Athletes Should Know Foot Type When Buying Footwear: IL Podiatrist

Remember when sneaker shopping was as simple as "Ooh, Mom, I want the pink ones!"? These days the color is the last of a dozen dizzying decisions you have to make—extra support? comfy cushioning? both?!—to pinpoint a perfect pair. Luckily, cutting through the confusion can be a cinch if you determine your foot type first. Not only will jogging in a shoe that fits properly feel easier, says Megan Leahy, DPM, a podiatric physician in Chicago, IL, but it will also reduce your risk for injury.

Dr. Megan Leahy

There are three main categories of running shoes: flat, neutral, or high arched, each designed for a specific type of arch. You can predict which group is the most foot-friendly for you.

INTERNATIONAL PODIATRY

SALSA and University of Cardiff's Transatlantic Healing Collaborative

The University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA), under the direction of Professor of Surgery David G. Armstrong, DPM, PhD, continued its goal of "spreading the SALSA" in its unique collaborative with the renowned Wound Healing Research Unit at the University of Cardiff, United Kingdom. The Collaborative, which has contributed to research and education, took on a new arm this week with Armstrong's lecture to Cardiff University's Masters of Science program in Tissue Repair and Wound Healing.

Dr. David Armstrong Lecturing from SALSA Univ. of Arizona to Cardiff, Wales

"As a graduate of that program, it is an absolute pleasure to work with them in this capacity." noted Armstrong. "Our Universities' collective Biocommunications units are absolutely world-class on both sides of the Atlantic. Telemedicine and telemedical education are now part and parcel of SALSA's mission." The MSc program, founded and run by Cardiff University's Professor Keith Harding, is the first and only Masters program of its type in the world.

PRACTICE MANAGEMENT TIP OF THE DAY

How to Handle a Bad Reference

Should you tell job seekers when their references make unfavorable comments about them? It depends:

Yes, tell them if you are still interested in hiring them. If the reference’s comments go against everything else you have learned about a candidate, briefly describe what you heard, and offer the person a chance to explain the other side of the situation.

No, keep it to yourself if a reference confirms your gut feeling about a candidate. After all, you would not want the candidate to feel that your decision was based entirely on the bad reference.

Caution: Treat references’ comments with confidentiality, just as you would every other piece of information you uncover during the hiring process.

QUERIES - (NON- CLINICAL)

Query: CR Processors

I am in the market for a CR processor. I am leaning toward Kodak POC models. I was wondering if anyone has one, and what their experiences have been. Also, are there any cons with the Scan-X units, that seem to be the most popular of these devices?

RESPONSES / COMMENTS (CLINICAL) - PART 1

RE: Post Alcohol Injection Complication (Bret Ribotsky, DPM)
From: Multiple Respondents

After 30 years of recommending and performing dilute 4% alcohol injections for the treatment of neuromas, I have never advocated mixing it with steroid. I think that this is unwise, for a variety of reasons. But, I have also never seen a serious complication of any kind in all of these years with the use of 4% ethyl alcohol injections. It would be very important to find out several things in this particular case, such as, what was the type and concentration of alcohol used, what was the type and concentration of the corticosteroid used, and how much volume and how many injections were done.

G. Dock Dockery, DPM, Seattle, WA, gdockdockery@aol.com

I have never seen anything close to this type of complication following alcohol injections since giving alcohol injections over the last 10 years. There have been prior discussions on using different concentrations of the alcohol mixture. What was the specific concentration of alcohol and cortisone? Someone injecting Kenalog 40 mixed with a 50% alcohol solution is a lot different than using a 4% solution mixed with Decadron.

Mathew M. John, DPM, Marietta, GA, footdoc@afcenters.com

I don't think we have enough information to draw the conclusion that the patient’s wounds can be entirely blamed on the alcohol injection. The picture indicates that this patient likely suffers from venous and arterial insufficiency. This is what I would choose to focus on.

RESPONSES / COMMENTS (CLINICAL) - PART 2

RE: Calcaneal Ostectomy  and Fat Pad (Chris Seuferling, DPM)
From: Ronald D. Worley, DPM, Greg Aposperis, DPM

This is a classic example of a patient for whom injectable fillers such as Sculptra, Jevaderm, etc. would help. Injecting these fillers for fat pad loss was pioneered by Suzanne Levine, DPM of NYC. The patient in question should have the partial ostectomy of the calcaneous first, followed by Sculptra injections to restore the lost padding. Fat transfers do not work very well due to the damage to the fat cells during the harvesting.

Ronald D. Worley, DPM, San Diego, CA, drrdworley@yahoo.com

The x-ray appears osteoporotic for a 70 year old.  If the pain is to the level that she is considering an amputation, and conservative measures have failed to be of ANY benefit, have you considered CRPS?

I would suggest eliminating any neurogenic cause for her pain, as further surgery would fail and may make matters worse. To my knowledge, autologenous fat transfers have failed in the long run.

Greg Aposperis, DPM, Santa Barbara, CA, santabarbarafootclinic@yahoo.com

RE: PSR-24 Discrimination (Name Withheld)

I would file an ethics complaint with your state podiatry society and the APMA against those DPMs on staff you feel are responsible for requiring a 3-year program. Of course, they are already grandfathered in, and have nothing to worry about, except protecting their turf.
