TX Podiatrist Discusses How to Buy Athletic Shoes

If podiatrist Dr. Danny Wheat had his preference, more of us would be wearing athletic shoes. Wheat has a few things he thinks people should look for when they buy athletic shoes. "The heel counter should be running vertical to the ground," he said. "And the heel cup should give you support. If you push down on it, you shouldn't be able to crush it all the way to the sole."

Wheat added that the sole should bend at the ball of the foot and not at the arch. Also, because most of us have an inward curvature of our foot at the small toe, our shoes should have an in-toed or a straight last, which refers to the shape of the shoe. "If you get an out-toed last, you're going to be fighting that shoe all the time," he said.

Samantha Bark, DPM, will join the medical staff of Avita Health System's Orthopedic Center. She is a fellowship-trained podiatric foot and ankle surgeon. She will care for patients with a variety of foot and ankle conditions, varying from common conditions to complex reconstructive cases.

Query: Scope of Practice

I work for a large multispecialty clinic. We have recently implemented our EMR system. This system was built for family practice/internal medicine and has been revised for the specialties. In this system, there is a “best practices” section that comes up if a patient is not at ideal BMI. Is it out of scope of practice for a podiatrist to ask questions about BMI, and then provide counseling?

To my colleagues who are using Amazing Charts (AC) EMR , has there been any addition to the template list that is specific for podiatry? Research - The primary purpose of this fellowship is to provide the Podiatric Surgeon who has completed a minimum of a three year residency, and who is committed to a part-time/full-time academic career in Podiatry, further education on research of the Diabetic Foot. The fellow is expected to complete several clinical or basic research projects during the term. This fellowship is a two-year experience during which the fellow will function as an Instructor / Clinical.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as an Instructor / Clinical and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. I am curious as to where my colleagues order their surgical shoes. I routinely use plain flat surgical shoes as well as the ortho-wedge style surgical shoes, which seem to be much more expensive than the flat ones.

I apply the "keep it simple" philosophy with Charcot joint surgery. Most of the time, a TAL/gastroc lengthening with an aggressive planning of the prominent bone(s) works well. There's no reason to remove the entire cuboid. Just plane the bone down with osteotomes and rasps until it's no longer sticking out. I like to use my fingers to feel the bones rather than use a C-arm to visulaize it. I typically use a lateral incision along the CC joint.

First, it appears that there may be a posterior tibial fracture on the lateral x-ray (Incidental finding or rotated fibula?).
 
On the x-rays presented, the “marker” appears under the cuboid prominence and maybe the 5th metabase on the lateral view. The “AP” view does not show this, but the view or the foot is skewed.
 
Removal of the cuboid would undoubtedly destabilize the...

RE: Verrucous Carcinoma (Elliott Udell, DPM)

I do not know what the literature states regarding Dr. Udell's specific question, but what I know is from what I learned as a student, as a resident, and as a practitioner for the last 22 years. A wart is a local low-grade lesion that is confined to the dermis. If left untreated, it will follow a "chronic course" (as stated in the literature). Usually one has to wait up to 12-15 years (if I recall correctly) to be able to make a diagnosis of verrucous carcinoma, but these lesions "can" transform themselves into a carcinoma. I remember as a resident doing a BKA, and while in private practice, amputating part of another patient's foot. I don't  think either of them cared what the literature said.

I would like to clarify the previous post that the diagnosis of verruca vulgaris has been confirmed by shave biopsy. The lesion is painful and there is no hyperhydrosis present. I have treated him for less than 6 months without much improvement. My medical supplier does not carry Canthranone. I will try to convince the patient to take some time off and proceed to either laser or surgical excision. Thanks for all the replies and comments.
