IA Podiatrist Discusses Plantar Fasciitis
Plantar fasciitis is the most common complaint podiatrists see in the office, says Dr. Mindi Feilmeier, a podiatrist with the Foot and Ankle Clinic at Des Moines University. The plantar fascia is a big ligament on the bottom of the foot that runs from the heel through the arch and up into the forefoot. “Its main job is to really help support the arch and the foot when people are standing and walking,” said Feilmeier, who is also an assistant professor in DMU’s College of Podiatric Medicine and Surgery.

It’s usually a combination of factors that lead to plantar fasciitis, which occurs when the ligament becomes stretched and pulled, resulting in “micro tears” in the plantar fascia, she said. Feilmeier encourages seeing a podiatrist, who can diagnose plantar fasciitis with a thorough physical examination involving a biomechanics exam of the foot and leg, and watching a patient walk. An x-ray may also be taken to rule out something more serious, like a stress fracture, and to provide an overall view of the foot, Feilmeier said.

Barefoot and Minimalist Running are Wrecking Feet: NY Podiatrist
The recent trend toward minimalist running shoes or barefoot running can be problematic for those who've been running in supportive shoes for many years, says Dr. Sherri Greene, a New York podiatrist. "Since most people don't train to strengthen their feet, I find many have very weak muscles and tendons. And it's not a good idea if you have biomechanically-challenged feet (e.g. flat feet) or neuropathy."

Greene recommends wearing supportive shoes or, if you're hooked on the idea of barefoot running, train to strengthen your feet where they may tolerate a minimalist shoe.

Wrong Size Shoes Can Exacerbate Bunions: MA Podiatrist
Bunions can be made worse by the wrong shoe, or by carrying extra weight, or prolonged periods of standing or walking. Dr. James P. Ioli, chief of podiatry at Brigham and Women’s Hospital in Boston, said wearing the correct shoe size is a good first step in preventing bunions. It’s important to make sure that your toes have enough room. Don’t wear shoes that jam the toes together. High heels are also particularly bad because they move the body’s weight forward and change the mechanics of the foot.

Even if you think you know your shoe size, Dr. Ioli said, the size of your foot can change over time. In women who have given birth, the foot ligaments often relax, causing the foot to get longer and wider. Unfortunately, there are not really any effective non-surgical options to reduce the size or reverse the development of a bunion, said Dr. Ioli. But non-surgical treatments can reduce the pain or discomfort from a bunion and keep it from worsening.

Apple Rolls Out the Health-Enabled Apple Watch
In a much-hyped event Tuesday, Apple rolled out its new smartwatch that's able to collect health data and share it with providers through the tech giant's HealthKit app, which has drawn interest from major healthcare players such as the Mayo Clinic and Memorial Sloan-Kettering.

The device will be able to track medical and fitness data, including pulse rate, distance, and an accelerometer. That ability to track data will pair with its new HealthKit app, which will debut with the new iOS operating system. The app can aggregate health data from various wearable devices, and store and send them to providers or other entities as the consumer desires.

Query: CMS Flexibility in Meeting Meaningful Use
I am in my 4th year of reporting Meaningful Use, and I am scheduled to report Stage 2 this year(2014). I use Practice Fusion as my EHR. It is 2014 certified. I was under the impression (according to various sources) that I have the option of reporting Stage 1 this year under the new CMS final rule if I attest that I have not been able to fully implement Stage 2 into my practice work flow despite having 2014 certified technology. Is this still true under the final CMS rule? Is this rule officially effective yet?

Response: The rule has been finalized. You are correct about being able to report Stage 1 2014 objectives and quality measures (there are also other options using 2011 CEHRT or a combination of the two) if you can attest to not being able to fully implement the 2014 CEHRT.
However, and this is the key, the inablility to fully implement the 2014 CEHRT must be related to delays in availability of the software. If you are going to attest to this, I recommend that you have something in writing from Practice Fusion indicating that there were delays in making the 2014 CEHRT available to their subscribers. Otherwise, if you are audited and cannot document that there were delays in availability, you could have problems. The entire final rule is posted on the CMS website and does cite some examples as to what constitutes inability to fully implement 2014 CEHRT.

This would be a great case for MIS surgical correction. The first metatarsal angle would lend itself well to a Silver minimally invasive surgical bunionectomy; preferably, combining this with a minimally invasive Akin osteotomy. For the hammertoe corrections, she would do well with MIS correction as well. For treating the metatarsalgia, I recommend an endoscopic gastrocnemius recession if the patient has equinus.
All of these procedures can be performed with percutaneous incisions. I do not recommend performing the Keller procedure due to the prospect of exacerbating the metatarsalgia.

I'm not sure we're looking at the same x-ray, but there is an exostosis projecting dorsally, which is causing the nail problem. So the phalanx must be addressed to give the patient a chance for a healthier nail bed and plate.
Your patient might consider a permanent total nail removal. It would certainly look less obvious than her current nail. I did that quite a few times for patients, and they generally agreed that it was at least a cosmetic improvement over their severely dystrophic nail. I had more than one female patient tell me that they painted the nail bed to match the other nails' polish and no one was the wiser. My sister-in-law came up with a twist on that. Each summer, she glues an artificial nail on the bed and paints that. Unless you look really closely, it's undetectable. For the rest of the year while she's in closed shoes, she doesn't bother.

I have been experiencing similar issues to Dr. Borreggine's more recently, and I have been in practice for more than 30 years. I believe that our patients are becoming more non-compliant with ambulation, causing more swelling. We lead busier lives, and despite all of our instructions, patients tend to maintain as much of their pre-surgical activity level as they can. I believe your issues are not dressing-related.
