screws with titanium compression plates, and is designed for midfoot and hindfoot procedures including talonavicular, calcaneocuboid, naviculocuneiform, and tarsometatarsal fusion applications, as well as Evans calcaneal osteotomies.
| Dr. Paul Dayton |
Paul Dayton, DPM, of the Foot & Ankle Center of Iowa in Des Moines, performed the first case, which involved a talonavicular joint fusion. Dayton, a paid consultant of Treace, said the implant offered "a highly stable construct with an anatomic shape designed to promote fusion in these larger midfoot and hindfoot applications." He noted the GreatReleaseXM instrument made soft-tissue release "significantly more efficient."

FINANCIAL TIP OF THE DAY
7 Money Behaviors That Can Hold Back Financial Success
These everyday decisions that may be costing you more than you realize.
7A. Don't hold grudges against past decisions
Everyone has a money regret: a car you overpaid for, an investment you sold too soon, a credit card balance that lingered. Carrying that around doesn’t help the next choice. Give past you some grace and commit to one rule you’ll use going forward. Maybe it’s:
Get three quotes before any repair ‘Sleep on’ purchase ideas that will exceed $200 Automate retirement contributions on payday.
Then take one action this week to put that rule in place. Progress is the antidote to regret. You can also write your rule on a sticky note and put it where you make decisions whether on your laptop, in your wallet or inside your investing app’s notes.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query: AI and Pre-Authorization?
Are there any risks if I use an AI program to assist me with my pre-authorization notes? Will I be flagged? Can the insurance company know if I used an AI search engine? It only seems fair if they do it, why can't I?
Codinghelpline com Subscriber
Response from Codinghelpline.com: This has response has to be broken down into several parts:
1. AI programs can certainly assist with pre-authorization and help assess medical necessity. You would need to provide accurate details about the patient’s insurance plan and any plan-specific requirements. The recommendation should still be reviewed before submission to confirm it supports approval of the pre-authorization request.
2. I do not believe your request would automatically be flagged, provided you review and personalize it before submitting it. Avoid sending requests that read like generic or repetitive machine-generated language. Ideally, the AI system should improve over time based on your edits and refinements.
3. The insurance company likely would not know AI was used if the request is individualized and medically specific. However, repeatedly submitting nearly identical requests could eventually trigger additional scrutiny or follow-up questions regarding medical necessity and coverage justification.
