NYCPM/ISMMS Graduates Its First DPM/MPH
Dr. Michael Gazes has become the first student to graduate from the Icahn School of Medicine at Mount Sinai's (ISMMS) newly affiliated dual DPM/MPH degree program with the New York College of Podiatric Medicine (NYCPM). He successfully completed the MPH portion of his education in September 2013 and graduated with a Doctorate in Podiatric Medicine this May. "I liked the idea of working towards both degrees simultaneously, but it took extreme organization and a lot of strict scheduling for me to complete the coursework to my satisfaction. Classes at the NYCPM typically ran from 8am to 4pm, and then I would go directly to Mount Sinai for evening classes until 6 or 7 pm several times a week," said Gazes.

"For my practicum, I assisted in establishing a research database for the Foot Center of New York. This required getting an IRB approval and then examining the details of nearly 1,000 patient charts," said Gazes. He matched into a podiatric medicine and surgery residency with an added credential in Reconstructive Rearfoot and Ankle Surgery at Yale – New Haven Hospital. He’ll be starting in July 2014.

NJ Podiatrist Active in Original Research on Gas Gangrene
As an associate doctor of podiatry at New Jersey Foot and Ankle Centers, Maryellen Brucato, DPM of Oradell, is making inroads in the field. Her original research was published in the Journal of Foot and Ankle Surgery. The Infectious Disease Society of America has defined gas gangrene as an infection caused by Clostridium species. However, according to Brucato’s findings, rarely are the gas-forming soft tissue infections of the lower extremity, known as gas gangrene, actually Clostridium. She presented her research at the American College of Foot and Ankle Surgeons Annual Scientific Meeting in Orlando, FL.

Dr. Brucato remains active academically and also holds an attending position at Morristown Medical Center where she is able to teach the residents clinically as well as academically. She also serves as the clinical research coordinator for the podiatric surgery residency program there.

TX Podiatrist Promotes Proper Use of Semmes-Weinstein Test to Detect DPN
Lions Club members were at the Athens law office of Stark and Groom on Wednesday to demonstrate a test to reveal the presence of diabetic peripheral neuropathy. Lions Clubs are partnering with the not-for-profit Lower Extremity Amputation Prevention Alliance to detect DPN. “If a person can feel 10 grams of force, then they are said to have protective sensation,” podiatrist Dr. Dwight Bates said. “If a person cannot feel it, they are at-risk of a wound in the skin getting infected.”

Bates said the simple Semmes-Weinstein test can greatly reduce the grim consequences of DPN, but only if people are trained to use it effectively. “Just knowing you’re at-risk is not enough,” Bates said. “You’ve got to take action. Just having a diabetic ulcer is a 50-percent chance of dying within five years (worse than most malignancies),” Bates said.

They were once the lowly 99-cent beach sandal at the corner drug store. But today, flip-flops have been elevated to a fashion mainstay for millions of wearers. While they are fun and cheap, they also can be a pain — literally.

"I see a lot of patients complaining of foot pain from wearing flip-flops constantly," says podiatrist Dr. Lori Steiner. "They suffer from overuse injuries of the foot, causing pain that extends from the heel and arch, and often all the way to the back and hip. And I’m certainly not alone on this; the American College of Foot and Ankle Surgeons blame regular wearing of flip-flops for an increase in heel pain in patients 15 to 25 years old, a group that normally doesn’t have this issue."

Query: Hallux Surgery for Paraplegic
A 52 year old white female paraplegic (due to MVA) presented with contracted plantar aspect of feet S/P skin graft due to a 3rd degree burn. She complains that when she stands erect transferring to and from her wheelchair, her right first toe curls under, causing pain, instability and, at times, ulcerations at the tip of the right hallux. She claims that she has loss of sensation to her feet, except for the right hallux.

Query: Legality of Advertising on Groupon
What are the legalities with selling services on Groupon? I see podiatry-related services often. I was under the assumption that giving away free services or reduced fee services was illegal. I also thought a disclaimer had to be put on all advertising like this that insurance was not going to be billed for "free or reduced fee services."

Editor's comment: PM News does not provide legal advice. The legality of advertising on Groupon varies from state to state. States such as New York and New Jersey do not allow fee-splitting, and thus advertising on Groupon would subject the provider to professional misconduct charges. Whether or not a disclaimer is required also varies from state to state. Thus, one should contact his/her state podiatry society or a healthcare attorney for more specifics.

In response to the use of arthroeresis only at hospitals, we too here in California are having a problem with reimbursement. Our office and local surgery center have had to refund tens of thousands of dollars after Blue Cross audited those claims. They still state it is "investigational" and will not cover the procedure. We now have patients pay cash for the implant and at the surgery center.

Dr. Benjamin asked if he was missing something. I think it is very clear that he is missing a lot. I applaud Dr. Rogers for his repeat effort. It takes a lot of courage to run for public office these days, as there are a lot of Dr. Benjamins out there. It also takes a lot of Benjamins.

I have been involved in local state politics in my home state of Maryland for over 35 years. Dr. Benjamin needs to know more about how Congress actually works. Initially, new Congress members get placed on committees that they might not desire, BUT as they learn the game and network, they can position themselves for more powerful and desired spots. ANY podiatrist in Congress can help to educate fellow lawmakers about the role and need for podiatric care in America. Podiatry was included in Medicare in part due to the efforts of Washington, DC podiatrist Charles Turchin, DPM who treated the family of Lyndon Johnson.
In Maryland, we have benefited from the niece of the late Stanley Kutz, DPM, Sen. Barbara Mikulski, a very senior and powerful legislator. ANY inroads we can make by any political means will pay off down the road. To criticize novice Congressmen for not jamming through major legislation shows that one does not know how the game is played. Sometimes, our lobbyists take years to convince legislators of the merits of proposed legislation. Hats off to Dr. Rogers for his effort.

I appreciate the comments of my colleagues Drs. Borreggine and Dr. Steinberg. The Illinois Podiatric Medical Association (IPMA) has been working diligently to restore access to podiatric care for Medicaid beneficiaries in Illinois since 2012. Our services were eliminated due to the passage of the Illinois Save Medicaid Resources Together (SMART) Act. This occurred due to the fact that we are not a mandated service as defined under Title XIX. The act initially eliminated podiatric care to the public relying on Medicaid for their insurance needs.
