Runners Must Increase Mileage Gradually: MD Podiatrist
Podiatrist Dr. Lee Firestone says that runners, in trying to “get the miles in,” can overdo it. “The ratio of the long run — the weekend run — should never be more than half of the total mileage for the week,” Firestone says. This means that if your mileage goal for the week is 20, you should do no more than 10 on the weekend.

And in terms of upping the weekly distance and workload, that should be done over time. Firestone says: no more than a 10 percent increase in total distance per week. “The bones get stronger when we walk and run, as do muscles and tendons,” Firestone says. “But we have to let them adapt gradually.”

Congressional Sighting at OHFAMA Seminar
Congressmen need to maintain their professional license if they are physicians and surgeons. Congressman Dr. Brad Wenstrup is no exception. The Congressman, who is on Congressional break until after the election, recently had time to catch up on continuing education credits for his OH license. He attended the 2014 Quickie Seminar in Dayton, OH. The Quickie Seminar, which was hosted by the Midwest Podiatry Academy of the OH Foot and Ankle Medical Association (OHFAMA), is a local CME event. It hosted 65 attendees.

OHFAMA Immediate Past President and Midwest Academy President Congressman Wenstrup was a good sport to help promote OHFAMA’s upcoming Centennial Celebration during the seminar. The Annual OHFAMA Scientific Seminar June 4-6, 2015 in Columbus will be the kickoff celebration of OHFAMA’s centennial year. Academies are being asked to take candid photos to be used at centennial functions to promote 100 years of organized professional podiatry in Ohio.

TN Podiatrist Joins Murfreesboro Medical Clinic
Murfreesboro Medical Clinic has announced the addition of Andrew J. LaMay, DPM to the Podiatry department. Dr. LaMay is a Fellow of the American College of Foot and Ankle Surgeons. Dr. LaMay received his DPM degree from the University of Osteopathic Medicine and Health Sciences in 1990. He completed his podiatric surgical residency at Humana Hospital in Pembroke Pines, FL in 1991.

A New York native, Dr. LaMay moved to Middle Tennessee in 1991 where he opened private practice facilities. He currently serves on the Medical Executive Committee at Nashville Surgery Center and has participated in FDA trials for new joint prosthetics. Dr. LaMay is the current President of the Middle Tennessee Podiatric Medical Association.

MT Podiatrist Seeks Vacant Seat on State Supreme Court
Dr. W. David Herbert, a Billings attorney who also is a licensed podiatrist in Wyoming, has filed to run for justice of the Montana Supreme Court. Herbert, 70, filed this week for the seat now held by Justice Jim Rice, who is seeking re-election. “I’m very concerned about the future of our constitutional republic,” Herbert said. He is a believer in jury nullification, or as he calls it, jury independence. Under that theory, a jury can acquit a criminal defendant if the jurors disagree with the law or believe it shouldn’t be applied in that particular case.

Herbert lost races in Wyoming as a Libertarian Party candidate for the U.S. Senate in 1996 and for the U.S. House in 2008. He moved to Montana later in 2008. Herbert was licensed to practice law in California, but now is inactive there. He was admitted to practice law in Montana in October 2009. He also is licensed as a podiatrist in Wyoming, where he still has a practice.

PRESENT e-Learning Systems Help Unmatched Graduates Prep for Re-Application
In August 2013, over a year ago, when the residency shortage was headline news in podiatry, PRESENT e-Learning Systems extended an offer to all unmatched graduates for a free subscription to PRESENT Online Residency Education. It had been widely opined in PM News and other online forums that the best thing that unmatched students can do to find residency placement for themselves after going unmatched is to maintain/improve their skills and knowledge in podiatry and medicine. PRESENT’s hope was that viewing the same lectures and reading the same ezines as residents would help the unmatched graduates in that endeavor until they subsequently were successful in finding a good residency placement for themselves.
Since then, over 40 unmatched graduates have enrolled in the program. They have collectively viewed a total of 569 lectures, with each resident viewing up to 197 lectures. Their education outcome scores, comparing their performance on the lecture pre-tests to the lecture post-tests, averaged a 244% improvement! Based on these metrics, it seems that the program has helped these unmatched podiatry school graduates improve themselves professionally and prepare themselves well for their next application for residency placement.

Query: Ingrown Toenails in a Toddler
Treatment advice is needed for a 1.5 year old otherwise healthy male with B/L hallux ingrown toenails. I've never done a procedure on a patient this young, so I want to make sure I make the safest and most effective decision.

I am considering taking the patient to the O.R. to do permanent removal of both borders of the hallux nail B/L. However, I'm not sure of the best procedure - phenol versus surgical matrixectomy? Is the technique the same as with an adult patient? What type and amount of local anesthetic should I use? What's the post-op course, etc.? Also, if anyone has any conservative treatment options that have been helpful, then please share them.

Meaningful Use Core Measure #7 requires that you PROVIDE patients with the ability to view, download, and transmit their Health Information. This is actually a two-part measure. The first part requires that you PROVIDE access. In order to do this, you only need to set up access to the portal so that the patient may visit the portal should they wish. It does NOT make a difference if the patient actually visits the website to view, download, or transmit their information. You must set up access for over 50% of your patients. This is simple and an administrative task that can be assigned to the front desk.

To my mind, the real question is who owns the access to past medical records. This isn't to start a flame war over the advisability of online charting, free or otherwise, vs. office systems. Each has their own problems. This is merely to show that for each "solution" forced on us by a "benevolent" Big Brother, we find another problem. In this case, could the EHR company have allowed access to past records and blocked current entries?

RE: The Cost of Reconstructive Foot Surgery

Has anyone taken a real look at the cost of reconstructive surgery on feet? I wonder why our government is squealing about physican fees and doing its best to cut our fees when it costs $1,500 for a tiny titanium plate and $200 for a small screw. One would think that they would be going after the unrealistic costs of implantables or medication rather than our livelihood, which took years of our lives and hundreds of thousands of our dollars to achieve. The advances made in internal fixation over the last 25 years of practice afford my patients better results- there is no debate in my simple mind as to the advantage of most newer hardware.
The problem is the cost benefit ratio. I have yet to actually believe all the rhetoric given as excuses for the costs by these companies. I don’t understand why the government doesn’t force these suppliers to lower their fees just as it does with us. At this point, when I have a more complicated surgery, I have to ask an administrator what budget I have for the surgery so that I can choose the cost-appropriate hardware. Can I use a Steinman pin or a locking compression plate? It is no longer what hardware or device would be best for the patient. Something is just not right about that!
